Factors Associated With the Incidence and Timing of Total Knee Arthroplasty Infection.

BACKGROUND: Periprosthetic joint infection (PJI) following total knee arthroplasty remains a challenging clinical problem. This study examined variables related to the incidence and timing of PJI. METHODS: We retrospectively reviewed 8462 primary total knee arthroplasties performed at our institution between 2006 and 2018 for PJI. The mean follow-up is 3.7 years. Eighty-seven variables including patient-reported diagnoses, demographics, and medications were collected. Time to infection, bacterial organism, success of infection treatment, and variables associated with infection are reported.

Erectile Dysfunction as a Cardiovascular Risk Factor: Time to Step Up?

BACKGROUND: Erectile dysfunction (ED) is a major health problem that affects a significant proportion of the general population, and its prevalence is even higher in patients with CV risk factors and/or disease. ED and cardiovascular (CV) disease share several common pathophysiological mechanisms, and thus, the potential role of ED as a predictor of CV events has emerged as a significant research aspect. OBJECTIVE: The purpose of this review is to present and critically discuss data assessing the relation between ED and CV disease and the potential predictive value of ED for CV events. METHO

Risk Factors and Timing of Early Stricture Recurrence After Urethroplasty.

OBJECTIVE: To compare recurrence after urethroplasty, identifying associated risk factors for early recurrence. MATERIALS AND METHODS: Among 262 urethroplasties (2001-2010) with ≥6 months of follow-up, we identified 65 patients (24.8%) with recurrence (defined by obstruction in the area of repair on cystoscopy). RESULTS: Median stricture length was 4.5 cm (range 1-24 cm). Median follow-up was 85.2 (6.7-160.1) months, with median time to recurrence of 8.0 (0.5-88.0) months. Substitution urethroplasty was the most frequent repair (70.8%), followed by excision and primary anastomosis (23.1%). W

Environmental factors and puberty timing: expert panel research needs.

Serono Symposia International convened an expert panel to review the impact of environmental influences on the regulation of pubertal onset and progression while identifying critical data gaps and future research priorities. An expert panel reviewed the literature on endocrine-disrupting chemicals, body size, and puberty. The panel concluded that available experimental animal and human data support a possible role of endocrine-disrupting chemicals and body size in relation to alterations in pubertal onset and progression in boys and girls. Critical data gaps prioritized for future research ini

Investigating factors related to the effects of time-out on stuttering in adults.

BACKGROUND: Response-contingent time-out has been shown to be an effective technique for enhancing fluency in people who stutter. However, the factors that determine individual responsiveness to time-out are not well understood. AIMS: The study investigated the effectiveness of using response-contingent time-out to reduce stuttering frequency in adults who stutter. In addition, it investigated the predictive value of participants' stutter severity, age, previous treatment history, and type of stutter on the responsiveness to time-out conditioning. METHODS & PROCEDURES: Sixty people who stutt

Dual radiobiological interpretations of retrospective clinical data: the time factor.

Dual interpretations are different radiobiological mechanisms that explain theoretically the same observed results. Radiobiological interpretations of the time factor are most frequently based on changes in total dose that produce a given effect. If this dose is increased by different mechanisms (e.g. increasing overall time and decreasing dose per fraction) at the same time, proposals for altered fractionation schemes based on the choice of one or the other mechanism, in principle, can lead to erroneous predictions of outcome. This is especially the case when the analyses are based on retrosp

Temperature-driven modulation of factors influencing full-Marathon time in male recreational runners.

The weather during full-marathon events is expected to become warmer due to global warming, making heat preparations a key challenge for runners. To gain insight into this challenge, we examined whether associations between full-marathon finish time and selected factors differed across the Tsukuba Marathon 2023 and 2022, and the Hokkaido Marathon 2023, which represent optimal, moderate, and extreme temperatures, respectively. Questionnaire-based measurements of full-marathon personal best (PB), age, running experience (career), body mass index (BMI), weekly running distance and frequency, long

Exploration of factors associated with turnaround time when evaluating non-tuberculous mycobacterium cultures.

