Termination of convulsion seizures by destabilizing and perturbing seizure memory engrams.

Epileptogenesis, arising from alterations in synaptic strength, shares mechanistic and phenotypic parallels with memory formation. However, direct evidence supporting the existence of seizure memory remains scarce. Leveraging a conditioned seizure memory (CSM) paradigm, we found that CSM enabled the environmental cue to trigger seizure repetitively, and activating cue-responding engram cells could generate CSM artificially. Moreover, cue exposure initiated an analogous process of memory reconsolidation driven by mammalian target of rapamycin-brain-derived neurotrophic factor signaling. Pharmac

Updated review of rescue treatments for seizure clusters and prolonged seizures.

INTRODUCTION: Although the treatment of epilepsy primarily focuses on prevention, recurrent seizures are unfortunately an ongoing reality, particularly in people with epilepsy who live with chronic refractory seizures. Rescue medications are agents which can be administered in urgent/emergent seizure episodes such as seizure clusters or prolonged seizures with the goal of terminating seizure activity, preventing morbidity, and decreasing the risk of further seizures. AREAS COVERED: This review first discusses clinical opportunities for rescue medications, with particular attention focused on

Risk of recurrence of seizures following single unprovoked idiopathic seizure.

A prospective study was conducted to look for various factors that could predict the risk of recurrence of a single unprovoked idiopathic seizure. Seventy six patients with a history of single episode of seizure ultimately completed the study and the data regarding age, sex, duration of seizure, time of occurrence of the ictus, interval between onset and referral, family history of seizure and alcohol consumption were analysed. All patients of symptomatic epilepsy and those with an abnormal scan were excluded. The patients were randomized into two groups, one of which received anti epileptic m

New therapies in the management of acute or cluster seizures and seizure emergencies.

Acute repetitive seizures, also known as cluster or serial seizures, may present considerable health risks, including death, to patients with epilepsy and often are a cause of psychosocial stigma. Fortunately, new formulations and drug-delivery systems approved by the Food and Drug Administration (FDA) over the past 5 years offer patients and health care providers greater control over acute repetitive seizures and emergency situations. Drugs that can be given intravenously, such as diazepam, phenytoin and valproate sodium solution, typically have the most rapid onset but are limited to use in

Semiology of temporal lobe seizures: value in lateralizing the seizure focus.

PURPOSE: To determine the lateralizing value of the clinical manifestations of seizures in patients with temporal lobe epilepsy (TLE), we made a retrospective videotape analysis of complex partial seizures (CPS) in 55 patients who underwent temporal lobectomy and were seizure-free postoperatively for >2 years. METHODS: Blinded to clinical details, we reviewed videotapes from video-EEG telemetry monitoring with attention paid to seizure semiology. RESULTS: Useful lateralizing features included unilateral clonic activity (with the seizure focus contralateral in all patients), unilateral dyston

Seizures and seizure-like states in the child: an approach to emergency management.

A practical approach to the emergency management of seizures and seizure-like nonepileptic states in childhood has been presented. The management of tonic-clonic, absence, partial complex, and focal status epilepticus was discussed. Management of the child who has had a recent episode but does not present with a seizure was outlined, with particular attention to relevant history taking, general and neurologic examinations, laboratory tests, and treatment options. Finally, seizure-like nonepileptic disorders were reviewed and differential diagnostic points highlighted.

Bupropion seizure proportion among new-onset generalized seizures and drug related seizures presenting to an emergency department.

Bupropion is a relatively new and popular medication with seizures as its major side effect. This drug can produce seizures with an overdose. The purpose of this investigation was to determine the relative importance of this medication as the etiology of new-onset seizures relative to other drugs and new-onset seizures in general. The study design was a retrospective case series. All new onset generalized seizures were evaluated over a 4-year period in subjects 16 years of age and older. Etiologic diagnosis was determined from the neurology consultation and all patients with new-onset seizures

Seizures in childhood cerebral adrenoleukodystrophy.

AIM: To describe seizure prevalence in childhood cerebral adrenoleukodystrophy (CCALD) and the relationship to disease severity/progression. METHOD: This was a retrospective observational cross-sectional study of patients with CCALD at two leukodystrophy centers between 2008 and 2024 compared to patients with X-linked adrenoleukodystrophy (X-ALD) without cerebral lesions. Patients with CCALD were under 18 years old, had genetically confirmed X-ALD, and had brain magnetic resonance imaging (MRI) consistent with CCALD. Statistics included: descriptive values (mean, median, range) and clinical v

Hyperglycemia-induced Occipital Lobe Seizures.

