Ethical Considerations in Surgery for Single-suture Craniosynostosis.

Singe-suture craniosynostosis (SSC) describes the premature fusion of one cranial suture, which restricts cranial growth and consequently results in unaffected regions presenting a compensatory expansion. Surgery can redistribute intracranial volume, reduce the risk of elevated intracranial pressure, and improve head shape, potentially leading to improved neurocognitive function and social acceptance. However, there is limited evidence that surgery for SSC improves neurocognitive function and social acceptance. Given the inherent surgical risks and uncertainty of outcomes, the conditions under

[Autoimmune encephalitis-Diagnostic and therapeutic decision tree from a psychiatric, neurological and ethico-legal point of view : Approach in cases of lack of ability to give consent and permissibility of compulsory treatment].

Patients with severe mental illnesses who are unable to give consent often need a rapid diagnosis and treatment but due to the psychiatric symptoms they often reject such measures. In the routine practice, the question arises to what extent the patient's expressed will should dictate the treatment steps and whether a decision against the patient's will is medically reasonable, ethically justifiable or even demanded and legally permissible. Autoimmune encephalitides, such as N‑methyl-D-aspartate receptor (NMDAR) encephalitis, have recently become important differential diagnoses due to their re

Fluctuating capacity and advance decision-making in Bipolar Affective Disorder - Self-binding directives and self-determination.

For people with Bipolar Affective Disorder, a self-binding (advance) directive (SBD), by which they commit themselves to treatment during future episodes of mania, even if unwilling, can seem the most rational way to deal with an imperfect predicament. Knowing that mania will almost certainly cause enormous damage to themselves, their preferred solution may well be to allow trusted others to enforce treatment and constraint, traumatic though this may be. No adequate provision exists for drafting a truly effective SBD and efforts to establish such provision are hampered by very valid, but also

Review of self-exclusion from gambling venues as an intervention for problem gambling.

Self-exclusion programs are required to be provided by gambling operators in many international jurisdictions in an attempt to provide an option for those who have gambling problems to avoid further gambling. However, minimal robust and comprehensive research has been conducted to evaluate the effectiveness of self-exclusion programs. There is much scope for reform and greater cohesion between jurisdictions, particularly neighbouring jurisdictions that would offer greater protection to individuals and industry bodies. This review outlines the evidence surrounding existing self-exclusion strate

Ethical issues in cancer screening and prevention.

November 2009's announcement of the USPSTF's recommendations for screening for breast cancer raised a firestorm of objections. Chief among them were that the panel had insufficiently valued patients' lives or allowed cost considerations to influence recommendations. The publicity about the recommendations, however, often either simplified the actual content of the recommendations or bypassed significant methodological issues, which a philosophical examination of both the science behind screening recommendations and their import reveals. In this article, I discuss two of the leading ethical con

Psychological and ethical considerations concerning the debreather and assisted suicide: a commentary on Ogden.

This article offers a commentary on the report by Russel Ogden (2010/this issue) on the use of the "debreather" for suicide assistance by the NuTech (or New Technologies for Self-Deliverance) program. The emergence of NuTech is set within its historical and political context. Nu Tech is criticized for its anarchic and extreme advocacy of self-determination to the neglect of other psychological and ethical considerations (e.g., the individual's emotional state, the mutability of his or her quality of life, and his or her decision-making capacity). Some of the risks of covert, unregulated, assis

Subtly encouraging more deliberate decisions: using a forcing technique and population stereotype to investigate free will.

Magicians' forcing techniques allow them to covertly influence spectators' choices. We used a type of force (Position Force) to investigate whether explicitly informing people that they are making a decision results in more deliberate decisions. The magician placed four face-down cards on the table in a horizontal row, after which the spectator was asked to select a card by pushing it forward. According to magicians and position effects literature, people should be more likely to choose a card in the third position from their left, because it can be easily reached. We manipulated whether parti

Chapitre 4. Autonomy of minors questioned: Russian case with the loss of medical secrecy.

What is the value of confidentiality? Russian society faced this issue in 2020, when minors aged 15 to 18 lost their privacy. The amendment to the Federal Law, caused the situation, was received ambiguously, but quickly ceased to be the subject of public discussion. In my article, I study this event in a bioethical context, considering the topics of privacy, autonomy and relativity. Social discussion was not productive, since the arguments of both sides contained a double-edged argument: depending on existing relations in a family, the amendment will have a positive or negative meaning. By poi

"I Want to Hold an Umbrella Over You Because I Have Been in the Rain": Exploring Patient Influencers' Motivations to Share Eating Disorders Experiences from a Self-Determination Theory Perspective.

