Examining the Case for Palliative Care in Patients With Systemic Sclerosis.

Systemic sclerosis (SSc) is a complex, multiorgan disease that causes substantial and progressive symptoms and impairs quality of life. International guidelines recommend early, integrated palliative care for patients with advanced cardiopulmonary disease, such as heart failure and interstitial lung disease, as this care can improve patient, caregiver, and healthcare outcomes. In this article, we examine the potential need and role for palliative care in SSc. We propose that early, integrated palliative care could improve symptom control and quality of life and recommend a research agenda for

Nursing a Patient With Amyotrophic Lateral Sclerosis Stage 4B With Epilepsy: A Case Study.

Amyotrophic lateral sclerosis (ALS) is a rare motor neuron disease characterised by progressive muscle weakness, which can eventually lead to death. So far there is no effective cure for it. This case report discusses the nursing of a patient with stage 4B amyotrophic lateral sclerosis complicated with epilepsy and sudden disturbance of consciousness during his stay in an intensive care unit (ICU). After 34 days of treatment and care, the patient regained consciousness with stable vital signs before being transferred out of the ICU. This report focusses on nursing interventions adopted in term

What rationale for treatment of occupational interstitial lung diseases with the drugs approved for idiopathic pulmonary fibrosis?

PURPOSE OF REVIEW: To critically discuss the rationale for the use of drugs approved for idiopathic pulmonary fibrosis (IPF) to treat occupational interstitial lung diseases (OILDs). RECENT FINDINGS: Although IPF and OILDs share several clinical, radiological and probably pathogenetic features, currently, OILDs do not have a standard of care. In recent years, our knowledge and understanding of ILDs has improved substantially. Recently, the progressive pulmonary fibrosis (PPF) phenotype, which refers to non-IPF fibrotic ILDs that progress despite appropriate treatment, has been defined. OILDs

Mobilization of intensive care cardiac surgery patients on mechanical circulatory support.

Historically, intensive care cardiac surgery patients remained on bed rest for several days postoperatively to prevent complications and promote rest and healing. Over time, the cardiac surgery discipline has acknowledged the benefits of early mobility, including reduced risk of venous thromboembolism and pulmonary emboli, improved pulmonary toilet, prevention of pneumonia, decreased length of stay, reduced deconditioning, and need for rehabilitation, among others advantages. These benefits have changed clinical practice, with emphasis on early extubation, progressive mobility, and reduced len

A pictorial overview of technology-assisted care options for bariatric patients: one hospital's experience.

Best practice guidelines to avoid pressure ulcers and skin breakdown among obese patients include early and progressive mobility, rigorous turning schedules, and proper skin care. However, implementation of some these guidelines may increase the risk of patient and caregiver injury. An acute care hospital implemented safe patient handling protocols that involved equipment purchase and extensive training for all care staff. The new equipment facilitated repositioning, including boosting and turning, lateral transfers, vertical transfers and ambulation, and bathing and toileting. All healthcare

The Embedding of a Specialized Nurse Practitioner Palliative Care Clinic within a Multiple Sclerosis Comprehensive Care Center.

People with multiple sclerosis (PwMS) face an uncertain future as the nature of the disease can be progressive and unpredictable, leading to chronic serious symptomatology impacting quality of life. Serious illness discussions and symptom management should be part of routine care in all outpatient neurology clinics. Yet few standard clinical approaches to palliative care exist for this population. Providers frequently lack understanding about when to incorporate specialist palliative care. The MS-Support project established a specialized nurse practitioner (NP)-led palliative care clinic embed

Advanced (stage D) heart failure: a statement from the Heart Failure Society of America Guidelines Committee.

We propose that stage D advanced heart failure be defined as the presence of progressive and/or persistent severe signs and symptoms of heart failure despite optimized medical, surgical, and device therapy. Importantly, the progressive decline should be primarily driven by the heart failure syndrome. Formally defining advanced heart failure and specifying when medical and device therapies have failed is challenging, but signs and symptoms, hemodynamics, exercise testing, biomarkers, and risk prediction models are useful in this process. Identification of patients in stage D is a clinically imp

Toward a treat-to-target approach in the management of juvenile idiopathic arthritis.

Recent advances in the management of juvenile idiopathic arthritis (JIA) render disease remission an attainable goal in many, if not most, patients. This has led to suggestions that future treatment guidelines include an overriding goal to achieve clinical remission or, at least, minimal disease activity. Furthermore, implementation of treatment strategies aimed at achieving and maintaining tight disease control in standard paediatric rheumatology practice has been proposed. A compelling argument is available at this time to suggest that the incorporation of treat-to-target approach in the man

Rosacea: how to recognize and treat an age-related skin disease.

Rosacea is an age-related disorder of the central portion of the facial skin whose peak onset occurs in persons in their 40s and 50s. A chronic and progressive condition of flare-ups and remissions, rosacea can be disfiguring if left untreated. Rosacea can be characterized as having three stages. Target areas for all symptoms include the cheeks, nose, chin, or forehead. Rosacea resembles a number of other dermatologic conditions, particularly acne vulgaris. The combination of oral and topical antibiotic therapy usually brings about remission. The key is to recognize the early signs and clinica

[A personal exploration of oral health in amyotrophic lateral sclerosis].

