[Combined scaphoid and distal radius fractures in adults].

Although isolated distal radius and scaphoid fractures are common, the combination of both fractures is rare, with a reported frequency between 0.5% and 6%. This rarity is probably due to the fact that both fractures share the same injury mechanism. Published studies are limited, but most patients are males in their 40s and the injuries are typically due to high-energy trauma. In most studies, the distal radius fracture is displaced while the scaphoid fracture is not. Since the functional outcome depends of the severity of the radius fracture, we believe, as do others, that it is logical to fi

[Complex pain regional syndrome after distal radius fractures].

Complex regional pain syndrome (CPRS) greatly affects the outcomes of distal radius fractures in terms of functional recovery, time away from work and patient satisfaction. The diagnosis is sometimes difficult to make in the immediate post-injury period. The disproportionate nature of the pain in the absence of differential diagnoses and the presence of a pain-free interval should arouse suspicion. The natural history consists of spontaneous improvement except in some refractory forms. The treatment is multidisciplinary combining a physician specializing in pain, therapist and psychologist coo

Improvements in Pinning Fixation for Distal Radius Fractures.

Despite the increasingly widespread use of the volar plate, the pinning of the distal radius keeps its place as a basic technique in the treatment of extra-articular and intra-articular fractures with dorsal displacement and little comminution. To date, there are 2 types of pinning recognized as stable and reliable: the "cross-pinning" with 2 styloid pins and a dorso-ulnar one and the multiple mixed pinning, which is an improved version of the classical technique of Kapandji. This article offers a modified technique of mixed multiple pinning: the pinning type Fort-de-France, which offers a mor

Complications of pediatric distal radius and forearm fractures.

Distal radius and forearm fractures represent a large percentage of pediatric fractures. The most common mechanism of injury is a fall onto an outstretched arm, which can lead to substantial rotational displacement. If this rotational displacement is not adequately addressed, there will be resultant loss of forearm motion and subsequent limitations in performing the activities of daily living. Good initial reductions and proper casting techniques are necessary when treating distal radius and forearm fractures nonsurgically; however, maintaining an acceptable reduction is not always possible. Ultrasound-assisted closed reduction of distal radius fractures.

PURPOSE: To assess the accuracy and ability of ultrasound for monitoring closed reduction for distal radius fractures. METHODS: Consecutive patients undergoing ultrasound-guided closed reduction of acute, displaced distal radius fractures between January 2003 and December 2006 at our department were enrolled. The control group was extracted from patients who underwent a closed reduction for similar fractures under fluoroscopy or without any imaging assistance. To confirm the accuracy of the ultrasonography measurements, displacement distance values were compared with those on radiographic ima

Distal radius fractures: surgical treatment with internal fixation.

AIM OF THE STUDY: To evaluate the efficacy of plates and screws with angular stability in osteosynthesis of fractures of the distal radius. MATERIALS AND METHODS: We analyzed 52 patients undergone surgery for fracture of the distal radius at the Clinica Ortopedica dell'Università di Trieste; in half of the patients osteosynthesis was provided using Synthes' LCP plates, in the other half we utilized a multidirectional and angular stable plate ("Aptus Radius" by Medartis). Mean follow-up was 28 ± 10 months, patients were evaluated with DASH score (disability of arm shoulder and hand) and the mo

Minimally invasive plate osteosynthesis for distal radius fractures.

BACKGROUND: Fractures of distal radius are common injury in all age groups. Cast treatment with or without close reduction is a viable option. However, the results are often unsatisfactory with restricted function. The open reduction and internal fixation often results in extensive soft tissue dissection and associated high rates of infect and delayed/nonunion. The distractor/external fixator have reported good functional and anatomical results but the incidence of pin traction infection nerve injury and cosmedic deformity are high. We introduced a modified operative technique for minimally in

Factors in redisplacement of paediatric distal radius fractures.

Redisplacement after manipulation can occur in distal radius fractures. We performed a retrospective study of 73 patients undergoing manipulation with or without K-wire fixation. From the preoperative, intraoperative and postoperative films we measured anteroposterior and lateral angulation and translation. Reangulation of each fracture was calculated. Factors demonstrating significant correlation with reangulation were age, initial radial translation, ulna fracture, distance from fracture to physis and inadequate correction of translation or angulation. This emphasizes the importance of adequ

Percutaneous pinning of fractures of the distal radius.

Reduction of fractures of the distal radius is often supplemented with percutaneous pinning, but there is little evidence that this affects the clinical outcome. A total of 43 patients with pinned, and 296 with conservatively-treated, fractures were reviewed a mean of 6 (range 3-13) years after injury. We found controls among the conservatively-treated patients who matched 30 of the patients with pinned fractures with respect to age, sex, trauma energy, and radiographic measurements at injury. Clinical and radiological results of the two groups were compared and contrasted. There was a trend f

Correction of nascent malunion of distal radius fractures.

