SPARC: a new approach to quantifying gait smoothness in patients with Parkinson's disease.

BACKGROUND: Impairments in biomechanics and neural control can disrupt the timing and muscle pattern activation necessary for smooth gait. Gait is one of the most affected motor characteristics in Parkinson's disease (PD), but its smoothness has not been well-studied. This work applies the recently proposed spectral arc length measure (SPARC) to study, for the first time, gait in patients with PD. We hypothesized that the gait of patients with PD would be less smooth than that of healthy controls, as reflected in the SPARC measures. METHODS: The gait of 101 PD patients and 39 healthy controls

From clutter to confidence: home environmental triggers and adaptive solutions for freezing of gait in Parkinson's disease.

Freezing of gait (FOG) is a disabling symptom in Parkinson's disease (PD), often triggered by home environmental factors such as cluttered or poorly lit spaces. Despite growing recognition of these factors, home environmental adaptations (HEA) remain an underexplored yet modifiable domain in FOG management. This scoping review synthesises current evidence on HEA and their potential to mitigate FOG by addressing environmental triggers and enhancing person-environment fit. It highlights case examples, summarises literature on environmental triggers, and discusses multidisciplinary assessment and

The Role of Flexor Hallucis Longus Tendon Transfer in the Surgical Treatment of Foot Drop.

Foot drop has various etiologies, such as neurologic (peripheral nerve injury, neuropathy, lumbar radiculopathy, and cerebral lesions) or muscular (extensor muscle injury and compartment syndrome) causes. Tendon transfers can aid in the restoration of dorsiflexion in the treatment of foot drop. The tibialis posterior tendon is the primary tendon used in these surgeries for transfer. In this present study, the flexor hallucis longus tendon is transferred to the dorsum of the foot because of following knee dislocation that caused a nonfunctioning posterior tibial tendon. At the sixth month posto

Brainstem lesions and gait.

The brainstem contains virtually all of the important structures involved in experimental models of locomotion, encompassing control of upright posture, balance, and stepping. The physiologic basis for these functions is intricately related. Studies of the effects of lesions and disease on these functions in humans are limited to clinical observation and hampered by the anatomic complexity of closely spaced structures and lack of selectivity of lesions. Accordingly, any description of the clinical effects of brainstem lesions on gait and posture is imprecise because weakness and ataxia either

Peroneus longus transfer for drop foot in Hansen disease.

Leprosy or Hansen's disease is a chronic infectious disease caused by the Mycobacterium leprae. Nerve injury is a central feature of the pathogenesis of leprosy that results in autonomic, sensory and motor neuropathy. One of the most common secondary disabilities caused by Hansen's disease is the drop foot and it is found in 2% to 5% of newly-diagnosed leprosy patients. Unlike the clinical picture of traumatic injury of the common peroneal nerve where both of its branches (the deep peroneal nerve and the superficial peroneal nerve) are involved, in leprosy there is the possibility of isolated

[Function disorders of otoliths: clinical aspects and therapy options].

In the clinical routine, the diagnostic of the lateral gait is often comparable with a complete peripheral vestibular function examination. Diseases of the otolithic organs, with the saccule and utricle are not identified with conventional clinical examination methods. In more than 50% of patients with thermal hypostimulation, orientation tests show a simultaneous pathology of the utricle function. Participation of the saccule has been described for vestibular neuropathy, Menière's disease, bilateral vestibulopathy and acoustic neurinoma. Both otolithic organs are included in posttraumatic ver

Deciphering Freezing of Gait: What Neuropathology Reveals About an Episodic Phenomenon.

Freezing of gait (FoG) is an episodic, debilitating motor phenomenon defined as paroxysmal episodes wherein there is an inability to step effectively, despite attempting to do so. Although FoG is most commonly associated with Parkinson's disease, it manifests across a spectrum of progressive neurodegenerative and potentially transient non-neurodegenerative conditions that affect overlapping locomotor circuits. The episodic nature of FoG implies transient disruptions of a distributed locomotor network rather than fixed structural lesions, rendering its neuropathological basis particularly chall

Management Approaches to Spastic Gait Disorders.

Spastic gait presents clinically as the net mechanical consequence of neurological impairments of spasticity, weakness, and abnormal synergies and their interactions with the ground reaction force in patients with upper motor neuron syndromes and with some neuromuscular diseases. It is critical to differentiate whether the primary problem is weakness or spasticity, thus better understanding different phenotypes of spastic gait disorders. Pelvic girdle abnormality plays a pivotal role in determining the clinical presentation of gait disorders, since it determines the body vector and compensator

Identifying Subtle Motor Deficits Before Parkinson's Disease is Diagnosed: What to Look for?

Motor deficits typical of Parkinson's disease (PD), such as gait and balance disturbances, tremor, reduced arm swing and finger movement, and voice and breathing changes, are believed to manifest several years prior to clinical diagnosis. Here we describe the evidence for the presence and progression of motor deficits in this pre-diagnostic phase in order to provide suggestions for the design of future observational studies for an effective, quantitatively oriented investigation. On the one hand, these future studies must detect these motor deficits in as large (potentially, population-based)

Why we should study gait initiation in Parkinson's disease.

