Current options for the treatment of impetigo in children.

Impetigo contagiosa is a common, superficial, bacterial infection of the skin characterised by an inflamed and infected epidermis caused by Staphylococcus aureus, Streptococcus pyogenes or both. The less common bullous impetigo is characterised by fragile fluid-filled vesicles and flaccid blisters, and is invariably caused by pathogenic strains of S. aureus. In bullous impetigo, exfoliative toxins are produced, although these are restricted to the area of infection and bacteria can be cultured from the blister contents. In the rare variant, staphylococcal scalded skin syndrome, the exfoliative

Humoral immune deficiency and hemifacial microsomia seen in one family.

We present a patient with hemifacial microsomia and immune deficiency. The patient is a 5-year-old with grade III microtia and Pruzansky type I right mandibular hypoplasia. She developed 25 pulmonary infections in 3 years, required hospitalization every 6 weeks to receive antibiotics, and experienced recurrent herpes stomatitis and esophagitis, staphylococcal bacteremia, urinary tract, sinus, and ear infections. She had low total IgG, IgG1, IgG2, IgA, and anti-pneumococcal antibody levels. She was unable to maintain protective pneumococcal titers following vaccination. The patient's 7-year-old

Erythromycin: a microbial and clinical perspective after 30 years of clinical use (2).

Erythromycin, first introduced for clinical use 30 years ago, was found to be effective for the treatment of gram-positive bacterial infections. Emergence of resistance and the advent of penicillinase-resistant penicillins limited the use of erythromycin for serious staphylococcal infections; however, erythromycin remains among the drugs of choice for the treatment of acne, infections of the skin and soft tissues, streptococcal pharyngitis, bronchitis, pneumonitis, diphtheria, carriers of pertussis, and, when administered with a sulfonamide, otitis media. Erythromycin is the drug of choice for

Real-World Experience with Oritavancin Therapy in Invasive Gram-Positive Infections.

INTRODUCTION: Oritavancin is a novel lipoglycopeptide approved for acute bacterial skin and skin structure infections. The pharmacokinetic profile and convenient one-time dosing make oritavancin an enticing option for other serious Gram-positive infections requiring prolonged treatment courses. Unfortunately, data for using oritavancin in these populations are limited. METHODS: We report ten cases of oritavancin use for invasive Gram-positive infections in our health system, and provide a review of the currently available literature regarding oritavancin therapy for invasive infections. Bacteria and the skin: clinical practice and therapy update.

Any doctor using antibiotics should be aware of the increasing worldwide problem with multiresistant bacteria, with the majority of hospital-based infections in some countries being caused by these bacteria. Proper use of antibiotics is therefore mandatory for any physician, including for dermatologists, who treat bacterial infections of the skin. Detailed knowledge is needed of when to use topical versus systemic antibiotics, and for how long such treatments should be given. Besides the clinical symptoms of bacterial infections and treatment guidelines, an increased awareness has focused on t

The integration of physiologically-targeted skin care in the management of atopic dermatitis: focus on the use of a cleanser and moisturizer system incorporating a ceramide precursor, filaggrin degradation products, and specific "skin-barrier-friendly" excipients.

Atopic dermatitis (AD) may be considered the "poster disease" for exemplifying the significance of abnormalities of the epidermal barrier that occur predominantly within the stratum corneum (SC) and upper epidermis. Specifically, impairments of the SC permeability barrier, antimicrobial barrier, and immunologic barrier contribute markedly to the fundamental pathophysiology of AD. The multiple clinical sequelae associated with epidermal barrier impairments inherent to AD include dry skin, pruritus, increased skin sensitivity to irritants and allergens, eczematous skin changes, staphylococcal sk

Skin infections in eastern Panama. Survey of two representative communities.

A skin infection survey of 1,084 people was carried out in two jungle villages in eastern Panama. Bacterial pyoderma was the most prevalent infection, affecting 25% of boys, 15% of girls, and 11% of those over 10 years of age. Streptococcus pyogenes and Staphylococcus aureus were recovered from 84% of pyodermas cultured. Nearly 60% of the cutaneous staphylococcal isolates were resistant to penicillin. Hippelates flies were seen feeding on purulent skin lesions and may have been important in transmission. Scabies, ringworm, candidiasis, and cutaneous leishmaniasis were in comparison with pyoder

IBT-V02: A Multicomponent Toxoid Vaccine Protects Against Primary and Secondary Skin Infections Caused by Staphylococcus aureus.

Staphylococcus aureus causes a wide range of diseases from skin infections to life threatening invasive diseases such as bacteremia, endocarditis, pneumonia, surgical site infections, and osteomyelitis. Skin infections such as furuncles, carbuncles, folliculitis, erysipelas, and cellulitis constitute a large majority of infections caused by S. aureus (SA). These infections cause significant morbidity, healthcare costs, and represent a breeding ground for antimicrobial resistance. Furthermore, skin infection with SA is a major risk factor for invasive disease. Here we describe the pre-clinical

Staphylococcal scalded skin syndrome in adults with obesity and type 2 diabetes: A case series.