OBJECTIVE: Early correction of malunion of distal radius fractures may be necessary in young, active patients. The aim of this study was to report the advantages and disadvantages of early correction of distal radius malunion. METHODS: Eleven patients (10 male, 1 female; average age 36 years, range: 20 to 54 years) with nascent malunion of distal radius fractures were included in this study. The malunions occurred after initial conservative treatment of unstable distal radius fractures. Mean interval between injury and corrective surgery was 12 (range: 8 to 15) weeks. Distal radius fractures: treatment using the Epibloc system.

The Epibloc system of percutaneous endomedullary internal fixation is best indicated for AO type A2-3 extra-articular fractures of the distal radius and AO type C1 slightly comminuted articular fractures. This system includes pins that can be inserted into the medullary canal and advanced without breaking through the second cortex. This fixation is stabilised by an external plate and rendered dynamic by the pins' elasticity which compresses the fractured surfaces. Moreover, this system results in transversal ligamentotaxis which helps prevent secondary reduction losses. The result of a prelimi

Dorsal nail plate fixation of distal radius fractures.

Intramedullary fixation of distal radius fractures is an emerging concept. The Dorsal Nail Plate is a new device that links a specifically designed stem to a small distal plate; bone fixation is supported by fixed-angle screws. The device is inserted dorsally using a minimally invasive technique ; its indications are unstable dorsally displaced extra-articular or simple articular distal radius fractures. To evaluate the results obtained with this implant, we retrospectively studied 47 extra-articular (36 cases) or simple articular (11 cases) dorsally displaced distal radius fractures in 46 pat

Arthritis predicting factors in distal intraarticular radius fractures.

INTRODUCTION: Intra-articular malunion with step off and gap formation is claimed to be crucial for radiocarpal degeneration. In addition to well-defined intraarticular malunion, the shape of the distal radius is important for unaffected wrist function. In typical intra-articular fracture patterns with a dorsoulnar and palmar ulnar fragment, alterations of the shape of the articular surface, in a kind of a cavity, without obvious step off can be observed. The aim of the present study is to determine the residual articular deformity following intra-articular radius fractures and to analyze thei

[Method of functional treatment of distal radius fractures].

PURPOSE OF THE STUDY To present the method of functional treatment of distal radius fractures, and to evaluate its results in a group of 57 patients. MATERIAL The method described here was used to treat a Colles fracture in 57 patients between 2001 and 2005. The average age of patients at the time of injury was 62 years (range, 19 to 82). The minimal interval between the end of treatment and the evaluation of the patient group was one year. METHODS The presented method is based on the principles of functional treatment of wrist fractures, as advocated by Sarmiento et al. When the patient has b

Combined fractures of the distal radius and scaphoid.

Combined fractures of the distal radius and scaphoid are uncommon, are usually the result of a high-energy trauma and there is no consensus regarding their optimal management. We present a retrospective study of ten patients, out of whom nine underwent internal fixation of their fractures. Open reduction and internal fixation were performed in six of the eight intraarticular fractures of the distal radius. After a mean follow-up of 40 months, eight patients reported no pain and the mean range of wrist motion was 55 degrees flexion and 71 degrees extension. Our current management protocol is ou

Complications of volar plating of distal radius fractures.

Open reduction and internal fixation has been shown to be effective in the treatment of unstable distal radius fractures. When a dorsal approach is used, extensor tendons rupture or irritation are frequent and well known complications. Complications associated with volar plate fixation have not been studied to the same extent. In this study a homogenous series of 90 patients treated by volar plate fixation were retrospectively evaluated, focusing on the complications observed. The overall rate of complications was 8% (7 cases). Tendon rupture or irritation of extensor (3 cases) and flexor tend

Fragment-specific internal fixation of distal radius fractures.

The treatment of complex, intra-articular distal radius fractures can be challenging. Goals of treatment include the restoration of articular congruity and maintenance of a stable reduction to allow for early range-of-motion, prevention of arthrosis, and ultimately return of upper extremity function. Proper understanding of individual fracture patterns is paramount in achieving these goals and avoiding complications. Through the use of limited surgical incisions and low-profile,anatomically contoured implants, fragment-specific internal fixation provides a rational approach to the treatment of

Assessment of articular displacement of distal radius fractures.

Intraarticular step and gap displacements represent the most common indication for surgical treatment of distal radius fractures. Most often, treatment decision making relies only on good-quality plain radiographs taken before and after reduction with measurement accuracy maximized by using the longitudinal axis method. When plain radiographs alone prove insufficient, CT scans or tomograms will significantly improve interobserver and intraobserver reliability of measurements, especially when evaluated using the arc method. Tomography is an effective method for postoperative evaluation of fract
