Feature selection for computerized fetal heart rate analysis using genetic algorithms.

During birth, timely and accurate diagnosis is needed in order to prevent severe conditions such as birth asphyxia. The fetal heart rate (FHR) is often monitored during labor to assess the condition of fetal health. Computerized FHR analysis is needed to help clinicians identify abnormal patterns and to intervene when necessary. The objective of this study is to apply Genetic Algorithms (GA) as a feature selection method to select a best feature subset from 64 FHR features and to integrate these best features to recognize unfavorable FHR patterns. The GA was trained on 408 cases and tested on

Intrapartum fetal heart rate characteristics of nonanomalous fetuses with trisomy 21.

OBJECTIVE: To compare the intrapartum fetal heart rate (FHR) characteristics and selected birth outcomes of nonanomalous trisomy 21 (T21) fetuses to matched controls. STUDY DESIGN: Nonanomalous, non-growth-restricted T21 infants were identified and matched by gestational age to presumed euploid controls. We excluded women with scheduled cesarean deliveries and multiple gestations. The incidence of abnormal FHR patterns within 3 hours of delivery and birth outcomes were compared between T21 fetuses and controls. The presence of any abnormal FHR pattern was the primary outcome. Birth outcomes i

Fetal heart rate parameters of anencephalic fetuses according to gestational age.

OBJECTIVE: To evaluate various fetal heart rate (FHR) parameters of anencephalic fetuses and to assess the effect of the fetal nervous system on FHR according to gestational age. STUDY DESIGN: The antepartum nonstress test was performed in 25 anencephalic fetuses. Each FHR parameter was analyzed using a computerized FHR analysis system for various gestational groups, and the results were compared with results from a normal control group (N = 25). RESULTS: The mean baseline FHR of anencephalic fetuses decreased with increasing gestational age, as in normal fetuses. FHR variability increased w

Intraoperative fetal heart rate monitoring during emergency neurosurgery in a parturient.

Malignant brain tumors during pregnancy are rare, and these patients seldom require immediate surgical intervention. A 27-year-old pregnant woman underwent emergency craniotomy. Anesthesia was induced with intravenous thiopental-fentanyl; it was maintained with isoflurane in oxygen and continuous intravenous remifentanil infusion. We used full stomach precautions but omitted succinylcholine for fear of increasing the intracranial pressure during induction of anesthesia. To detect fetal hypoxia and the effects of anesthesia on fetal hemodynamics, the fetal heart rate (FHR) was monitored using a

Diagnosis of an unsuspected maternal hemorrhage via fetal heart rate tracing.

Early detection of placental abruption often relies on the observation of vaginal bleeding; however, overt bleeding does not always occur. We report the case of an unsuspected placental abruption diagnosis that was prompted by an internal fetal scalp electrode tracing. The presence of a "normal" fetal heart rate (FHR) of approximately 150 beats per minute with poor variability, which matched the maternal heart rate (MHR), suggested that the tracing was not of fetal origin. An urgent ultrasound examination revealed a fetal demise with a possible concealed abruption, proving that the scalp elect

Effect of magnesium on fetal heart rate variability using computer analysis.

We studied 16 women, at 32 weeks' or more gestation who required magnesium sulfate (MgSO4) therapy for preterm labor or preeclampsia. A 60-minute Doppler fetal heart rate (FHR) tracing, analyzed by the Oxford Sonicaid System 8000, was obtained for 1 hour before and 2 hours after each patient received intravenous MgSO4 therapy. Maternal serum Mg2+ levels were obtained at the second monitoring session. Matched paired measures of FHR parameters were compared with the Student's t test. After MgSO4 administration, we noted significant falls in long-term variability, short-term variability, and tota

Placental pathology and abnormal fetal heart rate patterns in gestational diabetes.

Infants of diabetic mothers are considered to be at increased risk of fetal distress. We suggest that umbilical vasculitis is strongly associated with abnormal fetal rate patterns, which indicate fetal tissue hypoxia even in the absence of clinical evidence of intrapartum infection. In the present series, 132 term deliveries with Class A diabetes mellitus were compared with 159 uncomplicated term deliveries. Umbilical vasculitis and gestational age greater than 281 days were both highly correlated with abnormal fetal heart rate patterns in diabetic pregnancies (p less than 0.001). These effect

Effect of maternal magnesium sulfate treatment on fetal heart rate variability.

The effect of maternal magnesium sulfate treatment on fetal heart rate variability (FHRV) was evaluated by comparing the maternal serum magnesium level drawn during labor to the degree of FHRV seen concurrently on fetal rate heart monitor strips. The records of 57 patients treated with intravenous magnesium sulfate for hypertensive disorders of pregnancy were studied. Fetal heart rate variability was measured by both subjective assessment and objective measurement. The difference in beats/min between the average high and low fetal heart rates, at the time the magnesium level obtained, was used

Fetal heart rate decelerations after oxytocin infusion in an abdominal pregnancy.

