Metastatic adrenal neuroblastoma in an adult.

BACKGROUND: Neuroblastoma (NB) is a common malignancy in children, but rarely occurs in adults. Accepted unfavourable prognostic factors include age > 1 year, low histologic grade and advanced stage, MYCN amplification, chromosomal aberrations, elevations of neuron specific enolase and lactate dehydrogenase, and increased catecholamine metabolites in urine or serum. In adults, abdomen/retroperitoneum are the primary sites and in children the adrenal gland. CASE REPORT: A 51- year-old man was admitted to our hospital with hypertension and a large right retroperitoneal mass. Clinically, phaeoch

Perinatal factors and risk of neuroblastoma.

Neuroblastoma is a malignancy commonly diagnosed during infancy or early childhood, raising speculation about the role of perinatal factors and risk of disease. Using a case-control design, cases included 155 infants and children aged 0-5 years with histologically confirmed neuroblastoma diagnosed and reported to the New York State Cancer Registry between 1976 and 1987. Controls were randomly selected from the State's Livebirth Registry and were frequency matched to cases on year of birth (n = 310). Medical records of cases were used to verify histology and stage of disease. Data on perinatal

Surgical treatment of neuroblastoma with micrometastasis.

BACKGROUND/PURPOSE: The aim of this study was to define the role of surgery in neuroblastoma with micrometastasis, which is detectable only by the polymerase chain reaction (PCR) method. METHODS: Fifty samples (peripheral blood 9, bone marrow 41) were harvested sequentially from 27 neuroblastoma patients, and the micrometastases were examined using the previously described single-step PCR method. The results were reviewed with the clinical courses. RESULTS: Radical surgery was performed in 9 patients with bone marrow micrometastasis. Event-free survival was obtained in 2 patients with stage

Neurodevelopmental anomalies in children with neuroblastoma.

BACKGROUND: Case reports have associated neuroblastoma, a cancer derived from the embryonal neural crest, with aganglionosis coli and neurofibromatosis type I. The aim of the current study was to test the hypothesis that neuroblastoma is part of a global defect in neurodevelopment. METHODS: Neuropathologic findings from autopsies of children who died of neuroblastoma during the period 1980-1995 at the Children's Hospital of Pittsburgh were reviewed for macroscopic and microscopic abnormalities. As controls, autopsies of children who had died of other primary extracranial cancers over the same

[Cardiomyopathy induced by catecholamines in neuroblastoma].

BACKGROUND: Cardiovascular disorders associated with neural crest tumors are congenital malformations rather than cardiomyopathies. CASE REPORT: A 18 month-old girl developed heart failure one week after discovery of an adrenal neuroblastoma with excessive secretion of catecholamines. Investigation showed dilated cardiomyopathy without myocardial hypertrophy. Besides chemotherapy, the patient was given nitrous derivatives, diuretics and converting enzyme inhibitor. His cardiac condition gradually improved so that partial resection of the tumor was possible 2 years later. CONCLUSIONS: The car

Hypervitaminosis A as a complication of treatment for neuroblastoma.

A case of hypervitaminosis A (HVA) as a complication of therapy for stage-IV neuroblastoma is presented. The patient was randomized to a trial of 13-cis -retinoic acid (a vitamin A-related compound) after completing routine chemotherapy. This acid was given as a means of maturing potential minimal residual disease. A routine follow-up bone scan revealed areas of increased activity, initially along the midshaft of the right ulna and subsequently bilaterally, which were ultimately found to be due to HVA. Hypervitaminosis A has not been previously reported in this setting, and awareness of the co

Cytopathology of neuroblastoma, ganglioneuroblastoma and ganglioneuroma.

Four cases of neuroblastomas, two cases of ganglioneuroblastomas and one case of ganglioneuroma diagnosed by fine needle aspiration cytology (FNAC) are presented. Five tumors were in the retroperitoneum and two tumors occurred in the posterior mediastinum. Complete clinical history, physical examination, radiological investigation, laboratory parameters, cytological study of multiple slides aided with special stains and cell block preparation of aspirated material helped in diagnosis of these tumours. Cytohistopathological correlation was carried out from surgically resected specimens and from

[Diagnosis and therapy of olfactory neuroblastoma].

Esthesioneuroblastomas (EN) exhibit problems in early diagnosis and therapy due to their localization at the frontal skull base. Analysis of six cases with EN (four male, two female; average age, 34.8 years) showed atypical initial symptoms, beginning as nasal bleeding, hyposmia and frontal headache. CT scans demonstrated hypo- to isodense tumors at the anterior skull base with extension to the sinuses and orbits. Five patients were operated on by an extranasal approach; one patient required orbital exenteration with later reconstructive surgery of the orbit by a microvascularly adapted forear

Neuroblastoma in a patient with Sotos' syndrome.

Sotos' syndrome, or cerebral gigantism, is a disorder of growth regulation. Tumours have occasionally been reported in children with Sotos' syndrome, but it is uncertain whether this is a coincidence, or whether it is aetiologically related to the underlying disorder of growth. We report a 15 month old child with a paraspinal neuroblastoma and Sotos' syndrome and suggest that children with this condition may be at higher risk for developing tumours than the general population.

Neuroblastoma: the role of MR imaging.

Thirty-five MR examinations were performed in 17 children with neuroblastoma during the course of their diagnostic evaluation and treatment. Findings were confirmed by correlation with the results of other imaging techniques, biopsies, and surgical and autopsy findings. MR accurately detected the lesions and their extent in all patients. It correctly evaluated the possibility of resectability and showed metastases, response to treatment and development of complications. MR appears to be a reliable technique for the diagnosis, staging, and follow-up of children with neuroblastoma.

[An autopsy case of adult neuroblastoma].

The patient was a 31-year-old man with neuroblastoma of the right adrenal gland, which presented at first as metastatic spinal epidural tumor. Catecholamine and its products in the blood and urine and neuron-specific enolase in the blood were increased. Six months after right adrenectomy followed by chemotherapy, the patient died due to cachexia. Autopsy revealed widespread metastasis. By electron microscopy, the centers of the Homer-Wright rosettes were occupied by neuropil-like structures consisting of a number of cytoplasmic processes of tumor cells, but no cored vesicles were found either

Localization procedures in pheochromocytoma and neuroblastoma.

Localization procedures are required in catecholamine-producing tumors after clinical and biochemical confirmation. Computed tomography, ultrasound and/or 131I-metaiodobenzylguanidine (131I-MIBG) scintigraphy was performed in patients with pheochromocytoma, neuroblastoma and metastases of carcinoid tumors. Whereas computed tomography and ultrasound reflect morphological abnormalities, adrenomedullary scintigraphy depends on hormonal activity and other factors. 131I-MIBG scintigraphy has the advantage of detecting extraadrenal, multilocular and malignant pheochromocytomas. Especially small lesi

Chromosome analysis of a human neuroblastoma.

Samplings of tumor cells from a patient with Stage IV neuroblastoma were analyzed for chromosome constitution. Chromosome preparations of the tumor cells from a bone marrow sample were compared to preparations of a solid metastatic tumor after growth in the nude mouse host or followed by culture. Six separate chromosome studies were done. The tumor karyotype demonstrated an overall stability, maintaining the consistent abnormalities of a 1p-, +17, and -22. Chromosomes 5 and 9 were also involved in structural abnormalities in sublines of the tumor cells. Double minutes were seen in all preparat
