(Mis)understanding the intersection between development policies and data collection: Experiences in Afghanistan.

There is a misalignment between a development community focused on improving the welfare of vulnerable populations and the challenges of addressing widespread illicit drug crop production in drug producing countries. Drawing on the example of Afghanistan, this paper argues that the reasons for the development communities failure to engage is in large part a function of the way that illicit opium poppy and the illicit economy is currently perceived and understood by policy makers, practitioners and scholars. Much of the problem lies with the various statistics used to describe and quantify opiu

Harm reduction as a complex adaptive system: A dynamic framework for analyzing Tanzanian policies concerning heroin use.

Contrary to popular belief, policies on drug use are not always based on scientific evidence or composed in a rational manner. Rather, decisions concerning drug policies reflect the negotiation of actors' ambitions, values, and facts as they organize in different ways around the perceived problems associated with illicit drug use. Drug policy is thus best represented as a complex adaptive system (CAS) that is dynamic, self-organizing, and coevolving. In this analysis, we use a CAS framework to examine how harm reduction emerged around heroin trafficking and use in Tanzania over the past thirty

Developing environmental health indicators as policy tools for endemic fluorosis management in the People's Republic of China.

Drinking groundwater containing naturally occurring elevated concentrations of fluoride has given rise to extensive dental and skeletal fluorosis affecting many millions of people in China. This paper describes three sets of indicators useful for environmental and human fluorosis management purposes, namely, descriptive indicators (the past), response indicators (the present) and performance indicators (the future). Each of the sets of indicators was further detailed following the Organisation for Economic Co-operation and Development (OECD) pressure-state-response model modified to include an

Children in all policies: lessons from a global collaboration to promote the health and wellbeing of children and future generations.

Currently children's needs, perspectives, and rights are not adequately included in public policies, with negative consequences for the health and wellbeing of children and future generations. The 2020 WHO-UNICEF-Lancet Commission reviewed threats to children's health and concluded that children's needs and voices should be centred in all policies for a sustainable future. Since 2021, Children in All Policies 2030, a global collaboration of policy makers, scientists, and advocates, has implemented the Commission's recommendations by fostering new approaches to participatory, intersectoral poli

The causation-prevention chain in infrastructure safety measures: A consideration of four types of policy lock-ins.

Safety policies typically follow Lasswell's linear decision cycle paradigm: diagnostics, prescription, application, monitoring, and appraisal. Contemporary policy research highlights the existence of complexities in policy-making, which trigger policy lock-ins. We consider four cases in which the complex nature of the causation-prevention discourse leads to decision-making lock-ins, which deter safety progress. The four cases are conflicting narratives, missing causation inferences, prevention and mobility mismatch, and a tension between policy transfer and existing policy environments. The ca

COVID-19 kit distribution in six Latin American countries: bureaucratic autonomy, populism, and evidence-based policymaking.

During the COVID-19 pandemic, governments varied widely in the extent to which policy decisions aligned with scientific evidence. We examine decision-making regarding the public distribution of COVID-19 kits containing ivermectin in six Latin American countries in 2020 and 2021, despite international scientific authorities advising against using these medicines outside of clinical trials. We focus on institutions, ideology, and interests. Theories emphasising the centrality of institutions predict that autonomous scientific institutions are more likely to resist political pressure to adopt ine

Policy priorities in palliative and end-of-life care: An exploration of issues reflected in public strategy.

OBJECTIVES: To map the issues and priorities reflected in palliative and end-of-life care policy in Australia. METHODS: A qualitative documentary policy analysis examined the content of national and state-based policies and strategies on palliative and end-of-life care that were published in Australia and identified during July-September 2020. Summative content analysis was used to quantify policy characteristics, key domains and depth of coverage across strategy documents and to identify emergent themes. RESULTS: Nine policies comprising a total of 304 pages published from 2005-2019 and cov

Measuring the growth of infectious disease modelling publications and their impact on policymaking: A large language model-assisted bibliometric review.

BACKGROUND: Infectious disease modelling (IDM) is increasingly used to understand disease transmission and inform public health policy. Its growth and influence on policy have not been quantified, partly due to the large volume of literature. Using a large language model (LLM)-assisted review, we quantified the expansion of IDM publications, trends in policy citations, and regional disparities in research contributions and uptake. METHODS: An LLM-assisted bibliometric review was conducted in Embase, Medline, and Scopus, identifying IDM publications to December 2024 via GPT-4o (OpenAI). Eligib

Competing coalitions in alcohol policymaking in Malawi: a retrospective case study analysis of the alcohol sachet ban.

BACKGROUND: Harmful alcohol consumption has significant public health implications across Africa and disproportionately affects vulnerable populations. In Malawi, the emergence of alcohol sachets - small, affordable plastic packets containing high-strength spirits - and their consumption, has raised substantial public health concerns. In particular, the resulting access and consumption amongst young people, led to a ban on those products by the Malawi Government in 2017. This study focuses on the policy process of the sachets ban, from agenda setting, to formulation and covers the modalities f

Web-based models to inform health policy: A scoping review.

