The role of invasive hemodynamic monitoring on the outcomes of cardiogenic shock in patients with acute myocardial infarction (AMI-CS) is controversial. We evaluated whether invasive hemodynamic monitoring improved in-hospital mortality in patients with AMI-CS. This study analyzed 1513 patients with AMI-CS presenting between 2020 and 2022. Patients were classified into 2 groups on the basis of invasive monitoring. Patients receiving invasive hemodynamic monitoring (n = 602) were more likely to present with advanced cardiogenic shock stages, cardiac arrest, bradyarrhythmias, and required more m

IMPORTANCE: Evaluation and treatment of transitional circulation in neonates with extremely low gestational age (ELGA) varies greatly across centers.
OBJECTIVE: To determine whether multimodal hemodynamic monitoring in neonates with ELGA during the transitional period (first 72 hours of life) will improve cardiorespiratory-kidney health by decreasing vasoactive-ventilation-renal (VVR) score at 7 days.
DESIGN, SETTING, AND PARTICIPANTS: This unmasked, 2-arm randomized clinical trial included neonates born at a gestational age of 230 to 286 weeks and admitted to a neonatal unit in Canada. Pati

INTRODUCTION: Hemodynamic decompensation during catheter ablation occurs due to prolonged procedure time and irrigant delivery directly into the cardiac chambers. Real-time hemodynamic monitoring of patients undergoing catheter ablation procedures may identify patients at risk of decompensation; we set out to assess the feasibility of a novel, real-time, intracardiac pressure monitoring system using a standard irrigated ablation catheter.
METHODS: We studied 13 consecutive who underwent pressure measurement of the left atrium (LA) and left ventricle (LV) via transeptal access with a Swan Ganz

INTRODUCTION: Mechanical thrombectomy is revolutionizing treatment of acute stroke due to large vessel occlusion (LVO). Unfortunately, use of the modified Thrombolysis in Cerebral Infarction score (mTICI) to characterize recanalization of the cerebral vasculature does not address microvascular perfusion of the distal parenchyma, nor provide more than a vascular "snapshot." Thus, little is known about tissue-level hemodynamic consequences of LVO recanalization. Diffuse correlation spectroscopy (DCS) and diffuse optical spectroscopy (DOS) are promising methods for continuous, noninvasive, contra

The purpose of the study was to evaluate and identify patterns of changes in central and cerebral hemodynamics in neonates with low and extremely low birth weight and birth asphyxia, in order to achieve the early diagnosis of disorders of the central hemodynamics and autoregulation of cerebral blood flow, for timely fluid correction and inotropic therapy. Observation data on 44 preterm infants who were treated at the Kharkiv City Perinatal Centre and Regional Children's Clinical Hospital №1 of Kharkiv has been analyzed. Ultrasound control of the central and cerebral hemodynamics needs to be ca

BACKGROUND: Intradialytic hypotension (IDH) is the most common complication of hemodialysis (HD) and is associated with major adverse cardiovascular events and mortality. Current management strategies are reactive, and reliable noninvasive tools for early prediction are lacking. Electrical impedance tomography (EIT) offers noninvasive, real-time monitoring of thoracic fluid content (TFC) and cardiopulmonary parameters, potentially identifying early hemodynamic instability preceding IDH.
METHODS: We enrolled 18 patients with end-stage kidney disease and frequent IDH and monitored 73 HD session

BACKGROUND: Enhanced Recovery After Surgery (ERAS) has been shown to reduce perioperative complications, shorten intensive care unit (ICU) length of stay, and facilitate faster extubation in cardiac surgery patients, though the implementation benefits remains understudied. The role of continuous hemodynamic monitoring in optimizing ERAS for cardiac surgery has not been well established. This study evaluates the impact of ERAS implementation on cardiovascular patients undergoing elective isolated coronary artery bypass grafting (ISO-CABG).
METHODS: Retrospective analysis of prospectively colle

