Chiba osteotomy (Tibial condylar valgus osteotomy) for a large tibial varus deformity: Technical note.

Chiba osteotomy is an effective technique for advanced knee osteoarthritis (KOA). The principle of the osteotomy is to correct both varus deformity and intra-articular joint congruity through an L-shaped osteotomy from the medial tibial condyle to the lateral intercondylar eminence. Previous studies have demonstrated that Chiba osteotomy is an effective method for alignment correction surgery for severe knee osteoarthritis. However, these reports slightly differ from the original concept of Chiba osteotomy. This report describes the pre-operative planning and surgical technique of Chiba osteot

Opening Cuboid Wedge Osteotomy (Zoom Osteotomy) for Triplanar Correction of Flexible Pes Planovalgus Deformities.

Surgical correction of flexible pes planovalgus often involves reestablishing the length of the lateral column. This is a review of a cohort of patients who underwent an opening cuboid osteotomy with interpositional graft for triplanar correction of flexible pes planovalgus. The medical records of 35 patients involving 51 feet were reviewed. All patients were treated with an opening wedge osteotomy of the cuboid in combination with adjunctive procedures as needed for correction of the pes planovalgus deformity. Radiographs were obtained before and a minimum of 12 months after surgery. Preopera

Combination of Modified Scarf Osteotomy and Metatarsal Shortening Offset Osteotomy for Rheumatoid Forefoot Deformity.

With the progress of medical treatment for rheumatoid arthritis (RA), several joint-preserving forefoot surgical procedures have been established and performed. In this situation, we have been choosing the combined surgery: modified scarf osteotomy for the great toe and metatarsal shortening offset osteotomy for the lesser toes in RA cases. A retrospective observational study of 53 RA patients (mean follow-up period: 4.6 years) who underwent the surgery was completed. RA foot ankle scores were assessed, using the Japanese Society for Surgery of the Foot (JSSF) standard rating system, and a sel

[Management principle and clinical suggestions of osteotomy gap of opening wedge high tibial osteotomy].

OBJECTIVE: To summarize the management principle and clinical suggestions of the osteotomy gap of opening wedge high tibial osteotomy (OWHTO). METHODS: The related literature of the osteotomy gap of OWHTO in recent years was reviewed, summarized, and analyzed. RESULTS: Delayed union and non-union of the osteotomy gap are main complications of OWHTO. Tomofix plate, as locking steel plate, has the characteristics of angular stability and can better maintain the stability of the osteotomy gap, promote bone healing, and avoid loss of correction. There are some treatment options for the osteotomy

Advanced containment methods for Legg-Calvé-Perthes disease: triple pelvic osteotomy versus Chiari osteotomy.

INTRODUCTION: The goal of intervention in Legg-Calvé-Perthes disease (LCPD) is to prevent femoral head deformation by containing the head within the acetabulum. Currently, surgical containment methods are the mainstay of treatment, and pelvic osteotomies have been shown to be successful. They include triple pelvic osteotomy (TPO), Salter osteotomy, Chiari osteotomy and shelf procedure. The purpose of this study was to compare clinical and radiologic results for Chiari osteotomy and TPO in LCPD. METHODS: 29 children treated between 1980 and 2010 for LCPD in 2 centres were reviewed. 19 underwen

Open Wedge High Tibial Osteotomy with Distal Tubercle Osteotomy Lessens Change in Patellar Position.

The purpose of this study was to investigate the change in patellar position after open wedge high tibial osteotomy (OWHTO) with distal tubercle osteotomy (DTO), comparing outcomes of conventional OWHTO in young adults with proximal tibia varus deformity but no arthritic manifestations. Thirty-three patients (mean age, 31.8 years) subjected to OWHTO/DTO were matched with 30 patients (mean age, 33.5 years) undergoing conventional OWHTO. Patellar position, as measured in pre- and postoperative standing lateral radiographs, was compared. Patellar height was assessed via Insall-Salvati ratio, modi

Comparison of femoral head translation following eccentric rotational acetabular osteotomy and rotational acetabular osteotomy.

