OBJECTIVE: Extrapulmonary involvement by Pneumocystis carinii (PC) in acquired immunodeficiency syndrome (AIDS) patients is uncommon. Our purpose is to describe three AIDS patients with low cluster differentiation cell (CD4) counts, who were receiving aerosol pentamidine prophylaxis, and who had large noncalcified hilar and mediastinal lymphadenopathy.
MATERIALS AND METHODS: Chest radiography, CT, and clinical and laboratory records of three AIDS patients who had radiographic evidence of mediastinal and hilar lymphadenopathy were retrospectively reviewed.
RESULTS: Symptoms were quite similar

Thymomas are common primary mediastinal neoplasms. They can be benign or malignant. The diagnosis of malignant thymomas is based on capsular invasion, cellular atypia and distant metastases. We present a case of invasive malignant thymoma as diagnosed on PET and CT scan with histologic correlation and review of literature. A seventy year old Asian man presented with cough and dyspnea. A CT scan showed an anterior mediastinal mass, and a PET scan showed a hypermetabolic mass in the corresponding location. Resection of the mass revealed a minimally invasive thymoma based on evidence of capsular

Patients with retroperitoneal or mediastinal neoplasms of gonadal tissue type and clinically normal testes pose interesting therapeutic dilemmas. Three major groups of patients are recognized: (1) tumor is primary extragenital gonadal neoplasm; (2) occult microscopic primary neoplasm in a testicle with metastases; (3) area of fibrosis in a testicle representing local regression of tumor, but with distant metastases. Review of case reports often indicates lack of data sufficient for classification. Every effort should be made to group patients into proper categories, since treatment will vary.

Solitary plasmacytic lymphomas are rarely described in the lung. Generally, localized neoplasms develop into mediastinal or pulmonary lymphnodes and are hard to differentiate from other lung neoplasms. An involvement of the chest wall in multiple myeloma is not rare, and is generally associated with other skeletal localizations. A secreting solitary plasmacytoma of the ribs is rather uncommon: a personal review of the literature has showed, in the last ten years, only two cases of these tumors, secreting IgG lambda light chains. In this study we report a case of solitary plasmacytoma of the ch

Thymic carcinoma is an epithelial tumor derived from thymic epithelial cells. Thymic tumors may be associated with other simultaneous and/or metachronous extra-thymic tumors (e.g., lung cancer). Here, we report a case of simultaneous surgical management of lung and mediastinal neoplasm together with a review of the literature. During radiological follow-up for prostate and colorectal cancer, an 82-year-old man was diagnosed with lung cancer with simultaneous mediastinal suspected neoplasm. Both were surgically removed with a single intervention performed via a uniportal video-assisted thoracic

Intra-abdominal desmoplastic small round cell tumor (DSRCT) is a recently described, rare neoplasm that usually occurs in male adolescents, has a poor prognosis, and displays a typical clinicopathological profile. This aggressive neoplasm exhibits a predominantly intra-abdominal serosal pattern, with frequent pelvic extension, less frequent retroperitoneal involvement, and rare pulmonary and mediastinal spread. We report on the autopsy findings of a 9-year-old male who presented with a biopsy-proven DSRCT involving the abdomen, retroperitoneum, and scrotum and additional radiologic studies sug

Ectopic pancreatic tissue presenting as large anterior mediastinal mass is exceedingly rare and poses a significant diagnostic challenge, which could mimic common malignancies such as thymoma, teratoma or lymphoma. We report a case of a 49-year-old female who presented with pleuritic chest pain, new-onset atrial fibrillation with rapid ventricular response and nonsustained ventricular tachycardia. Computer tomography pulmonary angiogram (CTPA) identified a large peripherally enhancing anterior mediastinal mass with extensive central necrosis. The lesion exerted a significant mass effect, compr

INTRODUCTION: Amyloidosis is characterized by extracellular protein fibril deposition. Light-chain (AL) amyloidosis may be associated with plasma cell dyscrasias. Isolated mediastinal involvement is exceptionally rare and may mimic malignant tumors.
CASE PRESENTATION: A 69-year-old woman presented with a progressively enlarging anterior chest wall mass caused by an invasive mediastinal lesion. Biopsy revealed amyloid deposition. She underwent bilateral thoracotomy, sternectomy, en-bloc chest wall resection, and reconstruction. Final pathology confirmed AL amyloidosis associated with a kappa-r

A perivascular epithelioid cell neoplasm (PEComa) in the chest is rare, let alone in the mediastinum and lung. A 63-year-old man was admitted to our hospital with chest pain for more than 2 months and was found to have an opacity in his mediastinum and lung for 3 weeks. Enhanced chest computed tomography (CT) revealed a mass in both the left upper lobe and central anterior mediastinum. To identify the disease, a CT-guided percutaneous transthoracic needle biopsy of the upper left lung lesions was performed. The pathology result was consistent with epithelioid angiomyolipoma/PEComa. After a sta

BACKGROUND: The superior vena cava syndrome occurs when extrinsic compression or intraluminal occlusion impedes blood flow through this vessel. The most common underlying cause is a malignant neoplasm, especially a bronchogenic carcinoma. This article describes the cutaneous findings of this disorder.
OBSERVATIONS: Among the earliest and most prominent features are numerous, dilated, vertically oriented, and tortuous cutaneous venules or veins above the rib cage margin. Recognition of this cutaneous sign allowed us to make a diagnosis of lung cancer in several patients. Other features include

