Unilateral cataplexy associated with systemic lupus erythematosus.

A patient with systemic lupus erythematosus (SLE) developed attacks of unilateral cataplexy precipitated by laughter. Unilateral cataplexy has not been described previously in detail and its association with SLE is unique. The clinical details, investigations, and diagnostic criteria are discussed and a causal relationship between cataplexy and SLE is suggested.

Self-initiated humour protocol: a pilot study with an AI agent.

INTRODUCTION: Non-hostile humour and laughter have been known for therapeutic benefits in an individual's mental health and wellbeing. To this end, we evaluated the Self-Initiated Humour Protocol (SIHP), a new type of self-administrable laughter intervention that utilises spontaneous and self-induced laughter. Rooted in the core principles of the Self-Attachment Technique-in which an individual creates an affectional bond with their childhood self as represented by their childhood photo or personalised childhood avatar-SIHP provides an algorithmic framework for individuals to learn to laugh in

Sounds of emotion: production and perception of affect-related vocal acoustics.

In his writing Darwin emphasized direct veridical links between vocal acoustics and vocalizer emotional state. Yet he also recognized that acoustics influence the emotional state of listeners. This duality-that particular vocal expressions are likely linked to particular internal states, yet may specifically function to influence others-lies at the heart of contemporary efforts aimed at understanding affect-related vocal acoustics. That work has focused most on speech acoustics and laughter, where the most common approach has been to argue that these signals reflect the occurrence of discrete

Vocal display rules in everyday life: when is it appropriate to laugh, sigh or scream?

The appropriateness of common nonverbal vocalizations is guided by social norms, known as display rules. This preregistered, 7-day experience-sampling study examined how perceived appropriateness varies across situations. A total of 142 adults completed three surveys per day (2670 surveys total; 3821 vocalizations), rating appropriateness alongside situational cues and psychological situation characteristics. Laughter and sighs were most often judged appropriate, whereas screams and retches were least. Appropriateness was higher at home and in social places (e.g. parties) than in work/school o

Treatable fluctuating mental impairment in a patient with Bardet-Biedl syndrome.

Bardet-Biedl syndrome (BBS) is an autosomal recessive disorder characterized by rod-cone dystrophy, polydactyly, central obesity, mental retardation, and hypogonadism. Although many organs are involved in BBS, hyperammonemia caused by portal hypertension has been reported previously in only a single patient. We describe the second such patient with BBS and hyperammonemia, associated with fluctuating mental impairment. The patient was a 17-year-old boy with BBS. Esophageal, gastric, and rectal varices and mild hepatic dysfunction started to develop at 5 years of age. A liver biopsy showed dilat

[Angelman syndrome: a frequently undiagnosed cause of mental retardation and epilepsy. Case report].

The authors describe the case of a typical Angelman syndrome patient. The proband presents developmental delay, mental retardation, macrostomia, wide-spaced teeth, seizures, absent speech, jerky gait, and paroxysms of laughter. The cytogenetic and molecular studies showed a maternal deletion of 15q11q13. These results are in agreement with the clinical diagnosis of Angelman syndrome.

Angelman syndrome.

Angelman syndrome is characterized by severe mental retardation, absence of speech, bursts of laughter, ataxia, seizure disorder and facial dysmorphism. This report describes the first 3 children with Angelman syndrome from Bahrain. The diagnosis was based on clinical features and confirmed by the presence of microdeletion of 15q11q13 using fluorescence in situ hybridization.

Anorgasmia and cataplexy.

Ten married women with cataplexy were found to be rarely orgasmic. Cataplexy is characterized by recurrent episodes of short-lived generalized muscle paralysis. It is precipitated by arousing emotional precipitants such as laughter, fear, and anger. Patients learn to avoid situations exposing them to these precipitants. It is suggested that anorgasmia is a consequence of this general tendency to avoid arousal.

Pseudocataplexy: gelastic-atonic seizures.

Gelastic-atonic seizures are characterized by laughing and then loss of muscle tone. They are difficult to differentiate from laughter-induced cataplexy and may be indicative of Niemann-Pick disease Type C or variants. We studied an 11-year-old girl who had gelastic-atonic seizures that were triggered by laughing. The nature of her attacks was established by ictal EEG ambulatory recordings at home. There was no evidence of Niemann-Pick disease.

"Did You Climax or Are You Just Laughing at Me?" Rare Phenomena Associated With Orgasm.

INTRODUCTION: The study of the human orgasm has shown a core set of physiologic and psychological symptoms experienced by most individuals. The study of normal sheds light on the abnormal and has spotlighted rare physical and psychological symptoms experienced by some individuals in association with orgasm. These phenomena are rare and, as is typical of rare phenomena, their documentation in the medical literature is largely confined to case studies. AIM: To identify peri-orgasmic phenomena, defined as unusual physical or psychological symptoms subjectively experienced by some individuals as

Relaxed vigilance: humor as a regulatory capacity in care systems.