Fetal heart rate (FHR) decelerations during a contraction stress test are recognized as signs of possible uteroplacental insufficiency. Although these decelerations have been described commonly in intrauterine pregnancies, they have not been noted in extrauterine pregnancies. Reported herein are FHR decelerations associated with oxytocin infusion in an extrauterine pregnancy and a discussion of the possible pathophysiologic mechanisms.

Perinatal outcome in the absence of antepartum fetal heart rate acceleration.

The perinatal outcome of 27 pregnancies in which antepartum fetal activity testing revealed fetal heart rate acceleration to be either absent or less than 10 beats per minute for 80 minutes is presented. At delivery, each pregnancy was found to have one or more features consistent with uteroplacental insufficiency. These included fetal growth retardation (74%), oligohydramnios (81%), fetal acidosis (41%), meconium (30%), and placental infarction (93%). There were four fetal and seven neonatal deaths, for a perinatal mortality of 41%. Despite delivery of all live-born infants by cesarean sectio

Association between placental microplastic burden and intrapartum fetal heart rate abnormalities.

Microplastic (MP) contamination has emerged as a global environmental concern, with increasing evidence of its presence in human placental tissue. However, no previous human studies have directly evaluated the association between placental microplastic exposure and fetal cardiac physiology during labor. Our study aimed to investigate the relationship between placental MP accumulation and the occurrence of intrapartum non-reassuring fetal heart rate (NRFHR). A prospective cohort study was conducted among 2738 pregnant women recruited from China between 2023 and 2024. Placental MPs were quantifi

Extraction of compression indices from maternal-fetal heart rate simultaneous signals.

Intrapartum asphyxia is responsible for approximately 900 000 deaths per year worldwide. These numbers show the urgency of investing in the quality of fetal health care. The heart rate signal is a complex signal and sometimes behaves unpredictably. Thus, it becomes relevant to study approaches that take into account their complexity, namely non-linear compression-based methods. In this work, feature extraction was based on two approaches: univariate and bivariate. The univariate approach is concerned with the extraction of fetal, maternal and maternal-fetal compression ratios and the bivariate

The clinical significance of electronic fetal heart rate monitoring in twins.

OBJECTIVES: Fully effective intrapartum cardiotocographic (CTG) fetal heart monitoring is still missing. Visual analysis is far from credibility. Additional, computerized analysis techniques were proposed however they did not substantially decrease possible risks of fetal asphyxia. In twin pregnancies the problem is even more complicated. Our goal is to find the most valuable parameters in intrapartum CTG surveillance in twins, based on actual FIGO criteria. MATERIAL AND METHODS: Study included 58 women in labor who had been admitted to Delivery Department of tertiary care hospital with twin

Predelivery placenta-associated biomarkers and computerized intrapartum fetal heart rate patterns.

BACKGROUND: Increasing syncytiotrophoblast stress in term and postdate placentas is reflected by increasing antiangiogenic dysregulation in the maternal circulation, with low "proangiogenic" placental growth factor concentrations and increased "antiangiogenic" soluble fms-like tyrosine kinase-1 concentrations. Imbalances in these placenta-associated proteins are associated with intrapartum fetal compromise and adverse pregnancy and delivery outcome. Cardiotocography is widely used to assess fetal well-being during labor, but it is insufficient on its own for predicting adverse neonatal outcome

Challenges of Developing Robust AI for Intrapartum Fetal Heart Rate Monitoring.

Background: CTG remains the only non-invasive tool available to the maternity team for continuous monitoring of fetal well-being during labour. Despite widespread use and investment in staff training, difficulty with CTG interpretation continues to be identified as a problem in cases of fetal hypoxia, which often results in permanent brain injury. Given the recent advances in AI, it is hoped that its application to CTG will offer a better, less subjective and more reliable method of CTG interpretation. Objectives: This mini-review examines the literature and discusses the impediments to the su

Multi-Detector Heart Rate Extraction Method for Transabdominal Fetal Pulse Oximetry.

Intrapartum fetal well-being assessment relies on fetal heart rate (FHR) monitoring. Studies have shown that FHR monitoring has a high false-positive rate for detecting fetal hypoxia during labor and delivery. A transabdominal fetal pulse oximeter device that measures fetal oxygen saturation non-invasively through NIR light source and photodetectors could increase the accuracy of hypoxia detection. As light travels through both maternal and fetal tissue, photodetectors on the surface of mother's abdomen capture mixed signals comprising fetal and maternal information. The fetal information shou

Fetal heart rate variability with hypoxemia in an instrumented sheep model.

OBJECTIVE: To examine the effect of hypoxemia on fetal heart rate (FHR) variability, using an instrumented pregnant sheep model. METHODS: In this prospective study, 19 pregnant sheep were instrumented under general anesthesia, at a mean gestational age of 127 days. After a 5-day recovery period, hypoxemia was induced by attaching the mother to a rebreathing circuit. Hypoxemia was sustained for 120 min, following which it was reversed until maternal and fetal partial pressure of oxygen (pO2 ) returned to baseline. FHR recordings at baseline, after 30 and 120 min of hypoxemia and at recovery we