Health policies must be continually updated as new evidence is generated to ensure the optimal delivery of health interventions and prevention measures. Models are often used to study health problems, but their complexity limits their use by policy-makers. One way to facilitate their use among non-modellers is to develop user-friendly interfaces and make them available online. We conducted a scoping review of journal articles to identify and describe the currently available, interactive, freely available web-based health models that aim to inform health policy relevant to any disease or health

Policy advisory bodies during crises: a scoping review of the COVID-19 literature in Europe.

BACKGROUND: The COVID-19 policy context was characterised by high levels of uncertainty, imperfect knowledge and the need for immediate action. Therefore, governments in Europe tended to rely on expertise provided by advisory bodies to design their crisis response. Advisory bodies played a fundamental part in policy making during the crisis to optimise policy formulation. AIMS AND OBJECTIVES: During the COVID-19 crisis, the literature on policy advice grew considerably. To grasp the main research outcomes, we conduct a scoping review that interrogates the COVID-19 policy advice literature to

Policy analysis of the Global Financing Facility in Uganda.

BACKGROUND: In 2015, Uganda joined the Global Financing Facility (GFF), a Global Health Initiative for Reproductive, Maternal, Newborn, Child, and Adolescent Health (RMNCAH). Similar initiatives have been found to be powerful entities influencing national policy and priorities in Uganda, but few independent studies have assessed the GFF. OBJECTIVE: To understand the policy process and contextual factors in Uganda that influenced the content of the GFF policy documents (Investment Case and Project Appraisal). METHODS: We conducted a qualitative policy analysis. The data collection included a

Building evidence into youth health policy: a case study of the Access 3 knowledge translation forum.

BACKGROUND: Effective integration of evidence and youth perspectives into policy is crucial for supporting the future health and well-being of young people. The aim of this project was to translate evidence from the Access 3 project to support development of a new state policy on youth health and well-being within New South Wales (NSW), Australia. Ensuring the active contribution of young people within policy development was a key objective of the knowledge translation (KT) process. METHODS: The KT activity consisted of a 1-day facilitated forum with 64 purposively sampled stakeholders. Parti

Barriers to breastfeeding in Lebanon: A policy analysis.

Although the issue of breastfeeding in Lebanon has risen on the political agenda, the country does not meet international recommendations for early breastfeeding practices. This study analysed barriers to dissemination, implementation, and enforcement of key policies to improve early breastfeeding practices. We conducted interviews with stakeholders in breastfeeding policy in Lebanon and used a framework approach for analysing data. We found a disconnect between policy endorsement and translation on the ground, weak engagement of professional associations and governmental institutions, undue i

A decade of countering bioterrorism: incremental progress, fundamental failings.

The fear and disruption caused by the 2001 anthrax attacks understandably led Americans to seek enhanced biodefenses. However, the path followed since those attacks has left the country inadequately prepared to face further risks from biological attacks. Why has security against these threats been only partially achieved? This article suggests that our responses over the past decade can be sorted into 4 levels in order of increasing difficulty. First, we rapidly appropriated funds, augmented personnel, and mandated reorganization of agencies. Though not easy to accomplish, these steps were eas

Engaging physical activity policymakers.

Evidence for the health benefits of physical activity is overwhelming: physical activity is protective against type II diabetes, cardiovascular disease, breast and colon cancer, stroke, osteoporosis, depression, falls in older adults, and many other adverse health outcomes. Research also suggests that widespread physical inactivity exacts a heavy toll on the US economy, as well as on individual health. Researchers have estimated that those who are physically inactive impose greater costs on society than do smokers or problem drinkers. Increasing rates of physical activity may therefore be one

Framing drug policy reform: Third-sector advocacy and the 2024 New South Wales Drug Summit.

BACKGROUND: Drug policy remains a politically and socially contested domain, with third-sector voices often marginalised despite their central role in advocacy and service delivery. The 2024 New South Wales (NSW) Drug Summit provided a rare, government-convened deliberative forum for third-sector organisations to contribute to state drug policy debate. This study examined how third-sector organisations framed drug policy issues and proposed solutions in advocacy documents produced in the lead-up to the summit and assessed the extent to which these frames were reflected in the summit's official

A Policy Analysis on the Death of Alex Pretti and the Imperative to Protect the US Nursing Workforce.

Violence against nurses is a systemic threat that compromises workforce safety and quality care, especially in HIV nursing. The death of Alex Jeffrey Pretti, an intensive care nurse, exemplifies broader patterns of violence affecting nurses both within and beyond clinical settings and represents inherent risks of militarization within democratic societies. This commentary reviews epidemiologic evidence on violence against nurses, situated within systems of oppression, and assesses legislative efforts to protect the nursing workforce. Effective protection requires structural interventions such