Sustaining vigilance over time presents a regulatory challenge for care systems operating under conditions of uncertainty. When vigilance becomes chronically elevated, it may narrow attention, erode relational trust, and reduce adaptive capacity. This Perspective introduces relaxed vigilance, a regulatory condition in which vigilance remains active while cognitive flexibility and relational coordination are preserved. The article examines humor, particularly laughter, as a biologically and socially grounded process that may signal or support this regulatory balance. Drawing on literature from

[Hypothalamic hamartoma: detailed presentation of a case].

We describe a seven year old child with a hypothalamic hamartoma. Classical symptoms of hypothalamic hamartoma include gelastic epileptic laughter, precocious puberty, aggressiveness, and progressively worsening epilepsy. After a normal first few years of life, this case presents all these symptoms except the precocious puberty. He has a markedly morbid personality disorder: he assaults strangers and relatives, bites people, spits in their faces unpredictably, is coprolalic and coprophagic, has gelastic laughter, puts pencils, erasers, and other non-comestible objects in his mouth, chews and i

Effective use of sertraline for pathological laughing after severe vasospasm due to aneurysmal subarachnoid hemorrhage: case report.

Pathological laughing, one subgroup of psuedobulbar affect, is known as laughter inappropriate to the patient's external circumstances and unrelated to the patient's internal emotional state. The authors present the case of a 76-year-old woman with no significant medical history who experienced pathological laughing after subarachnoid hemorrhage (SAH) due to rupture of an aneurysm, which was successfully treated with craniotomy for aneurysm clipping. In the acute stage after the operation she suffered from severe vasospasm and resulting middle cerebral artery territory infarction and conscious

Gelastic seizures associated with hypothalamic hamartomas. An update in the clinical presentation, diagnosis and treatment.

Gelastic seizures are epileptic events characterized by bouts of laughter. Laughter-like vocalization is usually combined with facial contraction in the form of a smile. Autonomic features such as flushing, tachycardia, and altered respiration are widely recognized. Conscious state may not be impaired, although this is often difficult to asses particularly in young children. Gelastic seizures have been associated classically to hypothalamic hamartomas, although different extrahypothalamic localizations have been described. Hypothalamic hamartomas are rare congenital lesions presenting with the

Rectus sheath haematoma: a rare presentation of non-contact strenuous exercises.

Rectus sheath haematoma (RSH) is a well-documented but uncommon clinical condition. It is usually a self-limiting condition but can present as a life-threatening emergency. RSH after non-contact vigorous exercise is unknown. Two such cases secondary to yoga and laughter therapy sessions, respectively, are reported. One of them required surgical intervention, whereas the other was successfully treated conservatively.

Aggressive behavioral outbursts as an atypical presentation of giant pediatric trigeminal schwannoma: illustrative case.

BACKGROUND: Trigeminal schwannomas are rare intracranial tumors that predominantly occur in adults and only exceptionally present in the pediatric population. Behavioral disturbances, particularly pathological laughter and impulsive dysregulation, are uncommon neurological manifestations and typically indicate disruption of supranuclear or brainstem networks by an underlying structural lesion. Their association with trigeminal schwannoma remains exceedingly rare and poorly characterized. OBSERVATIONS: A 13-year-old boy presented with progressive behavioral disturbances, including pathological

Temporal lobe malacia as a rare cause of gelastic seizure: A case report.

BACKGROUND: Gelastic seizure (GS) is a rare type of epilepsy that most commonly appears in patients with hypothalamic hamartoma. It is rarely associated with other types of brain damage. This particular type of epilepsy is relatively rare and has few links to other brain lesions. Temporal lobe malacia is mostly caused by cerebral infarction or cerebral hemorrhage, which can lead to seizures. We report a case of GS in a woman with temporal lobe malacia which was reported for the first time in the literature. CASE SUMMARY: A 73-year-old female, diagnosed case of GS, presented with repetitive st

The Brain Correlates of Laugh and Cataplexy in Childhood Narcolepsy.

UNLABELLED: The brain suprapontine mechanisms associated with human cataplexy have not been clarified. Animal data suggest that the amygdala and the ventromedial prefrontal cortex are key regions in promoting emotion-induced cataplectic attacks. Twenty-one drug-naive children/adolescent (13 males, mean age 11 years) with recent onset of narcolepsy type 1 (NT1) were studied with fMRI while viewing funny videos using a "naturalistic" paradigm. fMRI data were acquired synchronously with EEG, mylohyoid muscle activity, and the video of the patient's face. Whole-brain hemodynamic correlates of (1)