Inadequate antinociception during skull pin fixation may cause hemodynamic instability in intracranial surgery. The optimal concentration of remifentanil to provide adequate antinociception and stable hemodynamics during skull pin fixation under analgesia nociception index monitoring is unknown. This study is to assess the 90% effective concentration of remifentanil for skull pin fixation under hemodynamic and analgesia nociception index monitoring. Twenty-six patients were enrolled for intracranial surgery, anesthesia was induced and maintained under total intravenous anesthesia using target-

BACKGROUND: Arterial invasive monitoring is the most common method in the USA for hemodynamic monitoring during atrial fibrillation (AF) ablation. Although studies have shown favorable comparison between non-invasive and invasive hemodynamic monitoring (IHM) in non-cardiac procedures under general anesthesia, limited data is available for complex cardiac procedures such as AF ablation in the USA. With progressive improvement in AF ablation procedural safety, particularly with routine use of intracardiac echocardiography (ICE) to monitor for pericardial effusion, it is unclear if invasive hemod

OBJECTIVE: Circulatory shock is a life-threatening syndrome resulting in multiorgan failure and a high mortality rate. The aim of this consensus is to provide support to the bedside clinician regarding the diagnosis, management and monitoring of shock.
METHODS: The European Society of Intensive Care Medicine invited 12 experts to form a Task Force to update a previous consensus (Antonelli et al.: Intensive Care Med 33:575-590, 2007). The same five questions addressed in the earlier consensus were used as the outline for the literature search and review, with the aim of the Task Force to produ

BACKGROUND: The pathophysiology of neurocardiogenic syncope is heterogeneous. This study aim was to analyze whether extended monitoring during tilt-table testing provided additional information on the hemodynamic and vegetative state prior to neurocardiogenic syncope.
METHODS: This retrospective analysis is based on data of head-up tilt-table testing of 40 unselected consecutive patients with a history of unexplained syncope. For optimized characterization of the type of syncope, monitoring included electrocardiogram (ECG), blood pressure measurements and cardiac output, peripheral vascular r

Data, obtained by invasive hemodynamic monitoring, are now routinely available in our Intensive Care Unit, and are processed by means of a small calculator to a cardiovascular profile, including O2 transport/consumption, vascular resistance and ventricular work values. Patients with clinically persistent shock states, after 12-24 hours optimal classical cardiovascular therapy, are invasively studied. Vasodiltor therapy frequently improves not only hemodynmic parameters, but also oxygen delivery to the tissues. Tolmesoxide, a new peripheal vasodilator, has been used in eleven patients. Prelimin

Background and Objectives: Indications for liver transplants are increasing worldwide due to the growing number of transplants performed on patients with significant cardiovascular and respiratory risk factors. Additional support for this trend comes from the growing use of marginal organs, which is made possible by donations made after circulatory death (DCD). Liver transplantation (LT) in such high-risk patients may be challenging and may require perioperative Extracorporeal Membrane Oxygenation (ECMO). There is a lack of evidence on the best hemodynamic monitoring techniques for patients un

BACKGROUND: Invasive hemodynamic monitoring devices provide relevant insights into the hemodynamic status of heart failure (HF) patients. Additionally, circulating biomarkers have been advocated as additives in HF management by offering prognostic information and therapeutic targets. However, limited data exist correlating serially measured biomarkers to hemodynamic pressures provided by invasive sensors. This study aims to investigate the association between serial biomarker patterns and pulmonary artery (PA) pressure dynamics in HF patients.
METHODS AND RESULTS: This study is a substudy of

Neonates undergoing cardiac surgery involving aortic arch reconstruction are at an increased risk for hypoxic-ischemic brain injury. Deep hypothermia is utilized to help mitigate this risk when periods of circulatory arrest are needed for surgical repair. Here, we investigate correlations between non-invasive optical neuromonitoring of cerebral hemodynamics, which has recently shown promise for the prediction of postoperative white matter injury in this patient population, and invasive cerebral microdialysis biomarkers. We compared cerebral tissue oxygen saturation (StO2), relative total hemog
