Hang Fire - Acute Crush Injury of Fingers - Till Appropriate Intercede - A Case Report.

INTRODUCTION: Pediatric crush injuries of the hand may present with fractures and threatened digital vascularity, requiring careful differentiation between venous congestion and arterial ischemia. CASE REPORT: A 6-year-old boy sustained a crush injury to the left hand with phalangeal fractures and threatened vascularity of the middle finger. The patient was managed conservatively with immobilization, limb elevation, hydration, anticoagulation, thermal support, and serial vascular monitoring. Progressive recovery of perfusion and viability was achieved without surgical intervention. CONCLUSIO

Salvage of Circumferential Multiplane Degloving Injury of Hand with Single Viable Finger - Case Report.

INTRODUCTION: Circumferential multiplanar degloving injuries of hand and wrist are devastating injuries which can severely compromise what is salvageable for achieving a functional hand, ending in amputations if not well managed. CASE REPORT: We described a complete multiplanar degloving injury of hand in a 27-year-old Asian male due to a compressing machine, which was successfully salvaged with a complete free latissimus dorsi flap cover with skin graft for coverage. CONCLUSION: An unconventional one-stage surgical option for soft-tissue coverage using a complete latissimus dorsi free flap

Closed Traumatic Zone 2 Flexor Tendon Injury of the Long Finger - A Case Report.

Closed rupture of the flexor digitorum profundus (FDP) tendon causes loss of flexion at the distal interphalangeal joint. Following trauma, these are known to present as avulsion fractures (Jersey finger) commonly in ring fingers. Traumatic tendon ruptures at the other flexor zones are seldom noted and are often missed. In this report, we present a rare case of closed traumatic tendon rupture of the long finger FDP at zone 2. Though it was missed initially, was confirmed with Magnetic Resonance Imaging and underwent successful reconstruction using an ipsilateral palmaris longus graft. Level of

Oligodendroglial ring finger protein Rnf43 is an essential injury-specific regulator of oligodendrocyte maturation.

Oligodendrocyte (OL) maturation arrest in human white matter injury contributes significantly to the failure of endogenous remyelination in multiple sclerosis (MS) and newborn brain injuries such as hypoxic ischemic encephalopathy (HIE) that cause cerebral palsy. In this study, we identify an oligodendroglial-intrinsic factor that controls OL maturation specifically in the setting of injury. We find a requirement for the ring finger protein Rnf43 not in normal development but in neonatal hypoxic injury and remyelination in the adult mammalian CNS. Rnf43, but not the related Znrf3, is potently

The Association of Finger Growth Plate Injury History and Speed Climbing in Youth Competition Climbers.

INTRODUCTION: Finger growth plate injuries are the most common injury among youth climbers, and the association between these injuries and speed climbing, a mandatory discipline in the 2021 Olympics, has not been examined previously. Our primary purpose was to examine the demographic and training characteristics of adolescent competition climbers who reported a history of a finger growth plate injury compared to those who did not report a history of a finger growth plate injury. Our secondary purpose was to determine whether training characteristics differed between adolescent competition clim

Combined dislocation of the trapezoid and finger carpometacarpal joints-the steering wheel injury: case report.

Two cases of combined dorsal trapezoid dislocation and multiple finger carpometacarpal joint dislocations are described. In both cases, a common mechanism involved a head-on motor vehicle collision while the subjects held tightly to the steering wheel. The patients were treated with open reduction and pinning, with good short-term results.

The acute effects of mild traumatic brain injury on finger tapping with and without word repetition.

This study aimed to investigate the acute effects of mild Traumatic Brain Injury (mTBI) on the performance of a finger tapping and word repetition dual task in order to determine working memory impairment in mTBI. Sixty-four (50 male, 14 female) right-handed cases of mTBI and 26 (18 male and 8 female) right-handed cases of orthopaedic injuries were tested within 24 hours of injury. Patients with mTBI completed fewer correct taps in 10 seconds than patients with orthopaedic injuries, and female mTBI cases repeated fewer words. The size of the dual task decrement did not vary between groups. Whe

Transposed replantation of fingers at forearm bones in severe segmental injuries across the hand and wrist.

