Monogenic Forms of Male Infertility.

Male infertility is a multifactorial and heterogeneous pathological condition affecting 7% of the general male population. The genetic landscape of male infertility is highly complex as semen and testis histological phenotypes are extremely heterogeneous, and at least 2000 genes are predicted to be involved in spermatogenesis. Genetic factors have been described in each etiological category of male reproductive impairment: (1) hypothalamic-pituitary axis dysfunction; (2) quantitative and qualitative alterations of spermatogenesis; (3) ductal obstruction/dysfunction. In 25% of azoospermic and i

Lifestyle causes of male infertility.

OBJECTIVE: To examine the potential effects of lifestyle factors on male reproductive health. Evidence of a global decline in human sperm quality over recent decades has been accumulating. Environmental, occupational, and modifiable lifestyle factors may contribute to this decline. This review focuses on key lifestyle factors that are associated with male infertility such as smoking cigarettes, alcohol intake, use of illicit drugs, obesity, psychological stress, advanced paternal age, dietary practices, and coffee consumption. Other factors such as testicular heat stress, intense cycling train

Schistosomiasis (bilharziasis) and male infertility.

Schistosomiasis (bilharziasis) is an endemic parasitic disease of a major source of morbidity and mortality for developing countries. It is caused by different species of blood flukes (trematode worms) of the genus Schistosoma, the most important of which are S. haematobium which infests the genitourinary tract and S. mansoni and S. japonicum which infest the gastrointestinal tract. The incidence of schistosome-induced male infertility is not known and probably underestimated being overshadowed by the more severe and often fatal urinary and gastrointestinal complications. Several causes may co

Microtubular Dysfunction and Male Infertility.

Microtubules are the prime component of the cytoskeleton along with microfilaments. Being vital for organelle transport and cellular divisions during spermatogenesis and sperm motility process, microtubules ascertain functional capacity of sperm. Also, microtubule based structures such as axoneme and manchette are crucial for sperm head and tail formation. This review (a) presents a concise, yet detailed structural overview of the microtubules, (b) analyses the role of microtubule structures in various male reproductive functions, and (c) presents the association of microtubular dysfunctions w

Sexual dysfunction and male infertility.

Infertility affects up to 12% of all men, and sexual dysfunction occurs frequently in men of reproductive age, causing infertility in some instances. In infertile men, hypoactive sexual desire and lack of sexual satisfaction are the most prevalent types of sexual dysfunction, ranging from 8.9% to 68.7%. Erectile dysfunction and/or premature ejaculation, evaluated with validated tools, have a prevalence of one in six infertile men, and orgasmic dysfunction has a prevalence of one in ten infertile men. In addition, infertile men can experience a heavy psychological burden. Infertility and its as

Oxidative stress and male infertility.

DNA damage, largely owing to oxidative stress, is a leading cause of defective sperm function. High levels of oxidative stress result in damage to sperm DNA, RNA transcripts, and telomeres and, therefore might provide a common underlying aetiology of male infertility and recurrent pregnancy loss, in addition to congenital malformations, complex neuropsychiatric disorders, and childhood cancers in children fathered by men with defective sperm cells. Spermatozoa are highly vulnerable to oxidative stress owing to limited levels of antioxidant defence and a single, limited DNA-damage detection and

Semen proteomics and male infertility.

UNLABELLED: Semen is a complex body fluid containing an admixture of spermatozoa suspended in secretions from the testes and epididymis which are mixed at the time of ejaculation with secretions from other accessory sex glands such as the prostate and seminal vesicles. High-throughput technologies have revealed that, contrary to the idea that sperm cells are simply a silent delivery vehicle of the male genome to the oocyte, the sperm cells in fact provide both a specific epigenetically marked DNA together with a complex population of proteins and RNAs crucial for embryogenesis. Similarly, -omi

[Male infertility: non-surgical therapy].

Infertility is defined as the inability of a couple to conceive after 12 months of unprotected intercourse and affects 15% of couples with male component of 50%. The failure of spermatogenesis can result from hypothalamic, pituitary or testicular disorders although in the majority of cases it remains idiopathic. The diagnostic process includes medical history, semen analysis, hormonal studies, genetic studies and radiological evaluation.Targeted hormonal therapies are available for patients whose infertility is caused by altered levels of androgens, prolactin, or TSH. Main treatments aim to re

Novel markers of male infertility.

Diagnostic tests should detect disease, have prognostic value, and aid in clinical decision making. Nowhere else in laboratory medicine does one have to interpret a subject's results within the dynamic of a couple as in reproductive medicine. Abnormal markers of male reproduction do not necessarily mean sterility, but instead indicate problems with spermatogenesis, sperm maturation, transport through epididymis and ejaculatory duct, or abnormal ejaculatory function. Decades of research suggest that one test will never fit all scenarios and a battery of assays evaluating different aspects of ma

[Azoospermia factor and male infertility].

