Preoperative identification of patients with increased risk for perioperative bleeding.

PURPOSE OF REVIEW: Although the overall complication rate in cardiac surgery has been decreased, perioperative bleeding increasing morbidity and mortality is still frequent. Furthermore, the widespread use of new antithrombotic and antiplatelet agents presents an additional challenge in daily practice. Therefore, identifying patients with increased bleeding risk would be advantageous to optimize perioperative management. RECENT FINDINGS: Bleeding classifications are frequently discussed, but are of little relevance for the perioperative setting. In the nonsurgical setting the most relevant ri

Bivalve cartilage inlay myringoplasty: an office-based procedure for closing small to medium-sized tympanic membrane perforations.

OBJECTIVE: To determine whether bivalve inlay cartilage-perichondrium myringoplasty (BCM) is successful in closing tympanic membrane perforations in an office setting. STUDY DESIGN: Retrospective case review. SUBJECTS AND METHODS: Adult patients with chronic perforations underwent BCM under local and topical anesthesia. Success was defined as complete closure of perforation at follow-up of at least 1 month. Predictors of success were identified by comparing the success and failure groups on pre- and postoperative pure tone average (PTA), patient demographics (age, gender), and characteristic

The contribution of proteomics to the identification of biomarkers for cutaneous malignant melanoma.

Despite significant advances in treatment of the melanoma during the past decade, the survival rate is little improved. A contributory factor to the poor outcome is the lack of appropriate sensitive and specific diagnostic, prognostic, and therapeutic response biomarkers. Many serum biomarkers have been evaluated in melanoma but their poor sensitivity and specificity remain serious limitations for their routine use in the clinical setting. Advances in proteomic instrumentation and methodology represent a very promising approach for improving the detection of new candidate markers or pattern of

A 5-year evaluation of a methadone medical maintenance program.

Methadone medical maintenance (MMM) is a model for the treatment of opioid dependence in which a monthly supply of methadone is distributed in an office setting, in contrast to more highly regulated settings where daily observed dosing is the norm. We assessed patient characteristics and treatment outcomes of an MMM program initiated in the Bronx, New York, in 1999 by conducting a retrospective chart review. Participant characteristics were compared with those of patients enrolled in affiliated conventional methadone maintenance treatment programs. Patients had diverse ethnicities, occupations

Can spinal deformity patients maintain proper arm positions while undergoing full-body X-ray?

UNLABELLED: Obtaining proper lateral full-body X-rays is paramount in accurately and consistently evaluating sagittal spinal alignment. This study explored the patient compliance rate of maintaining standardized arm position (fingers on the clavicles with shoulders in 45° of forward elevation while the patient is in a free-standing posture) during full-body X-rays at a single institution. HYPOTHESIS: The compliance rate of arm positioning during full-body X-rays varies depending on operative status (preoperative vs postoperative), age, and diagnosis. DESIGN: Retrospective cohort. INTRODUCTI

International Consensus on Standard Outcome Measures for Neurodevelopmental Disorders: A Consensus Statement.

IMPORTANCE: The use of evidence-based standardized outcome measures is increasingly recognized as key to guiding clinical decision-making in mental health. Implementation of these measures into clinical practice has been hampered by lack of clarity on what to measure and how to do this in a reliable and standardized way. OBJECTIVE: To develop a core set of outcome measures for specific neurodevelopmental disorders (NDDs), such as attention-deficit/hyperactivity disorder (ADHD), communication disorders, specific learning disorders, and motor disorders, that may be used across a range of geogra

Developing a Diabetes Discharge Order Set for Patients With Diabetes on Insulin.

BACKGROUND: Insulin, a high-risk medication, is prone to prescribing errors. Patients with diabetes experience higher hospitalization rates and extended hospital stays. Prescription errors, such as missing orders, inappropriate insulin type, missing instructions, and lack of appropriate intensification of insulin regimens are common issues. This project explored the use of system-based interventions and educational tools to minimize errors and improve the quality of insulin discharge regimens. METHODS: A needs assessment and baseline chart review were conducted before adapting a diabetes orde

Risk factors for nosocomial pneumonia in the elderly.

PURPOSE: Pneumonia is now the second most frequent hospital-acquired infection in acute-care facilities, and recent studies indicate that the incidence rate for nosocomial pneumonia in long-term-care facilities is of similar magnitude. The mortality rate is high, especially in the elderly. With prevention of this complication as an overall priority, this study was designed to determine the risk factors associated with nosocomial pneumonia in the elderly in both acute-care and long-term-care settings. PATIENTS AND METHODS: An epidemiologic case-control study was undertaken to compare patients

A review of the literature on atrial fibrillation: rate reversion or control?

AIMS: The aim of this paper is to review the current literature describing the aetiology of atrial fibrillation and to examine the evidence for rate reversion and rate control. BACKGROUND: Atrial fibrillation is the most commonly seen arrhythmia within the clinical setting. Treatment depends on severity of symptoms, which are predominantly palpitations and shortness of breath. The primary complications from atrial fibrillation are thrombo-embolic events (such as a pulmonary embolus or stroke). OBJECTIVES AND METHODS: A comprehensive literature review on atrial fibrillation, rate reversion an

Central neck lymph node dissection for papillary thyroid cancer: comparison of complication and recurrence rates in 295 initial dissections and reoperations.

BACKGROUND: The American Thyroid Association recently changed its management guidelines for papillary thyroid cancer (PTC) to include routine central neck lymph node dissection (CLND) during thyroidectomy. We currently perform CLND during thyroidectomy only if enlarged central nodes are detected by palpation or ultrasonography; we perform CLND in the reoperative setting for recurrence in previously normal-appearing or incompletely resected nodes. Critics of this approach argue that reoperative CLND has higher complication and recurrence rates than initial CLND. We sought to test this argument,

Cardiopulmonary exercise testing in the assessment of pulmonary hypertension.

The assessment of patients with suspected or confirmed pulmonary arterial hypertension (PAH) and secondary pulmonary hypertension (PH) continues to evolve and, in recent years, evidence demonstrating that cardiopulmonary exercise testing (CPX) provides valuable information has grown at an impressive rate. The key premise supporting the use of CPX is that certain variables obtained provide insight into the degree of ventilation/perfusion mismatching secondary to altered pulmonary hemodynamics. In this article, we discuss the pathophysiology of PAH and secondary PH and its impact on cardiac func
