Essence of Support: A Phenomenological Study of Integrating Formal and Natural Supports to Enhance Quality of Life for Saudi Young Adults With Intellectual Disability.

BACKGROUND: Integrated supports are vital for quality of life, yet experiences in Saudi Arabia remain under-explored. This study explored how young adults with intellectual disability and their families navigate the integration of formal and natural support systems. METHODS: Using hermeneutic phenomenology, data were collected through initial and follow-up semi-structured interviews and two focus groups. The sample comprised 17 participants, including 10 young adults with intellectual disability living with their families and seven family members. RESULTS: Two main themes emerged: the critic

Emergency Department management of rib fractures.

Rib fractures resulting from blunt thoracic trauma are often associated with life-threatening complications of injury to cardiorespiratory systems. Given the risk for morbidity and mortality, the emergency clinician must be swift and thorough in diagnosing and managing these injuries. Society guidelines have been published to assist in determining best-practice approaches to pain control, imaging, and treatment. This issue reviews the recent studies and evidence for multimodal pain control, decision tools for diagnostic imaging, ventilatory support, and operative fixation. Scoring systems to d

Extracorporeal circulatory systems in the interhospital transfer of critically ill patients: experience of a single institution.

BACKGROUND AND OBJECTIVES: Critically ill patients with acute circulatory failure cannot be moved to other institutions unless stabilized by mechanical support systems. Extracorporeal heart and lung assist systems are increasingly used as a bridge to end-organ recovery or transplantation, and as an ultimate rescue tool in cardiopulmonary resuscitation. PATIENTS AND METHODS: From July 2001 to April 2008, we had 38 requests for extracorporeal support for interhospital transfer carried out by the air medical service. Respiratory failure was present in 29 patients, who were provided with pumpless

[Diagnosis and surgical treatment of postprostatectomy stress incontinence: recommendation of the working group Urologische Funktionsdiagnostik und Urologie der Frau].

Today, for the surgical treatment of postprostatectomy incontinence, several treatment options are available, e.g., adjustable and functional sling systems, artificial sphincter, bulking agents, and balloons. However, no recommendations in terms of specific diagnostic tools and differentiated treatment options for everyday life are available. Our aim is to provide some clinically relevant recommendations for the necessary diagnostic workup and different treatment options of postprostatetectomy incontinence to support clinical decisions in everyday life. Treatment selection should be based on c

"An illustrative socio-biological cascade of the concentric model for health-bound networks: social determinants, social support and biological responses".

INTRODUCTION: Health has been associated with social support. Supported patients with complex health issues have a better chance of living healthier, longer lives and enjoying a better quality of life. Research indicates that physical, mental, and emotional health are connected to both biological and social determinants. METHODS: This paper proposes a conceptual, society-to-biology model, the Concentric Model for Health-Bound Networks, that explores the complexity of social support and integrates the foundational principles of systems biology and systems thinking. This transdisciplinary model

Preventing falls: the use of machine learning for the prediction of future falls in individuals without history of fall.

Nowadays, it becomes of paramount societal importance to support many frail-prone groups in our society (elderly, patients with neurodegenerative diseases, etc.) to remain socially and physically active, maintain their quality of life, and avoid their loss of autonomy. Once older people enter the prefrail stage, they are already likely to experience falls whose consequences may accelerate the deterioration of their quality of life (injuries, fear of falling, reduction of physical activity). In that context, detecting frailty and high risk of fall at an early stage is the first line of defense

Artificial intelligence-powered predictive tools to improve end-of-life decision-making: mini-review.

BACKGROUND: Uncertainty around a patient's prognosis at the end of life remains a major barrier to timely palliative-care involvement and alignment of treatments with patient goals. Artificial intelligence (AI)-based tools have recently emerged to provide structured mortality predictions and identify patients at risk of deterioration to support clinical decision-making. OBJECTIVE: This mini-review summarises recent literature evaluating AI-based prognostic and decision-support tools in end-of-life and palliative care, focusing on predictive accuracy, early implementation outcomes, communicati

Psychological Aspects of Cardiac Care and Rehabilitation: Time to Wake Up to Sleep?

Psychological and psychosocial factors have long been linked to cardiovascular disease. These psychosocial factors, including low socioeconomic status, social support/isolation, stress and distress, personality, and sleep disturbance increase risk of cardiovascular events and negatively impact quality of life. These factors may have direct effects on cardiovascular disease via immune or neuroendocrine pathways, or more indirect effects, by, for example, limiting adherence to recommended therapies and cardiac rehabilitation. Most psychosocial risk factors can be assessed relatively easily using

Conceptual modelling for life sciences based on systemist foundations.

BACKGROUND: All aspects of our society, including the life sciences, need a mechanism for people working within them to represent the concepts they employ to carry out their research. For the information systems being designed and developed to support researchers and scientists in conducting their work, conceptual models of the relevant domains are usually designed as both blueprints for a system being developed and as a means of communication between the designer and developer. Most conceptual modelling concepts are generic in the sense that they are applied with the same understanding across

Light intensity and production parameters of phytocenoses cultivated on soil-like substrate under controlled [correction of controled] environment conditions.

To increase the degree of closure of biological life support systems of a new generation, we used vermicomposting to involve inedible phytomass in the intra-system mass exchange. The resulting product was a soil-like substrate, which was quite suitable for growing plants (Manukovsky et al. 1996, 1997). However, the soil like substrate can be regarded as a candidate for inclusion in a system only after a comprehensive examination of its physical, chemical, and other characteristics. An important criterion is the ability of the soil-like substrate to supply the necessary mineral elements to the

Adaptation to Climate Change and Climate Protection in Geriatric Care for Older People: Strategies, Infrastructure, and Good Practice Examples.

Anthropogenic climate change presents significant health challenges for older people with care needs across all care settings, from private homes and supported living arrangements to institutional care facilities. Those particularly at risk include individuals with limited mobility, multimorbidity, or social isolation. Care and support systems therefore need to implement measures for both climate change mitigation and adaptation. Mitigation encompasses reducing energy and water consumption, minimising waste, decreasing reliance on fossil fuels, managing chemicals, and strengthening political a

Invisible Care in a Low-Income Setting: Reflecting on Family Caregiver Fatigue in Dementia Care.

Dementia care is largely provided by family members, particularly in low-income settings where formal support systems are limited or absent. As a result, family caregivers face sustained emotional, physical, and psychological demands that place them at high risk of caregiver fatigue. Although caregiver burden in dementia care is well documented, less attention has been given to how caregiver fatigue is experienced, normalized, and rendered invisible within everyday life and health systems. This reflective paper draws on personal experience as a family caregiver for a relative with dementia in

Exploring the evolution of US medical students' social support networks across the continuum: a cross-sectional group interview study.

Perceived social support - feeling cared for and having access to a reliable network - has been linked to reduced stress and burnout, and greater quality of life among medical students. Yet, how perceived support systems evolve throughout medical school remains little explored. We examined how medical students' perceived social support systems shift during medical school adaptively to students' changing academic and emotional needs. Between January and May 2023, we conducted six hour-long group interviews with a cross-sectional convenience sample of 50 medical students (graduating Classes of 2

Perceived Social Support Attenuates the Association between Stress and Health-Related Quality of Life among Adults Experiencing Homelessness.

Health-related quality of life (HRQoL) is defined as a multidimensional subjective assessment of one's physical and mental health. Homelessness is associated with numerous stressors that can reduce HRQoL. Social support is defined as the availability of individuals, or resources provided by individuals, to cope with stress. Interpersonal social support may be important in buffering HRQoL from the negative implications of stress. Here, we examine this association in a marginalized group known for high rates of physical and mental health comorbidities: adults experiencing homelessness. Participa

[Focusing on profound autism: validity of clinical taxonomy and management of treatment-resistant symptoms].

The broad-spectrum diagnostic framework for autism spectrum disorder (ASD) in the Diagnostic and Statistical Manual of Mental Disorders (5th ed.) tends to obscure the specific medical and support needs of individuals with severe impairments. In response, the Lancet Commission on the future of care and clinical research in autism proposed the concept of "profound autism" in 2021, referring to individuals with ASD who are aged 8 years or older, have severe intellectual disabilities (IQ < 50) and/or minimal or completely absent verbal communication abilities, and require round-the-clock (24-hour)

Rationale and Initiative of the Impella in Cardiac Surgery (ImCarS) Register Platform.

OBJECTIVES: Cardiac support systems are being used increasingly more due to the growing prevalence of heart failure and cardiogenic shock. Reducing cardiac afterload, intracardiac pressure, and flow support are important factors. Extracorporeal membrane oxygenation (ECMO) and intracardiac microaxial pump systems (Impella) as non-permanent MCS (mechanical circulatory support) are being used increasingly. METHODS: We reviewed the recent literature and developed an international European registry for non-permanent MCS. RESULTS: Life-threatening conditions that are observed preoperatively often
