The hemodynamic management of a patient with a pheochromocytoma presents special challenges due to the episodic release of catecholamines from the tumor, which threatens to provoke a hypertensive crisis. We present a patient with metastatic pheochromocytoma (bone, lung, lymph nodes) who underwent successful coronary artery bypass graft (CABG) surgery following premedication with phenoxybenzamine and metyrosine as well as the use of intraoperative phentolamine for the management of a hypertensive crisis in the operating room.

We describe a 71-year-old woman who was admitted with pneumonia. She quickly developed severe pain and gangrene in all four extremities and had several fingers and crurae amputated. Pheochromocytoma was diagnosed by a 2 x 2 24-hour urine collection, computerized axial tomography and MIBG scintigraphy. The patient was suppressed with phenoxybenzamine hydrochloride and propanolol for three weeks and was subsequently laparoscopically adrenalectomized on her left side.

Chlorpromazine (CPZ) was shown to inhibit tumour necrosis factor (TNF) production in vivo. Several drugs sharing one of the various pharmacological activities of CPZ were tested in endotoxin-treated mice. It was found that alpha-blockers (prazosin, idazoxan, phenoxybenzamine), antiserotoninergics (methysergide, methergoline) and antihistamine (chlorpheniramine, promethazine) also inhibited TNF production indicating that the effectiveness of CPZ in protecting from endotoxin shock is due to its multiple pharmacological activities.

The case of an eight year old girl is presented who was seen because of secondary enuresis and recurrent urinary tract infection. Detailed examinations revealed urinary retention and sustained renal insufficiency (Creatinine clearance: 11 ml/min x 1.73 m2) due to a pseudo-neurogenic bladder. After improvement of renal function by continuous bladder drainage, bladder dysfunction was successfully treated by medication (Phenoxybenzamine, Baclofen), conventional physical therapy and biofeedback.

Salivation in response to clonidine was completely abolished by prazosin, phentolamine, phenoxybenzamine and dihydroergotamine. 3. The gamma-type of proteins was secreted in response to three of the analogues, whereas with p-aminoclonidine the alpha-type of proteins was secreted by the submandibular gland. 4. Albumin was specifically secreted by the submandibular gland in response to clonidine but not to isoproterenol or phenylephrine.