Azoospermia factor (AZF) microdeletions of the Y chromosome, which occur in 1 - 55% of infertile men, are closely associated with severe spermatogenic failure and represent the most frequent molecular genetic causes of azoospermia and severe oligozoospermia. Researches on AZF and its related genes, approaching the mechanisms of spermatogenic failure at the molecular level, are of great significance for the diagnosis, treatment and prognosis of male infertility. The detection of AZF microdeletions can provide scientific basis for correct diagnosis and reasonable therapy. This article outlines t

[Genetic causes of male infertility].

BACKGROUND: Male infertility is due to genetic factors in 15% of cases. Identification of genetic causes is important for both prognosis and treatment. The article gives an overview of the most frequent genetic causes of male infertility. MATERIAL AND METHOD: The article is based on literature retrieved through a search of Pubmed. RESULTS AND INTERPRETATION: Klinefelter syndrome, 47, XXY, and microdeletion in AFZ (azoospermia factor) c on the long arm of the Y chromosome are the most common genetic causes of male infertility. Both are present in about 10-15% of males with azoospermia. Other

[Microsurgical varicocelectomy for male infertility].

Varicoceles is the most common identifiable cause of male infertility, and varicocele repair is a major means for the treatment of the disease. Conventional techniques of varicocele repair including Palomo operation, open inguinal varicocelectomy and laparoscopic varicocelectomy have a relatively high rate of postoperative hydrocele formation, varicocele recurrence and testicular artery injury. Recently, microsurgical varicocelectomy has gained a worldwide application in the treatment of varicocele, which can preserve the testicular artery and lymphatic vessels ligate all the spermatic but vas

[Androgen receptor and male infertility].

The androgen receptor (AR) mediates androgen action determining male sexual phenotypes and promotion of spermatogenesis. Mutations in the AR cause various degrees of androgen resistance resulting in a range of androgen insensitivity syndromes. A single copy gene in the X chromosome encodes the AR. The gene contains a polymorphic triple repeat sequence [(CAG)n] with 9-36 repeats in the normal population, and displays ethnic dependence. In vitro, there is an inverse correlation between CAG repeat length and AR function. Associations exist between short alleles and prostate cancer in men or clini

Aromatase inhibitors for male infertility.

PURPOSE: Testosterone-to-estradiol ratio levels in infertile men improve during treatment with the aromatase inhibitor, testolactone, and resulting changes in semen parameters. We evaluated the effect of anastrozole, a more selective aromatase inhibitor, on the hormonal and semen profiles of infertile men with abnormal baseline testosterone-to-estradiol ratios. MATERIALS AND METHODS: A total of 140 subfertile men with abnormal testosterone-to-estradiol ratios were treated with 100 to 200 mg. testolactone daily or 1 mg. anastrozole daily. Changes in testosterone, estradiol, testosterone-to-est

Y chromosome and male infertility.

Male factor infertility accounts for about half the cases of couple infertility. In more than 60% of cases the origin of reduced testicular function is unknown but they may have an unidentified genetic anomaly. Microdeletions of the long arm of the human Y chromosome are associated with spermatogenic failure and have been used to define three regions of Yq (AZFa, AZFb and AZFc) that are recurrently deleted in infertile males. Several genes have been identified within this region and have been proposed as candidates for infertility. Many of these genes encode proteins involved in post-transcrip

Genetic contribution to male infertility.

Worldwide, of couples trying for a child, 2-7% fail to conceive. Extensive screening programmes of men attending infertility clinics show that chromosomal and gene disorders make a significant contribution to spermatogenic impairment. It appears that an orderly genome is essential for normal germ cell development, since numerical and structural chromosome abnormalities are found in association with germ cell breakdown. The most recent research indicates that genes on the Y chromosome and autosomes are involved in spermatogenic control.

Transrectal ultrasound in male infertility.

In the evaluation of the subfertile man who has severe oligospermia or azoospermia associated with a low volume ejaculate, the high resolution transrectal ultrasound (TRUS) has, in recent years, been a very important non-invasive imaging technique to detect abnormalities in the seminal vesicles, ejaculatory duct and the status of the prostate. We performed TRUS in 30 of these patients from January to December 1994 followed by scrotal exploration with vasogram using methylene blue, inspection of the epdidymis for dilatation and fibrosis together with testicular biopsy to evaluate testicular spe

Preventable causes of male infertility.

The initial evaluation of the infertile man includes a diligent search for testicular factors, gonadotoxins, and coital factors so as to identify existing causes of impaired fertility and to prevent further dimunition in fertility. There are also prophylactic measures in the treatment of all men that can prevent future infertility, such as prompt correction of cryptorchidism, testicular torsion, genital infection, and adolescent varicocele and proper precautions to limit occupational, medical, and recreational gonadotoxins.
