Seated Tai Chi Improves Dynamic Finger-Pointing Task and Sitting Balance Control in Subjects with Parkinson's Disease: A Prospective Comparative Observational Study.

INTRODUCTION: The aim of this study was to investigate the effects of 3 months of seated Tai Chi (TC) practice on the eye-hand coordination and sitting balance control of subjects with Parkinson's disease. METHOD: A prospective non-randomized study was conducted with 29 subjects practicing TC and 27 subjects as controls according to their preference. The seated TC group underwent 3 months of training with a total of 24 sessions (1 h/session, 2 sessions/week). The outcome measures included a dynamic finger-pointing task (reaction time, movement time, and accuracy), and a sequential weight-shif

Divergent subcortical activity for distinct executive functions: stopping and shifting in obsessive compulsive disorder.

BACKGROUND: There is evidence of executive function impairment in obsessive compulsive disorder (OCD) that potentially contributes to symptom development and maintenance. Nevertheless, the precise nature of these executive impairments and their neural basis remains to be defined. METHOD: We compared stopping and shifting, two key executive functions previously implicated in OCD, in the same task using functional magnetic resonance imaging, in patients with virtually no co-morbidities and age-, verbal IQ- and gender-matched healthy volunteers. The combined task allowed direct comparison of neu

Increased anterior cingulate cortex response precedes behavioural adaptation in anorexia nervosa.

Patients with anorexia nervosa (AN) are characterised by increased self-control, cognitive rigidity and impairments in set-shifting, but the underlying neural mechanisms are poorly understood. Here we used functional magnetic resonance imaging (fMRI) to elucidate the neural correlates of behavioural adaptation to changes in reward contingencies in young acutely ill AN patients. Thirty-six adolescent/young adult, non-chronic female AN patients and 36 age-matched healthy females completed a well-established probabilistic reversal learning task during fMRI. We analysed hemodynamic responses in em

Comparison of the basal ganglia and cerebellum in shifting attention.

The basal ganglia and cerebellum have traditionally been associated with motor performance. Recently, there has been considerable interest regarding the contributions of these subcortical structures to aspects of cognition. In particular, both the basal ganglia and cerebellum have been hypothesized to be involved in the control of attentional set. To date, no neuropsychological studies have directly compared the effects of basal ganglia and cerebellar dysfunction on the same attention shifting tasks. To this end, we employed an alternating attention task that has been used to demonstrate putat

Type 1 diabetes intervention clusters in resource-limited settings: a systematic review and economic evaluation.

BACKGROUND: Over 9 million people are living with type 1 diabetes globally, with a disproportionate burden in resource-limited settings where access to care remains constrained. Individual interventions have shown modest benefits, but coordinated intervention clusters could achieve greater health impact. We aimed to evaluate the cost-effectiveness of intervention clusters for type 1 diabetes management in resource-limited settings. METHODS: We conducted a systematic review (PRISMA 2020) searching PubMed/MEDLINE, Embase, Cochrane Central Register of Controlled Trials, Web of Science Core Colle

Attentional set-shifting deficit in Parkinson's disease is associated with prefrontal dysfunction: an FDG-PET study.

The attentional set-shifting deficit that has been observed in Parkinson's disease (PD) has long been considered neuropsychological evidence of the involvement of meso-prefrontal and prefrontal-striatal circuits in cognitive flexibility. However, recent studies have suggested that non-dopaminergic, posterior cortical pathologies may also contribute to this deficit. Although several neuroimaging studies have addressed this issue, the results of these studies were confounded by the use of tasks that required other cognitive processes in addition to set-shifting, such as rule learning and working

The Influence of Children's Pain-Related Attention Shifting Ability and Pain Catastrophizing Upon Negatively Biased Pain Memories in Healthy School Children.

The current study investigated the influence of children's ability to flexibly shift attention toward and away from pain information on the development of negatively biased pain memories, thereby employing a direct measure of attention control reliant on behavioral responses in the context of pain (ie, an attention switching task). The direct influence of children's attention-shifting ability and pain catastrophizing as well as the moderating role of this shifting ability in the relationship between pain catastrophizing and the development of negatively biased pain memories was examined. Executive functioning and spatial processing in anorexia nervosa: an experimental study and its significance for the allocentric lock theory.

PURPOSE: The allocentric lock theory (ALT) suggests that people with eating disorders have difficulties in multisensory integration in two reference frames-egocentric and allocentric, whereby the egocentric, but not allocentric, is impaired. This leads to a distorted body image that contributes to the development and maintenance of the disorder. The current study aimed to explore a facet of the ALT, namely, the visuo-spatial aspect, and its relation to cognitive flexibility in patients with anorexia nervosa (AN). METHODS: Fifty-five participants took part in the study: 20 AN patients and 35 c

Feasibility of nurse-led antidepressant medication management of depression in an HIV clinic in Tanzania.

OBJECTIVE: Sub-Saharan Africa has the highest HIV prevalence worldwide and depression is highly prevalent among those infected. The negative impact of depression on HIV outcomes highlights the need to identify and treat it in this population. A model for doing this in lower-resourced settings involves task-shifting depression treatment to primary care; however, HIV-infected individuals are often treated in a parallel HIV specialty setting. We adapted a model of task-shifting, measurement-based care (MBC), for an HIV clinic setting and tested its feasibility in Tanzania. MBC involves measuring

From authoring to editing: a competency framework for trainee use of ambient AI clinical documentation.