Staphylococcal scalded skin syndrome (SSSS) primarily affects children and rarely adults with immunodepression. We describe two cases of adults diagnosed with SSSS with no additional cause of immunological compromise other than obesity and uncontrolled diabetes. An increased risk of infection should be considered in cases of obesity and diabetes.

Mastitis and other diseases of the goat's udder.

Udder problems of modern dairy goats are similar to those seen in dairy cows. Anomalies of the goat's udder and teats are common, and many may be hereditary. Skin diseases of the udder include viral infections, mange, sunburn, wounds, and staphylococcal dermatitis. There are numerous known causes of caprine mastitis. These include streptococci, hemolytic and nonhemolytic staphylococci, corynebacteria, and mycoplasmas. Diagnosis of mastitis in goats is often difficult, as the udder secretion may remain grossly normal and somatic cell counts in nonmastitic goats are higher than the recognized no

Telavancin: a novel lipoglycopeptide for serious gram-positive infections.

Telavancin, a once-daily dosing lipoglycopeptide derived from vancomycin, has a broad-spectrum microbiologic activity against gram-positive bacteria, including vancomycin-resistant staphylococci. Telavancin displays a dual mode of action and a rapid bactericidal killing. The in vitro activity of telavancin is superior to vancomycin and comparable with, or greater than, linezolid, daptomycin and other novel lipoglycopeptides. Telavancin is effective against gram-positive pathogens in animal models of soft tissue infections and deep-seated infections including endocarditis, pneumonia and bactere

Coral Dermatitis or Infectious Dermatitis: Report of a Case of Staphylococcus Aureus Infection of Skin After Scuba Diving.

Skin lesion which develops after deep sea diving is termed as coral dermatitis. The corals are known to produce a toxic substance which when comes in to contact with human skin may elicit hypersensitive reactions. Most previous reports highlight the allergic reactions caused by deep sea diving. This is a rare case of staphylococcal skin infection in a second-year medical student caused by Staphylococcus aureus; he reported a history of deep sea diving before being presented to the hospital with skin rashes. This case highlights the importance of considering infectious aetiology in cases of cor

Not just in man's best friend: A review of Staphylococcus pseudintermedius host range and human zoonosis.

Staphylococcus pseudintermedius is one species in the commensal staphylococcal population in dogs. While it is commonly carried on healthy companion dogs it is also an opportunistic pathogen associated with a range of skin, ear, wound and other infections. While adapted to dogs, it is not restricted to them, and we have reviewed its host range, including increasing reports of human colonisation and infections. Despite its association with pet dogs, S. pseudintermedius is found widely in animals, covering companion, livestock and free-living species of birds and mammals. Human infections, typic

A Systemic Review on Staphylococcal Scalded Skin Syndrome (SSSS): A Rare and Critical Disease of Neonates.

The symptoms of Staphylococcal scalded skin syndrome (SSSS) include blistering of skin on superficial layers due to the exfoliative toxins released from Staphylococcus aureus. After the acute exfoliation of skin surface, erythematous cellulitis occurs. The SSSS may be confined to few blisters localized to the infection site and spread to severe exfoliation affecting complete body. The specific antibodies to exotoxins and increased clearence of exotoxins decrease the frequency of SSSS in adults. Immediate medication with parenteral anti-staphylococcal antibiotics is mandatory. Mostly, SSSS are

[Multiple neonatal staphylococcal cold abscesses in large skin folds].

Multiple neonatal staphylococcal cold abscesses in large skin folds (MNSCA) is a benign infection in newborns. It is characterised by abscess formation in large skin folds with mild local signs of inflammation but no systemic symptoms. Diagnosis is based on clinical presentation and culture of Staphylococcus aureus. In this case report, a 9-day-old girl with MNSCA is presented. MNSCA has an excellent prognosis, with or without treatment. Recognising this condition can prevent unnecessary diagnostic work-ups and antibiotic use.

[Atypical manifestation of bullous, multifocal erysipelas on the trunk-case report].

Erysipelas is an acute bacterial infection of the skin and lymphatic vessels, predominantly caused by Streptococcus pyogenes. Atypical localizations such as the trunk, as well as rare clinical variants including bullous or multifocal erysipelas, may complicate the diagnostic process and require differentiation from other blistering or widespread erythematous conditions, such as staphylococcal scalded skin syndrome (SSSS), toxic epidermal necrolysis (TEN) or cellulitis. In immunocompromised or oncologically pretreated patients, erysipelas should be considered even in unusual presentations.