Patients can become educator-influencers in promoting the public's understanding of health and illness; however, the underlying motivations driving their engagement in this role remain unclear. Drawing from the Self-Determination Theory, this study explores the motivations of patients-turned-influencers in sharing eating disorders (ED) experiences and information on Chinese social media. Through semi-structured interviews with 33 patient influencers, this study examines the motivations associated with three psychological needs: autonomy, competence, and relatedness. In terms of autonomy, influ

Associations of Wellbeing Levels, Changes, and Within-Person Variability With Late-Life All-Cause Mortality Across 12 Years: Contrasting Hedonic vs. Eudaimonic Wellbeing Among Very Old Adults.

Advanced old age has been characterized as a biologically highly vulnerable life phase. Biological, morbidity-, and cognitive impairment-related factors play an important role as mortality predictors among very old adults. However, it is largely unknown whether previous findings confirming the role of different wellbeing domains for mortality translate to survival among the oldest-old individuals. Moreover, the distinction established in the wellbeing literature between hedonic and eudaimonic wellbeing as well as the consideration of within-person variability of potentially relevant mortality

'I am old, and I will rather abandon life with my eyes open': a qualitative study of Norwegians aged 80 and older explaining their intensive care unit admission preferences.

BACKGROUND: The treatment preferences of patients aged 80 and older in the event of acute critical illness vary widely and are difficult to predict. To provide the appropriate level of care, it is necessary to better understand their personal treatment goals. OBJECTIVE: To gain insight into the underlying reasoning for intensive care unit (ICU) admission preferences in hypothetical events of acute critical illness stated by Norwegian survey respondents aged 80 and over. METHODS: A purposive sample of 202 Norwegian outpatients ≥80 years old who were considered potential ICU candidates provide

Graphic Pathographies as Ethical Texts: Autonomy and Agency in Narratives of Anorexia Nervosa.

Anorexia nervosa presents profound challenges to both patients and healthcare providers, as treatment often conflicts with the individual's values, voice, and sense of control. Traditional clinical approaches tend to prioritize medical outcomes over patient autonomy, frequently resulting in coercive interventions grounded in assumptions about impaired decision-making capacity. This paper examines how graphic pathographies, personal, visual narratives created by those living with illness, offer a compelling alternative lens on autonomy in anorexia nervosa by centering lived experience and moral

Examining Resident Burnout Through the Lens of Self-Determination Theory: The Role of General Causality Orientations.

Background Burnout continues to plague graduate medical education, and theory-informed approaches are lacking for effectively tackling this problem. Studies on personal factors that explain physician burnout have also neglected the role of self-determination. In self-determination theory, general causality orientations-autonomy, control, and impersonal-represent individual differences in self-determination that can be socialized and primed within environments, each relating to different motivation, behavior, and well-being outcomes. Objective To investigate how each general causality orientati

[Vertigo in the elderly: otorhinolaryngologic approach].

Dizziness is a common but little understood ailment of old people. Its presence is the origin of an important incapacity associated with lack of personal autonomy. Clinical and electronystagmographic findings of aging individuals enduring imbalance and coming to our Department allowed us to accept that those cases in which the cause could be identified it was on clinic data, being of scarce utility the other exams done.

STD-related knowledge, beliefs and attitudes of Xhosa-speaking patients attending STD primary health-care clinics in South Africa.

The primary aim of this study was to describe patients at sexually transmitted disease (STD) clinics in Cape Town, South Africa, in terms of gender, education and age differences relative to their STD knowledge and beliefs, their condom use, as well as their attitudes towards condom use and their condom-use behaviour. The information was collected with a view to developing a health education intervention. Structured interviews were conducted with 2978 randomly sampled Xhosa-speaking STD clinic attenders about their knowledge, beliefs and practices regarding STDs and related behaviours. More ma

The Effects of Autonomy-Enhancing Therapy for Eating Disorders: Protocol for a Single-Case Experimental Multiple Baseline Design.

BACKGROUND: Eating disorders (EDs) remain challenging to treat effectively, with low recovery percentages and high relapse rates. This study protocol explores autonomy-enhancing therapy as a therapeutic adjunct to existing day care treatment in the last phase of treatment. Autonomy-enhancing therapy is aimed at improving autonomy-connectedness: the capacity for self-steering while connected to others, which encompasses self-awareness, sensitivity to others, and the capacity for managing new situations. Higher autonomy-connectedness has been associated with improved ED recovery and lower relaps

Patients' perceptions of autonomy in palliative care: Two patient interview exemplars.

BACKGROUND: Although palliative care embraces person-centred principles and autonomy to meet patients' holistic needs, autonomy is not automatically assured and few studies cover patients' lived experiences. Consequently, current thinking about dying and palliative care practices would benefit from an in-depth understanding of patient experiences. Such research provides an opportunity to challenge assumptions about patient preferences for the amount and type of care and decision-making involvement, especially for the way patient preferences may vary between individuals and change through the p