There are situations in which amputated hands or fingers cannot be replanted directly back to their original positions. When there is severe segmental injury across the hand and wrist but one or several fingers are still healthy, the fingers can be selected to be replanted at the forearm bones to restore pinch function. This is different from the toe to antebrachial stump transplantation presented by Dr. Vilkki. From the results in four patients, the following conclusions are drawn: (1) When the fingers are replanted at the forearm bone(s), only pinch function can be obtained. There is no oppo

Overuse Injury to Proximal Phalanx in Throwing Athlete (Little Leaguer's Finger): A Case Report.

The young, overhead throwing athlete is prone to overuse injuries because of their variable skeletal maturity and often improper technique. An overuse injury to the digit(s) in the overhead throwing athlete, Little Leaguer's Finger, has never been discussed in the literature to our knowledge. We present a case of a 14-year-old man with atraumatic pain, swelling, and edema to his dominant pitching index finger after throwing greater than 90 pitches in one setting. Initial workup of the patient, including an MRI, ruled out an infectious process, and it was determined that there was a stress epip

[Postoperative care of a retrograde cuff avulsion injury of finger skin treated by microsurgical reimplantation].

Sheath avulsion of finger skin is a serious acute occupational hand injury. Most of them are mechanical injuries, which are difficult to deal with clinically, have many complications, and can lead to serious hand deformities and dysfunction. Microsurgical replantation is the main treatment for finger skin retrograde sleeve avulsion injury, but postoperative fine nursing and scientific personalized vocational rehabilitation are the key to affect the surgical effect and patients' return to society and work. In this paper, 16 patients with retrograde sheath avulsion of finger skin were followed u

Domestic Horse Bite: An Unusual Etiology of Crush Injury of the Fourth Finger-How to Manage?

Almost 2% of all emergency admissions involve an animal bite. While horses bite humans very rarely, their bites are mostly associated with fatalities. Herein, we report the case of a 23-year old bitten by a domestic horse causing a crush injury to his fourth finger with fracture dislocation of the proximal interphalangeal joint. The patient benefited upon arrival at the emergency department from copious irrigation with saline serum, tetanus toxoid, postexposure rabies vaccination, and prophylactic antibiotic therapy. In the operating room, surgical exploration found the ulnar digital pedicle s

Ultrasound evaluation of stress injuries and physiological adaptations in the fingers of adolescent competitive rock climbers.

BACKGROUND: The impact of high-intensity, repetitive training on the fingers of adolescent climbers is relatively unknown. OBJECTIVE: To evaluate this effect by ultrasound (US) and to confirm some findings by magnetic resonance imaging (MRI). MATERIALS AND METHODS: The US study was performed in 20 adolescent rock climbers (ages 10-17 years) and 6 non-climbing controls (ages 11-15 years). US was used to examine the third digit of the right hand for differences in thickness of soft tissue, flexor and extensor tendon, volar plate and bony and growth plate adaptations. In four climbers with fing

Finger and Toe Temperature Responses to Cold After Freezing Cold Injury in Elite Alpinists.

OBJECTIVE: To assess whether previous freezing cold injuries (FCI) would affect digit skin temperatures and rewarming rates during a follow-up cold stress test protocol. DESIGN: Nonrandomized control trial. METHODS: Twenty elite alpinists participated; alpinists with previous FCI requiring digit amputations (injured, INJ: n = 10 total, n = 8 male) were compared with ability-matched, uninjured alpinists (control, CON: n = 10, all male). Digit skin temperature was measured using infrared thermography as an index of peripheral digit perfusion after a cold stress test, which consisted of 30 minu