PURPOSE: Eccentric rotational acetabular osteotomy (ERAO) is a modification of rotational acetabular osteotomy (RAO); it has been reported that ERAO allows the femoral head to translate medially and distally. However, no study has compared femoral head translation following RAO or ERAO. The purpose of this study was to compare immediate postoperative translation of the femoral head after RAO and ERAO in comparison with the preoperative position by radiological methods. METHODS: Patients treated by RAO or ERAO between 2006 and 2014 were retrospectively evaluated. 19 hips (17 patients) were tre

Long-term neurosensory deficits associated with bilateral sagittal split osteotomy versus inverted 'L' osteotomy.

Ten cases of bilateral sagittal split osteotomies and nine cases of intraoral inverted "L" osteotomies were examined for postsurgical neurosensory disturbances. Neurosensory tests included brushstroke direction, temperature, and light touch. All patients completed a short questionnaire that elicited subjective information with respect to neurosensory symptoms and their perception of the surgical results. Analysis of the data obtained from the tests of neurosensory disturbances revealed no significant differences between the two surgical groups. The variance of the scores on each of the neurose

Proximal tibial osteotomy with absorbable spacer combined with fibular osteotomy has similar clinical outcomes to high tibial osteotomy in the treatment of knee osteoarthritis.

PURPOSE: Proximal tibia osteotomy with absorbable spacer combined with fibular osteotomy (TPOASI) is an emerging surgical technique for treating knee osteoarthritis (KOA); however, the efficacy of this procedure remains unknown. We hypothesize that TPOASI can achieve similar clinical outcomes to opening-wedge high tibial osteotomy (OW-HTO). The objective of this study is to compare the clinical results between these two procedures. METHODS: Patients who underwent TPOASI or OW-HTO from July 2016 to September 2020 were included. The following outcome parameters were determined before and after

Opening Wedge Distal Tibial Osteotomy.

Supramalleolar osteotomies of the distal tibial metaphysis or distal tibial metaphyseal-diaphyseal junction are indicated when angular, rotational, and/or translational malalignment of the ankle joint are present. Varus and valgus deformities of the ankle that can be readily corrected through opening wedge osteotomies of the distal tibia are the focus of this article. The authors present an in-depth description of the surgical technique for opening wedge osteotomy of the distal tibia, as well as a review of how to prevent and address the most common complications encountered.

Calcaneal osteotomy for hindfoot deformity.

Calcaneal osteotomy is an extra-articular procedure used for conservative surgical treatment of hindfoot deformity. It has static, architectural and dynamic effects, reorienting the tuberosity action point of the digastric muscle formation of the sural triceps and plantar aponeurosis. Calcaneal osteotomies vary in location, form and displacement, but can be categorized as tuberosity osteotomy, acting on talar position, and cervical osteotomy, acting on Chopart joint-line orientation. We here describe the 3 main calcaneal osteotomies we use for hindfoot deformity: talar varus/valgus, valgus fla

Mandibular feminization osteotomy-preliminary results.

Strong mandibular angles and a heavy chin are perceived as masculine features, so surgical feminization of female or transgender faces may warrant mandibular base narrowing (borders and angles) and a reduction in chin width and height. With this in mind, we have devised an interdental midline osteotomy (triangular in shape and with a caudal base) to accompany a box-shaped impaction osteotomy of the chin symphysis and horizontal triangular resection of the lateral mandibular border. The reduction of the intergonial width and of the mandibular border and chin widths takes place upon closure of t

Minimally Invasive Pedicle Subtraction Osteotomy.

For patients with significant spinal deformity, the pedicle subtraction osteotomy provides a powerful means for correction, albeit with high morbidity. With the trend toward minimally invasive spine surgery, multiple less invasive techniques have been devised; however, there seems to be an upper limit to the degree of correction possible. The mini-open pedicle subtraction osteotomy addresses these limitations by minimizing the extent of soft tissue destruction needed to perform the osteotomy and by using the rod-cantilever technique to achieve maximum lordosis. Preliminary data are promising,

Moberg Osteotomy for Hallux Rigidus.

Hallux rigidus is a common cause of pain and decreased range of motion of the first metatarsophalangeal joint, notably with loss of dorsiflexion. Hallux rigidus is the second most common disorder affecting the great toe. The primary cause of hallux rigidus is thought to be a traumatic event, a culmination of microtrauma, or an alteration in kinematics leading to a loss of articular cartilage and dorsal osteophyte formation. Surgical treatments include Moberg osteotomy, also referred to as a proximal phalanx extension osteotomy. This article discusses current techniques, developments, complicat