The gait initiation process is of particular interest in Parkinson's disease because it combines motor and cognitive components of movement preparation (referred to as anticipatory postural adjustments) and movement execution (the step by itself). Moreover, gait initiation in Parkinson's disease is often affected by motor blocks (a subtype of the "freezing of gait" phenomenon). Gait initiation disturbances in Parkinson's disease include delayed release of anticipatory postural adjustments, hypokinetic anticipatory postural adjustments (reduced scaling) and bradykinetic anticipatory postural ad

How tandem gait stumbled into the neurological exam: a review.

Tandem gait testing is an integral part of the neurological exam. It is informative in a wide variety of disorders ranging from cerebellar disease to vestibular and peripheral neuropathies, parkinsonism, and other neurodegenerative conditions. We discuss the history and development of tandem gait testing as well as its technique, utility, and limitations in the assessment of neurological conditions. Tandem gait has emerged as a tool in the assessment of cerebellar disease, Huntington disease, idiopathic Parkinson's disease, atypical parkinsonism, peripheral neuropathies, and vestibulopathies.

Quantitative Gait and Balance Outcomes for Ataxia Trials: Consensus Recommendations by the Ataxia Global Initiative Working Group on Digital-Motor Biomarkers.

With disease-modifying drugs on the horizon for degenerative ataxias, ecologically valid, finely granulated, digital health measures are highly warranted to augment clinical and patient-reported outcome measures. Gait and balance disturbances most often present as the first signs of degenerative cerebellar ataxia and are the most reported disabling features in disease progression. Thus, digital gait and balance measures constitute promising and relevant performance outcomes for clinical trials. This narrative review with embedded consensus will describe evidence for the sensitivity of digital g

Multi-Scale Temporal Analysis With a Dual-Branch Attention Network for Interpretable Gait-Based Classification of Neurodegenerative Diseases.

The accurate diagnosis of neurodegenerative diseases (NDDs), such as Amyotrophic Lateral Sclerosis (ALS), Huntington's Disease (HD), and Parkinson's Disease (PD), remains a clinical challenge due to the complexity and subtlety of gait abnormalities. This paper proposes the Dual-Branch Attention-Enhanced Residual Network (DAERN), a novel deep learning architecture that integrates Dilated Causal Convolutions (DCCBlock) for local gait pattern extraction and Multi-Head Self-Attention (MHSA) for long-range dependency modeling. A Cross-Attention Fusion module enhances feature integration, while SHap

Passive sensing of gait and medication-related fluctuations in Parkinson's disease.

BACKGROUND: Gait impairment is a hallmark symptom of Parkinson's Disease (PD). Traditional clinical assessments cannot capture real-world motor fluctuations, as they are sparsely performed. We validated the use of nearables, passive sensing technologies, including Kinect RGB-D cameras and ultra-wideband (UWB) radar, for continuous, objective assessment of gait fluctuations in PD within a home-like setting. METHODS: Fifteen PD patients with mild symptoms and fourteen age- and sex-matched healthy controls (HC) performed 4-metre walking tasks in a living lab facility. Patients repeated the task

Psychometric properties of wearable technologies to assess post-stroke gait parameters: A systematic review.

BACKGROUND: Wearable technologies using inertial sensors are an alternative for gait assessment. However, their psychometric properties in evaluating post-stroke patients are still being determined. This systematic review aimed to evaluate the psychometric properties of wearable technologies used to assess post-stroke gait and analyze their reliability and measurement error. The review also investigated which wearable technologies have been used to assess angular changes in post-stroke gait. METHODS: The present review included studies in English with no publication date restrictions that eva

Imaging: What can it tell us about parkinsonian gait?

Functional neuroimaging has provided new tools to study cerebral gait control in Parkinson's disease (PD). First, imaging of blood flow functions has identified a supraspinal locomotor network that includes the (frontal) cortex, basal ganglia, brainstem tegmentum, and cerebellum. These studies also emphasize the cognitive and attentional dependency of gait in PD. Furthermore, gait in PD and related syndromes like progressive supranuclear palsy may be associated with dysfunction of the indirect, modulatory prefrontal-subthalamic-pedunculopontine loop of locomotor control. The direct, stereotype

Tic-induced gait dysfunction.

BACKGROUND: Many neurological disorders impair gait, but only a few of them are episodic or paroxysmal, the most important ones being freezing of gait and paroxysmal dyskinesias. METHODS: We describe 4 patients with tic disorders (3 with Tourette syndrome, and 1 with a tic disorder secondary to vascular disease) in whom intrusion of complex motor tics interfered with normal progression of stepping, thus producing an episodic gait disorder. RESULTS: The involuntary movements that interfered with gait had features typical for tics, including their brief, sudden, irresistible, inapposite, and n