We describe 2 cases of malignant melanotic epithelioid renal neoplasms bearing TFE3 gene fusions. Both neoplasms occurred in children (an 11-y-old boy and a 12-y-old girl), and presented with disseminated metastatic disease including mediastinal and mesenteric adenopathy. Both neoplasms featured sheets of epithelioid cells with clear to finely granular eosinophilic cytoplasm set in a branching capillary vasculature. The neoplastic cells contained variable amounts of finely brown pigment confirmed to be melanin by histochemical stains. By immunohistochemistry, the neoplastic cells labeled for m

Teratomas are the most common congenital neoplasm. Fetal intracranial teratomas generally are large solid/cystic tumors that often completely replace normal brain tissue. Mediastinal teratomas are uncommon and rarely result in nonimmune hydrops fetalis. We describe two unusual presentations of fetal teratoma. The first is a fetus with massive hydrocephalus and marked facial deformities that were caused by a small intracranial teratoma. The second is a fetus with a mediastinal teratoma associated with non-immune hydrops fetalis. In both cases, compression by the mass resulted in lethal sequelae

Primary carcinoid tumors of the mediastinum were described for the first time in 1972 as thymic carcinoids. Our experience with 16 patients who underwent diagnostic and surgical procedures at the Mayo Clinic is presented. All of these patients had mediastinal carcinoid. The surgical procedures included node biopsy, anterior mediastinotomy (Chamberlain), median sternotomy and posterior lateral thoracotomy. Complete resection was possible in 9 (56.3%) patients, 3 (18.7%) had partial removal (debulking), and 4 (25%) had diagnostic biopsies only. The operative morbidity was 25%. There were no post

Synovial sarcoma is a soft tissue sarcoma. Its occurrence as a primary mediastinal neoplasm is very rare. We report a case of mediastinal synovial sarcoma in a 14-year-old girl with exertive dyspnoea at presentation. On physical examination, there was a superior vena cava syndrome and café-au-lait spots. Chest X-ray revealed a white left hemithorax with mediastinal deviation to the opposite side. Thoraco-abdomino-pelvic scanner showed a voluminous posterior cervico-mediastinal tissue mass. Computed tomography (CT)-guided biopsy of the mediastino-pleural mass was performed. Histological examina

Mediastinoscopy is often utilized for staging bronchogenic carcinoma. Whether it should be used routinely or selectively is controversial. Fifty-four patients with bronchogenic carcinoma were prospectively assigned to one of two groups based on the presence (group A) or absence (group B) of one or more of the following criteria: (1) central location; (2) mediastinal nodal enlargement; (3) involvement of laryngeal nerve; and (4) noncentral location plus linear stranding toward the hilum. Mediastinoscopy was performed on all patients in group A. Thoractomy for definitive staging was performed on

A woman in her mid-50s with Lynch syndrome and a history of malignant neoplasms was found to have a superior vena cava (SVC) thrombus and a necrotic mediastinal mass on surveillance imaging. The initial biopsy yielded necrotic debris, and the procedure was complicated by a mediastinal hematoma. Due to concern for possible infection, the patient was empirically treated with antibiotics while planning further workup. A broad-range 16S polymerase chain reaction (BRPCR) was performed on a formalin-fixed paraffin-embedded (FFPE) tissue, which was negative for fungi and mycobacteria but revealed DNA

Lymphatic malformation (LM) is a benign neoplasm that consists of abnormally formed lymphatic vessels and cavities. The incidence of LM is 6% among all benign tumors. Mediastinal localization is extremely rare, only case reports in adults are described. Clinical observation of 42-year-old patient with LM of cervical-supraclavicular area and large mediastinal component communicating with thoracic lymphatic duct is presented. The first stage was thoracoscopic clipping of thoracic lymphatic duct, the second one - radical surgery through combined cervico-sterno-thoracotomy.

Two cases of primary mediastinal lymphonode tuberculosis involved right bronchus were summarized in the report. Major clinical symptoms included cough and bloody sputum. Chest enhanced CT scan showed mediastinal lymph node enlargement with ring-shaped enhancement. Bronchoscopy suggested neoplasm in right bronchus. Diagnosis of tuberculosis was confirmed by histopathology in samples from lymph node puncture and brochoscopic biopsy. The clinical symptoms and medical imaging of patients were improved after transbrochoscopic interventional therapy and systemic chemotherapy.

Thymoma is the most common primary malignancy of the anterior mediastinum and the most common thymic epithelial neoplasm, but it is a rare tumor that constitutes less than 1% of adult malignancies. Computed tomography (CT) is currently the imaging modality of choice for distinguishing thymoma from other anterior mediastinal masses, characterizing the primary tumor, and staging the disease. However, magnetic resonance imaging is also effective in evaluating and characterizing anterior mediastinal masses and staging thymoma in patients with contraindications to contrast-material-enhanced CT such

Angiosarcoma is a rare neoplasm, accounting for only 1%-2% of all sarcomas. It occurs most frequently in the skin and soft tissue and rarely in the thoracic region. To our knowledge, a mediastinal angiosarcoma is extremely rare. We report on the surgical resection of a rare case of giant epithelioid angiosarcoma originating in the anterior mediastinum, followed by six courses of adjuvant chemotherapy (doxorubicin + ifosfamide). The patient is alive and asymptomatic 1 year after surgery. As the prognosis for unresectable cases is generally dismal, surgical resection and adjuvant therapy can be