Ambient AI has spread through clinical settings faster than curricula can adapt, shifting trainee documentation from authoring to editing notes. Authoring historically honed clinical reasoning for trainees, yet medical education is not prepared to develop the editing task now replacing it. We define this task as a teachable competency with four domains - error detection, clinical reconciliation, independent generation, and trust calibration - and describe how simulation can teach them.

Addressing the Treatment Gap for Pediatric Headache in Underserved Regions.

PURPOSE OF REVIEW: This review explores innovative strategies to address the treatment gap for pediatric headache disorders in underserved regions worldwide. RECENT FINDINGS: Global Burden of Disease studies indicate that the fastest growth in pediatric headache burden occurs in low- and middle-income countries, where reported prevalence likely underestimates true disease burden due to healthcare access barriers. Telemedicine demonstrates comparable efficacy, safety, and satisfaction to in person care. Artificial intelligence based diagnostic models achieve high accuracy for pediatric migrain

Visual attention in long-term survivors of leukemia receiving cranial radiation therapy.

The effect of cranial radiation therapy (CRT) on visual attention was examined in long-term survivors of childhood acute lymphoblastic leukemia (ALL) compared to peers with no history of ALL (n = 24) using a cued orienting task and a global-local task. ALL participants treated with CRT (n = 13) demonstrated an increased cost in response time with invalid spatial orienting cues and inefficient shifts of attention across hierarchical levels. ALL participants treated only with chemotherapy (n = 8) showed performance similar to the non-ALL comparison group. Participants with exposure to CRT early

Set shifting in children and adolescents with anorexia nervosa: an exploratory systematic review and meta-analysis.

OBJECTIVE: Set shifting inefficiencies in adults with anorexia nervosa (AN) are established, however the neurocognitive profile of children and adolescents with AN is less clear. This study aimed to provide a review of the literature. METHOD: Electronic databases were used to search for manuscripts. RESULTS: Meta-analysis was performed on seven studies using two neuropsychological tests (Trail Making Task, TMT; Wisconsin Card Sorting Task, WCST). The mean difference in outcome between AN and healthy control (HC) groups was standardized by calculating Cohen's d. Meta-analysis of TMT studies s

Impairment in flexible emotion-based learning in hallucination- and delusion-prone individuals.

Deficits in emotion-based learning are implicated in many psychiatric disorders. Research conducted with patients with schizophrenia using one of the most popular tasks for the investigation of emotion-based learning, the Iowa Gambling Task (IGT), has largely been inconclusive. The present study employed a novel, contingency-shifting variant IGT with hallucination- and delusion-prone university students to determine whether previous findings were due merely to the presence of psychosis. Following initial screening of a sample of 253 students (mean age = 20.13 years, S.D. = 3.27), 28 high (10 m

Schizophrenia patients show deficits in shifts of attention to different levels of global-local stimuli: evidence for magnocellular dysfunction.

Abnormalities of attention and visual perception are well documented in schizophrenia. The global-local task is a measure of attention and perceptual organization that utilizes visual stimuli comprised of large letters (global level) made up of smaller letters (local level). Subjects identify target letters appearing at either the global or local level of the stimulus. In this study, we used a version of the global-local task specifically designed to examine lateralized hemispheric processing and attention shifting in 30 schizophrenia patients and 24 normal controls. Global-local stimuli were

Risky decision making and cognitive flexibility among online sports bettors in Nigeria.

Online sports betting is a popular recreational activity in Nigeria. Like other forms of gambling, risk of pathological progression exists for gamblers who continue betting despite severe financial and psychosocial consequences. In the present study, we examined whether this population of gamblers shows deficits in decision making and cognitive flexibility that have been documented in Western gambling populations. Thirty-six online sports bettors and 42 non-gambling participants completed a version of the Iowa gambling task (IGT) and an established set-shifting task for the assessment of cogni

Colour or shape: examination of neural processes underlying mental flexibility in posttraumatic stress disorder.

Posttraumatic stress disorder (PTSD) is a mental disorder that stems from exposure to one or more traumatic events. While PTSD is thought to result from a dysregulation of emotional neurocircuitry, neurocognitive difficulties are frequently reported. Mental flexibility is a core executive function that involves the ability to shift and adapt to new information. It is essential for appropriate social-cognitive behaviours. Magnetoencephalography (MEG), a neuroimaging modality with high spatial and temporal resolution, has been used to track the progression of brain activation during tasks of men

[Development and implementation of a regional medical collaboration platform for hematological malignancies].

To address the challenges of regional collaboration in the treatment of hematologic malignancies, we conducted a prospective clinical study of a healthcare communication platform (Dr2GO®) designed to ensure seamless continuity of care by streamlining inter-hospital transfer coordination. By the end of 2024, nineteen patients were enrolled across the four participating institutions. The most frequently used clinical pathway was vaccination after allogeneic hematopoietic stem cell transplantation (7 cases). The median time from the referral request by the hub hospital to confirmation by the comm