Amyotrophic lateral sclerosis is a progressive neurodegenerative disease impairing motor functions, including those of the oral muscles. As a result, oral hygiene and oral care become challenging. Nonetheless, oral health in patients with amyotrophic lateral sclerosis often remains under-addressed. On the basis of interviews with Dr M.K.A. van Selms dental researcher and patient oral health in amyotrophic lateral sclerosis was explored, highlighting care, research and education. Dr van Selms identifies the neglect of oral hygiene in the care of patients with amyotrophic lateral sclerosis, and

[Gradual approach for the etiological diagnosis of the neurocognitive disorder].

Neurocognitive disorders (TNC) are of a public health concern. An early and accurate diagnosis is important to tailor a personalised care. We illustrate the importance of a graduated etiological diagnostic approach centered on the clinical presentation employing the case of a patient with a progressive neurovisual disorder suggestive of a common form of Alzheimer's disease. The results of the CSF biomarkers analysis argue against this diagnosis and justifies seeking a Lewy body disease as a differential diagnosis even if all the clinical criteria are initially incomplete. In this article, we i

An Australian Neuro-Palliative perspective on Huntington's disease: a case report.

BACKGROUND: Huntington's Disease (HD) is an incurable, progressive neuro-degenerative disease. For patients with HD access to palliative care services is limited, with dedicated Neuro-Palliative Care Services rare in Australia. We discuss the experiences of and benefits to a patient with late-stage HD admitted to our Neuro-Palliative Care service. CASE PRESENTATION: We present the case of a patient with a 16-year history of HD from time of initial genetic testing to admission to our Neuro-Palliative Care service with late-stage disease. CONCLUSIONS: Given the prolonged, fluctuating and heter

Determinants of patient delay in seeking care for worsening heart failure symptoms in out-of-hospital settings: protocol for a systematic review and meta-analysis.

INTRODUCTION: Worsening heart failure (HF) is common and associated with rehospitalisation, premature death, poor quality of life and high healthcare costs. Patients frequently delay seeking medical care for hours or days after symptom escalation, and longer delays are linked to more severe clinical status at presentation and worse outcomes. Existing quantitative and qualitative studies have explored multiple potential determinants of patient delay, but findings remain fragmented and inconsistent, partly due to differing sets of candidate predictors and variation in populations and healthcare

A complications-based clinical staging of obesity to guide treatment modality and intensity.

PURPOSE OF REVIEW: The current medical model for obesity management is BMI-centric because BMI is the predominant measure used to gauge disease severity, as well as indications for various treatment modalities. Recent advancements in therapy and understanding of the relationship between BMI and obesity-related complications call for a re-examination of this approach. RECENT FINDINGS: Advancements in treatment, including the recent approval of two new weight loss medications in the USA, have enabled development of new medical models for management of obesity. On the basis of accumulating data

Perioperative care of a patient with neuronal ceroid lipofuscinoses.

The neuronal ceroid lipofuscinoses (NCL) are a group of inherited, autosomal recessive, and progressive neurodegenerative diseases, which result from an enzymatic defect or the deficiency of a transmembrane protein, leading to the accumulation of lipopigments (lipofuscin) in various tissues. NCL results in the impairment of function in several end-organs including the central nervous system with loss of cognitive and motor function, myoclonus, and intractable seizures. Additional involvement includes the cardiovascular system with arrhythmias and bradycardia as well as impairment of thermoregu

Chronic Obstructive Pulmonary Disease: A Palliative Medicine Review of the Disease, Its Therapies, and Drug Interactions.

Despite significant advances in treatment, chronic obstructive pulmonary disease (COPD) remains a chronic and progressive disease that frequently leads to premature mortality. COPD is associated with a constellation of significant symptoms, including dyspnea, cough, wheezing, pain, fatigue, anxiety, depression, and insomnia, and is associated with increased morbidity. Palliative care is appropriate to support these patients. However, historically, palliative care has focused on supporting patients with malignant disease, rather than progressive chronic diseases such as COPD. Therapies for COPD

A multi-center randomized, controlled, open-label trial evaluating the effects of eosinophil-guided corticosteroid-sparing therapy in hospitalised patients with COPD exacerbations - The CORTICO steroid reduction in COPD (CORTICO-COP) study protocol.

BACKGROUND: The most commonly applied treatment for acute exacerbations of chronic obstructive pulmonary disease (AECOPD) is a 5-day course of high-dose systemic corticosteroids. However, this treatment has not been shown to reduce mortality and can potentially have serious side effects. Recent research has shown that, presumably, only a subgroup of COPD patients identifieable by blood eosinophil count benefit from a rescue course of prednisolone. By applying a biomarker-guided strategy, the aim of this study is to determine whether it is possible to reduce the use of systemic corticosteroids
