The authors present a technique for neovaginal reconstruction in gender-affirming vaginoplasty using an anteriorly based peritoneal turnover flap to create a full-length lining throughout the depth of the neovaginal canal. Between 2024 and 2025, 10 patients successfully underwent the procedure without major complications. At the 3-month follow-up, 80% of patients maintained neovaginal depth comparable to the immediate postoperative measurement. Internal neovaginal examination demonstrated that the neovaginal lining exhibited a mucosal surface, high elasticity, and self-lubrication. The potenti

OBJECTIVE: To generate surgeon consensus on metoidioplasty and phalloplasty gender-affirming surgery outcomes statements using a modified Delphi method.
MATERIALS AND METHODS: We invited metoidioplasty and phalloplasty gender-affirming surgery surgeons to anonymously participate in a two-round web-based Delphi survey to generate consensus on patient centered outcome statements generated from surgeon practice patterns, literature review, and patient input. Consensus was predefined as at least 70% agreement. Surgeons were asked to provide open ended feedback on statements which were thematicall

The increased awareness of the transgender population and their medical needs has given rise to a wide array of gender-affirming surgeries and hormonal therapies. To better understand the implication of testosterone therapy on female-to-male gender-affirming mastectomies, we conducted a retrospective cohort study based on the medical histories of 170 transgender males operated on by a single surgeon over 18 years. One hundred and one (59.4%) patients received hormonal therapy. The average age of patients in the testosterone treatment group was 20.6 ± 5.3 (range 14-49) years. The median weight

OBJECTIVE: Current available plastic surgery positions in craniofacial surgery are not commensurate with the growing number of trained plastic surgery residents and fellows interested in the field. Despite this, there is growing demand for surgery in other areas of the craniofacial field, such as facial gender-affirming surgery (GAS), in which academic plastic surgery programs can increase resident and fellow case exposure. The present study sought to characterize the increase in craniofacial surgery exposure with the introduction of a GAS rotation among plastic surgery residents at one academ

BACKGROUND: Studies comparing the surgical routes for sex-affirming hysterectomy in female-to-male (FtM) transgender individuals remain limited. This study compared the perioperative outcomes of vaginal total hysterectomy (VTH) and laparoscopic-assisted vaginal hysterectomy (LAVH) in an Asian FtM cohort.
METHODS: We retrospectively reviewed the data of individuals who were FtM transgender and underwent sex-affirming hysterectomy with bilateral salpingo-oophorectomy at a tertiary referral center between January 2016 and May 2026. Patients who underwent combined major sex-affirming procedures,

BACKGROUND: Transgender and non-binary (TGNB) individuals face disproportionately limited access to gender-affirming surgery (GAS) due to systemic inequities. This study examines how these inequities, such as race and ethnicity, impact the utilization of GAS and associated surgical complications across different GAS subtypes.
METHODS: Using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database, we retrospectively reviewed TGNB individuals who underwent GAS from 2012 to 2021. The primary outcome was the incidence of major complications (e.g., unpla

Well-known risk factors for anterior segment ischemia (ASI) following strabismus surgery include ipsilateral surgery on three or more rectus muscles, older age, and vasculopathy. ASI is rarely reported in young patients following uneventful strabismus surgery on two ipsilateral rectus muscles. We report a 30-year-old transgender female on long-term estrogen therapy who underwent strabismus surgery involving recessions of both lateral rectus muscles, the right inferior rectus muscle, and the left superior rectus muscle. The left eye developed severe ASI with hypotony maculopathy that was resist

INTRODUCTION: Various methods have been described to create a functional neovagina with feminizing (male-to-female) gender affirming surgery. Intestinal vaginoplasty using ileal or colon segments confers natural mucus production and greater canal depth with primary vaginoplasty. In this work we describe an alternative approach to primary and salvage vaginoplasty using right colon. We focus on relative advantages compared to use of other bowel segments, and we review patient outcomes.
METHODS: Transgender women who had previously undergone primary vaginoplasty underwent laparoscopic right colo

We assessed the outcomes of gender-affirming surgery (GAS, or sex-reassignment surgery) 4 to 6 years after first clinical contact, and the associations between postoperative (dis)satisfaction and quality of life (QoL). Our multicenter, cross-sectional follow-up study involved persons diagnosed with gender dysphoria (DSM-IV-TR) who applied for medical interventions from 2007 until 2009. Of 546 eligible persons, 201 (37%) responded, of whom 136 had undergone GAS (genital, chest, facial, vocal cord and/or thyroid cartilage surgery). Main outcome measures were procedure performed, self-reported co

Gender-affirming surgery (GAS) is often sought after to alleviate the distress of those who suffer from gender dysphoria (GD). While many studies have shown that a significant percentage of people benefit from this procedure, a number of individuals later regret their decision of undergoing surgery. Studies have illustrated what regret depicts, categorizing regret based on intensity, persistency, and sources, in the hopes to prevent an unwanted irreversible intervention. Here, an in-depth interview with a 35-year-old transwoman from Taiwan who underwent feminizing GAS at the age of 31 illustra

BACKGROUND: The adoption of telehealth has surged in recent years, particularly following the COVID-19 pandemic, due to its ability to increase access, reduce logistical burdens, and improve convenience for patients and providers alike. In the context of gender-affirming surgery (GAS), where barriers such as stigma, geographic distance, and cost persist, telehealth offers a promising alternative to traditional in-person care. Telehealth in the gender affirmation surgery setting, virtual vs. in-person initial consultations in a surgical setting, and surgical conversion rates by initial consulta

This narrative review provides an in-depth description of gender-affirming breast surgery within the context of publicly funded healthcare systems in the Nordic countries. A comprehensive literature search was conducted in collaboration with two information specialists, focusing on original research, reviews, and clinical guidelines published in English. The prevalence of transgender and non-binary individuals in the Nordic region ranges from 0.04% to 0.6%, depending on the country and study methodology. Gender-affirming treatment, including hormone therapy and surgical interventions such as b

INTRODUCTION: Gender Affirmation Surgery often referred to as 'Nirvana' in the transgender community in India is a major process that most trans women undergo. It is practiced in the traditional way as, the thayamma operation (midwife procedure) where the elderly transwoman cut off the penis and testicles without any anesthesia or medical supervision. However, the medical procedure of GAS in India is a recent advancement, which is accessible only to a few, the procedure that most trans women have undergone is penectomy (removal of the penis) and orchiectomy (removal of the testicles).