BACKGROUND: A positive culture of non-tuberculous mycobacteria (NTM) is a key diagnostic criterion for NTM disease. Due to the slow growth rate of NTM, the turnaround time (TAT) for culture specimens is often lengthy, posing significant challenges for the diagnosis and treatment of related diseases. This study aimed to explore factors influencing TAT in NTM culture testing, assess its potential clinical value, and identify ways to expedite clinical decision-making. METHODS: NTM identified by HSP65 sequencing in a tertiary hospital from June 2022 to May 2024 were retrospectively included, and

Factors Associated with Time to Initial Antiretroviral Therapy Discontinuation in the DC Cohort.

When an initial antiretroviral therapy (ART) regimen is effective and well-tolerated, it can be maintained for years as long as the patient adheres. Prior research has revealed that shorter initial ART duration is associated with regimen type, female sex, injection drug use as the HIV transmission category, and lower baseline CD4 count. We examined potential factors associated with initial regimen discontinuation among a subset of newly diagnosed virally unsuppressed PWH in the DC Cohort, an ongoing prospective observation study that uses electronic health record data from clinic sites to coll

Risk Factors Associated with First Time and Recurrent Shoulder Instability: A Systematic Review.

INTRODUCTION: The main cause of postoperative pain after abdominal surgery is the wound where laparotomy is made. Recently, laparoscopic procedures have become common in colorectal surgery. Although improving the tolerance of the surgery, postoperative pain management still remains a discussed problem. The use of an epidural catheter used to be generally recommended in the open surgery era; however, an ideal strategy for postoperative analgesic therapy after laparoscopy remains unclear. Reduced administration of opioid analgesic drugs after colorectal resection is a generally accepted goal. Pr

Factors Associated With Nonadherence to Lung Cancer Screening Across Multiple Screening Time Points.

IMPORTANCE: Screening with low-dose computed tomography (CT) has been shown to reduce mortality from lung cancer in randomized clinical trials in which the rate of adherence to follow-up recommendations was over 90%; however, adherence to Lung Computed Tomography Screening Reporting & Data System (Lung-RADS) recommendations has been low in practice. Identifying patients who are at risk of being nonadherent to screening recommendations may enable personalized outreach to improve overall screening adherence. OBJECTIVE: To identify factors associated with patient nonadherence to Lung-RADS recomm

Factors associated with prolonged cecal insertion time in patients undergoing water exchange colonoscopy.

BACKGROUND AND AIM: Water exchange is superior to air insufflation in reducing discomfort and increasing adenoma detection during colonoscopy. However, prolonged cecal insertion time is a drawback. This study aims to investigate the factors affecting cecal insertion during water exchange colonoscopy. METHODS: We pooled data from five randomized clinical trials that included patients undergoing water exchange colonoscopy. Logistic analysis was performed to determine the independent factors associated with prolonged cecal insertion time (> 15 min). RESULTS: The cohort included 912 patients (49

Process Factors Affecting Reperfusion Time in Patients With ST-Segment Elevation Myocardial Infarction.

OBJECTIVE: To determine process factors impacting myocardial infarction reperfusion time. BACKGROUND: An ST-segment elevation myocardial infarction (STEMI) is a life-threatening condition that necessitates emergent medical treatment; longer reperfusion times are associated with negative patient outcomes. Therefore, time is critical in the treatment of a STEMI. METHODS: Admissions data from Las Vegas hospitals were analyzed via multivariate regression analysis to determine predictors of reperfusion times. The analysis was based on 618 patients presenting with a diagnosis of STEMI at participa

Time course and risk factors of evolution from potential to overt autoimmune gastritis.

BACKGROUND: The natural history of patients with potential autoimmune gastritis (AIG), defined by the presence of serum anti-parietal cell antibody (PCA) positivity and no gastric histopathological alterations, is unknown. We therefore aimed to assess the natural history and clinical correlates of potential autoimmune gastritis (AIG). METHODS: In 2000-2019, we enrolled potential AIG patients by monitoring once a year (±6 months) histopathological evolution into overt AIG, defined as the occurrence of atrophy in the oxyntic mucosa. Factors affecting disease progression were assessed. RESULTS:
