Lorazepam treatment of acute and chronic catatonia in two mentally retarded brothers.

BACKGROUND: Acute catatonia of the retarded type responds rapidly to treatment with lorazepam. There is little information about (1) the efficacy of benzodiazepines in patients with longstanding catatonic syndrome; (2) the effect of longer-term lorazepam therapy in catatonic patients who do not respond to a brief trial of the medication; and (3) the development of catatonic syndromes in family members. METHOD: We gave lorazepam to two mentally retarded brothers with catatonia. One had become catatonic 2 weeks before starting treatment while the other had been hospitalized in a catatonic state

Disposition of lorazepam in Gilbert's syndrome: effects of fasting, feeding, and enterohepatic circulation.

The effects of fasting and feeding of a high-carbohydrate, low-fat diet on the glucuronidation and enterohepatic circulation (EHC) of lorazepam were examined in seven healthy men (age 18-30 years) and seven matched patients with Gilbert's syndrome. A simultaneous intravenous/oral dosing regimen was used, with half of each group receiving treatment with neomycin and cholestyramine (neo/chol) to block the EHC of the drug. Feeding increased the clearance of free lorazepam from 10.96 +/- 0.56 (mean +/- SD) to 14.11 +/- 1.21 mL/min/kg (P = 0.05) in patients with Gilbert's syndrome when examined in

Acute effects of triazolam and lorazepam on human learning, performance and subject ratings.

Triazolam (0,0.125, 0.25, 0.5, and 0.75 mg/70 kg) and lorazepam (0, 1, 2, 4 and 6 mg/70 kg) were compared in eight healthy men by using a double-blind, crossover design. Triazolam was chosen for study to examine experimentally whether it produces greater behavioral impairment than other benzodiazepines as reported previously. Lorazepam was chosen as the comparison drug because of its well documented behavioral effects and because it has a short metabolic half-life and no active metabolite, which eliminates the possible confounding effects of drug accumulation. Drug effects were assessed before

Subjective and behavioral effects of diphenhydramine, lorazepam and methocarbamol: evaluation of abuse liability.

The effects of orally administered placebo, diphenhydramine, lorazepam, methocarbamol and placebo were studied in volunteers with histories of recreational substance abuse including sedative/hypnotics. Placebo, diphenhydramine (100, 200 and 400 mg), lorazepam (1 and 4 mg) and methocarbamol (2.25 and 9 g) were tested in a randomized, double-blind crossover study using 14 subjects. Psychomotor and cognitive performance and subject- and observer-rated responses were measured daily before and for 5.5 hr after drug administration. The results showed that each of the drugs exhibited a different prof

Task-dependent development of cross-tolerance to psychomotor effects of lorazepam in man.

Development of cross-tolerance between lorazepam and four other benzodiazepines (BZ) was studied in two trials by measuring objective psychomotor performance and by subjective assessments. In trial I, nitrazepam 10 mg (NZ), temazepam 20 mg (TZ) or placebo were administered to 14 healthy students for 10 consecutive nights. After each pretreatment, psychomotor impairment by the challenge dose of lorazepam 3 mg (LZ) was measured. In trial II, responses to LZ after pretreatment with diazepam 5 mg (DZ) (8 subjects) or alprazolam 0.25 mg (AZ) (10 subjects), both t.i.d., were compared to LZ responses

A comparison of the effects of lorazepam with those of propranolol on experimentally-induced anxiety and performance.

In a double-blind cross-over study the effects of propranolol (80 mg) and of lorazepam (1 or 2.5 mg) were assessed in normal student volunteers using a number of performance tests and mood-rating and bodily symptom questionnaires. Drug effects on experimentally-induced anxiety were also studied. The high dose of lorazepam impaired performance in digit-symbol substitution, symbol copying and verbal learning tests, and increased subjects' ratings of dizziness. Both lorazepam and propranolol increased simple reaction time. Lorazepam but not propranolol increased ratings of sedation. Although the

Evaluation of anxiolytic and amnesic effects of intramuscular lorazepam as a pre-operative medication.

In a double-blind investigation in 21 patients undergoing minor plastic surgery the effects of 4 mg lorazepam or a mixture of 20 mg papaveretum/400 mug hyoscine given intramuscularly as premedicants were studied. Lorazepam produced superior preoperative and post-operative anterograde amnesia, with respect to both incidence and duration. There was no difference in the pre-operative anxiolytic and soporofic effects of the two regimens and post-operatively, analgesic requirements and the incidence of vomitting and restlessness were the same in both groups of patients.

Characterization of Extended-Release Lorazepam: Pharmacokinetic Results Across Phase 1 Clinical Studies.

PURPOSE/BACKGROUND: Once-daily extended-release (ER) lorazepam was developed to reduce fluctuations in plasma levels compared with lorazepam immediate-release (IR) for short-term anxiety relief. Here we report a series of phase 1 randomized, open-label, multiperiod crossover studies characterizing ER lorazepam pharmacokinetics and safety in healthy adults. METHODS/PROCEDURES: These phase 1 studies assessed the pharmacokinetics of ER lorazepam administered: (study 1) 3 mg once daily versus IR lorazepam 1 mg 3 times a day (TID; every 8 hours), (study 2) with or without food, and (study 3) intac

Front-loaded diazepam versus lorazepam for treatment of alcohol withdrawal agitated delirium.