Hyperosmolar hyperglycemic state (HHS) and nonketotic hyperglycemia (NKH) are recognized complications of type 2 diabetes mellitus that can cause seizures. Occipital lobe seizures with isolated visual symptoms are rare and underrecognized. We present the case of a 70-year-old male with uncontrolled type 2 diabetes who experienced sudden onset of positive visual phenomena and a right inferior visual field defect. The patient had a hemoglobin A1c > 14% (∼130 mmol/mol) (reference range: < 6.5%; < 48 mmol/mol), no ketones, a normal anion gap, and a normal serum osmolality. Brain magnetic resonance

Gyratory Seizures in Hypothalamic Hamartoma.

Gyratory seizures (GS) are a rare seizure type characterized by body rotation of ≥180° around its vertical axis. While GS have been documented in various epileptic syndromes, their occurrence in association with hypothalamic hamartomas (HH) has not been reported previously. This case report introduces the first documented instance of GS in a patient with a HH, a non-neoplastic tumor originating from the tuber cinereum. The patient, a 25-year-old female, with a history of recurrent seizures since childhood, initially presented with gelastic seizures, marked by inappropriate laughter, and subseq

Aortic Dissection Associated With Seizures.

We present a case of an extensive aortic dissection identified in a young male patient with atypical symptoms. The patient presented to the Emergency Department with severe sudden onset chest pain and seizure. In the Emergency Department, bedside Transthoracic Echocardiography (TTE) identified moderate pericardial effusion which correlated with a widened mediastinum on chest radiograph. The presence of aortic dissection was confirmed by an urgent CT angiogram of the thorax and abdomen. Our case highlights the importance of critical thinking in the atypical presentation of aortic dissection in

Neurocysticercosis-related seizures: Imaging biomarkers.

Neurocysticercosis (NCC)-a parasitic CNS infection endemic to developing nations-has been called the leading global cause of acquired epilepsy yet remains understudied. It is currently unknown why a large proportion of patients develop recurrent seizures, often following the presentation of acute seizures. Furthermore, the presentation of NCC is heterogenous and the features that predispose to the development of an epileptogenic state remain uncertain. Perilesional factors (such as oedema and gliosis) have been implicated in NCC-related ictogenesis, but the effects of cystic factors, including

Postoperative Epileptic Seizures in Children.

Background: Postoperative seizures (PS) occur in 10−15% of patients. This study aims to provide an update on the role of surgery in PS. Methods: All children undergoing a craniotomy for supratentorial lesions in the last 10 years were considered except those with preoperative seizures, perioperative antiepileptic drugs prophylaxis, head-injury and infections, repeated surgery, or preoperative hyponatremia. Children undergoing surgery for intra-axial lesions (Group 1, 74 cases) were compared with those harboring extra-axial lesions (Group 2, 91 cases). Results: PS occurred in 9% of 165 cases an

Ictal fear during parietal seizures.

Ictal fear is characterized by a subjective sensation of fear and consistent clinical manifestations during seizures. This phenomenon is rarely observed in parietal seizures. We report anatomical electroclinical correlations between an SEEG-recorded seizure and prominent fear semiology. The seizure onset zone was quantified using the Connectivity Epileptogenicity Index (cEI) method. Occurrence of fear during seizures was related to the involvement of the left inferior parietal cortex and the superior temporal gyrus without amygdala involvement. Our case confirms that parietal seizures can prod

Novel Therapeutics for Neonatal Seizures.

Neonatal seizures are a common neurologic emergency for which therapies have not significantly changed in decades. Improvements in diagnosis and pathophysiologic understanding of the distinct features of acute symptomatic seizures and neonatal-onset epilepsies present exceptional opportunities for development of precision therapies with potential to improve outcomes. Herein, we discuss the pathophysiology of neonatal seizures and review the evidence for currently available treatment. We present emerging therapies in clinical and preclinical development for the treatment of acute symptomatic ne

Seizures in patients with cancer.

Seizures are common in patients with cancer and either result from brain lesions, paraneoplastic syndromes, and complications of cancer treatment or are provoked by systemic illness (metabolic derangements, infections). Evaluation should include a tailored history, neurologic examination, laboratory studies, neuroimaging, and electroencephalogram. In unprovoked seizures, antiepileptic drug (AED) treatment is required, and a nonenzyme-inducing AED is preferred. Treatment of the underlying cancer with surgery, chemotherapy, and radiation therapy also can help reduce seizures. Benzodiazepines are
