Skilled Nursing Facility Network Capacity and Hospital Length of Stay.

IMPORTANCE: Delays in hospital discharge to skilled nursing facilities (SNFs) are widely viewed by hospital administrators and policymakers as a significant contributor to hospital crowding and adverse patient outcomes. However, the extent to which regional SNF capacity is associated with hospital length of stay (LOS) is not well understood. OBJECTIVES: To construct empirically derived hospital-SNF markets using a network science approach and to examine whether variation in SNF capacity within these markets is associated with hospital LOS. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectio

Supporting Health Care Staff With Family Meals During the COVID-19 Pandemic.

Family meals provide a unique opportunity for families to eat well and engage positively with each other. In response to the challenges presented by the COVID-19 (coronavirus disease 2019) pandemic, a new initiative to support health care staff to share healthy meals with their families was developed. At a hospital in Queensland, Australia, dietetic staff collaborated with the on-site food service retailer to develop and offer a range of hot meals that staff could take home for their families at the end of their day. The meals were nutritious, reasonably priced, and designed to feed a family o

Transport and Management of Patients With Confirmed or Suspected Ebola Virus Disease.

The foundation of safe care for patients with confirmed or suspected Ebola virus disease is effective infection control practice, which requires implementation of appropriate administrative policies, work practices, and environmental controls, accompanied by focused education, training, and supervision. In 2002, Emory University partnered with the Centers for Disease Control and Prevention to develop a capability for the evaluation and management of individuals with serious communicable disease. In 2005, the University of Nebraska developed a similar isolation capability. In each case, the hos

Clearing the air: Assessing healthcare professionals' awareness of air pollution's health consequences.

Air pollution (AP) significantly jeopardises health, with the Royal College of Physicians accepting the adverse effects of AP are not being sufficiently communicated to patients by healthcare professionals (HCP). To explore HCPs' understanding and attitudes toward AP and its health impacts, we conducted a service evaluation survey in a group of hospital doctors. A questionnaire comprising 20 questions about AP and its health associations was completed by 133 hospital doctors working at University Hospital Southampton NHS Foundation Trust, UK. While 65% (n = 86) of respondents strongly agreed t

Investigation of a COVID-19 outbreak at a regional prison, Northern Uganda, September 2020.

Despite implementing measures to prevent introduction of COVID-19 in prisons, a COVID-19 outbreak occurred at Moroto Prison, northern Uganda in September 2020. We investigated factors associated with the introduction and spread of COVID-19 in the prison. A case was PCR-confirmed SARS-CoV-2 infection in a prisoner/staff at Moroto Prison during August-September 2020. We reviewed prison medical records to identify case-patients and interviewed prison and hospital staff to understand possible infection mechanisms for the index case-patient and opportunities for spread. In a retrospective cohort st

[Development of Clinical Practice and Clinical Pharmacy Research toward the Appropriate Use of Medications].

We have engaged in clinical practice and pharmaceutical research aimed at the appropriate use of medications. In pharmacist-to-pharmacist collaboration using medication guidance information-sharing documents, it was possible to monitor treatment efficacy and adverse effects through sharing clinical data, or making prescription proposals. Our hospital's pharmacist services contributed to expanding the calculation of drug management and guidance fees by pharmacists in clinics with beds. The drug interaction that occurred between enteric-coated omeprazole tablets and Maalox® granules suggested th

Integrated palliative care service for Parkinson's disease in the UK: impact on place of death.

OBJECTIVES: Patients with advanced Parkinson's disease and related disorders (PDRD) experience significant symptom burden, yet access to specialist palliative care (SPC) is often limited. The Bristol service uses existing resources to form a multidisciplinary meeting (MDM) attended by both movement disorders and palliative care specialists. This integrates primary and SPC and allows sharing of expertise to holistically address unmet palliative care needs. Here, we evaluate this model. METHODS: We conducted a retrospective review of case notes of patients discussed in the Parkinson's Disease P

More that unites us than divides us? A qualitative study of integration of community health and social care services.

BACKGROUND: The integration of community health and social care services has been widely promoted nationally as a vital step to improve patient centred care, reduce costs, reduce admissions to hospital and facilitate timely and effective discharge from hospital. The complexities of integration raise questions about the practical challenges of integrating health and care given embedded professional and organisational boundaries in both sectors. We describe how an English city created a single, integrated care partnership, to integrate community health and social care services. This led to the d

Workforce Wellbeing and Future Pandemic Preparedness in Emergency Departments: Lessons Learned While Caring for Persons Experiencing Homelessness During COVID-19.

Background/Objective: Research has indicated that providing care during the COVID-19 pandemic negatively impacted healthcare providers' wellbeing and burnout. Much of the literature has focused on emergency department (ED) physicians and nurses, including the distinct challenges they faced caring for persons experiencing homelessness (PEH), including addressing unmet basic needs, limited discharge options, and disruptions to community-based health and social services during the pandemic. Comparatively little research has examined the experiences of other multidisciplinary team members caring f

Improving clinical practice in primary care for the prevention and control of noncommunicable diseases: a multi-actor approach to two regional pilot projects in Kazakhstan.

Improving access to quality services is integral to achieving better outcomes for noncommunicable diseases (NCDs). In Kazakhstan, like other countries with historically centralized governance models, key to improving quality is instilling a common and shared understanding of the roles and responsibilities in correspondence with the multifaceted nature of quality of care. This review details the experience of two pilot projects implemented in Kazakhstan's regions of Kyzylorda and Mangystau over a three-year period with the aim to improve clinical practice through a multi-actor, multi-interventi