BACKGROUND: Front-loaded diazepam is used to rapidly control agitation in patients with severe alcohol withdrawal syndrome (AWS). Our institution began using front-loaded lorazepam in August 2017 secondary to a nation-wide shortage of intravenous (IV) diazepam. Currently, there are no studies comparing lorazepam to diazepam for frontloading in severe AWS. METHOD: Retrospective cohort study of all adults presenting to the emergency department with a diagnosis of AWS and prescribed the institution's alcohol withdrawal agitated delirium protocol 8 months pre and post shortage of IV diazepam were

Use of lorazepam for analgosedation during mechanical ventilation in pediatric intensive care.

INTRODUCTION: Lorazepam has been used off-label for analgosedation in pediatric intensive care units (PICU) as an alternative to midazolam. While its intermediate duration of action makes it suitable for continuous sedation, there is limited evidence to guide dosing in children. This study illustrates how pharmacokinetic modeling and extrapolation principles can be used to (1) identify regimens that maintain the desired analgosedation levels and (2) optimize the design of a prospective protocol in children requiring mechanical ventilation. METHODS: Pharmacokinetic data and COMFORT-B scores fr

Lorazepam and Survival in Asian Patients with Pancreatic Cancer: A Retrospective Cohort Study.

PURPOSE: Lorazepam is frequently used in pancreatic cancer patients receiving chemotherapy for its antiemetic properties and to mitigate psychological distress. However, N-unsubstituted benzodiazepines like lorazepam may adversely impact pancreatic cancer progression by stimulating fibrosis and inflammatory signaling. This study aimed to retrospectively compare the survival rates of pancreatic cancer patients exposed to lorazepam with those who were not exposed to real-world clinical practice. METHODS: Data were retrospectively reviewed from patients aged ≥ 18 years with pathologically confirm

Comparative effectiveness of lorazepam versus diazepam for acute catatonia: A retrospective analysis.

OBJECTIVE: Catatonia is a neuropsychiatric disorder that can respond rapidly to treatment with benzodiazepines. Historically, lorazepam is the most used agent for "the lorazepam challenge test" (LCT), but comparative evidence for alternative benzodiazepines remains limited. A temporary intravenous (IV) lorazepam shortage within our health system created a natural experiment by restricting access to lorazepam. We use this natural experiment to compare a single dose of IV diazepam vs. IV lorazepam for the acute treatment of catatonia in a LCT. METHODS: Retrospective comparative effectiveness st

Lorazepam Versus Diazepam in the Management of Emergency Department Patients With Alcohol Withdrawal.

STUDY OBJECTIVE: Alcohol withdrawal is a common emergency department (ED) presentation. Although benzodiazepines reduce symptoms of withdrawal, there is little ED-based evidence to assist clinicians in selecting appropriate pharmacotherapy. We compare lorazepam with diazepam for the management of alcohol withdrawal to assess 1-week ED and hospital-related outcomes. METHODS: From January 1, 2015, to December 31, 2018, at 3 urban EDs in Vancouver, Canada, we studied patients with a discharge diagnosis of alcohol withdrawal. We excluded individuals presenting with a seizure or an acute concurren

Single dose phenobarbital in addition to symptom-triggered lorazepam in alcohol withdrawal.

OBJECTIVE: The purpose of this study was to evaluate the safety and efficacy of a single parenteral dose of phenobarbital in addition to symptom-triggered lorazepam for the acute management of alcohol withdrawal syndrome (AWS). METHODS: This was a retrospective chart review of adult patients who presented to the Emergency Department with moderate or severe symptoms of alcohol withdrawal. Patients were included if they received at least 4 mg of lorazepam through the hospital's Alcohol Withdrawal Order Set on hospital day one. Patients who received a single parenteral dose of phenobarbital on h

Lorazepam, fluoxetine and packing therapy in an adolescent with pervasive developmental disorder and catatonia.

Packing therapy is an adjunct symptomatic treatment used for autism and/or catatonia. Here, we report the case of a 15-year-old boy with pervasive developmental disorder who developed catatonia. At admission, catatonic symptoms were severe and the patient required a feeding tube. Lorazepam up to 15 mg/day moderately improved the catatonic symptoms. On day 36 we added fluoxetine and on day 62 we added packing therapy (twice per week, 10 sessions). After three packing sessions, the patient showed a significant clinical improvement (P<0.001). At discharge (day 96), he was able to return to his sp

A double-blind trial of gabapentin versus lorazepam in the treatment of alcohol withdrawal.

INTRODUCTION: Some anticonvulsants ameliorate signs and symptoms of alcohol withdrawal, but have an unacceptable side effect burden. Among the advantages of using anticonvulsant agents in this capacity is their purported lack of interaction with alcohol that could increase psychomotor deficits, increase cognitive impairment, or increase intoxication. The aim of this study was to evaluate alcohol use and symptom reduction of gabapentin when compared with lorazepam in the treatment of alcohol withdrawal in a double-blinded randomized clinical trial. METHODS: One hundred individuals seeking outp

Pharmacokinetic and pharmacodynamic analysis of sedative and amnesic effects of lorazepam in healthy volunteers.

This study describes for the first time the pharmacokinetic and pharmacodynamic modeling of the psychomotor and amnesic effects of a single 2-mg oral dose of lorazepam in healthy volunteers. Twelve healthy volunteers were included in this randomized, double-blinded, placebo-controlled two-way crossover study. The effect of lorazepam was examined for a battery of tests that explored mood, vigilance, psychomotor performance, and memory. The pharmacokinetic and pharmacodynamic modeling of these tests was performed using the indirect response model. Vigilance and psychomotor performance were signi
