What happens after being not-on-track? Patterns of change, profiles and prediction of at-risk patients.

OBJECTIVES: Clinicians often struggle to accurately identify patients at risk of treatment failure. Feedback-informed treatment offers real-time outcome monitoring to help detect and address potential treatment failures early. This study examined patterns of change among not-on-track (NOT) patients during feedback-informed treatment, explored how patients' characteristics related to these patterns, and assessed the feasibility of predicting them. METHOD: Data from 573 NOT patients in a university outpatient psychotherapy clinic were analyzed. Growth mixture modeling identified distinct change

The effects of sex and trauma type on PTSD symptom reduction in Cognitive Processing Therapy.

Cognitive Processing Therapy (CPT) is an evidence-based treatment for posttraumatic stress disorder (PTSD) that is effective for many, but not all, people. The present study investigated a possible interaction of sex and trauma type to predict variable responses to treatment. Veterans from an outpatient Veterans Health Administration clinic (N = 400) completed the PTSD Checklist before CPT initiation and after treatment completion. Veterans' index traumatic events were coded by PTSD specialists as military operations-related or nonmilitary operations-related traumas. The final symptom levels w

Examining Heterogeneity of Patient Outcomes in a Randomized Controlled Trial for Binge-Eating Disorder Testing Cognitive-Behavioral Therapy and Lisdexamfetamine, Alone and Combined.

OBJECTIVE: Although trials have documented the overall effectiveness of cognitive-behavioral therapy (CBT) and lisdexamfetamine (LDX) for binge-eating disorder (BED), the nature and extent of individual patient variability in outcomes have not been examined. This secondary analysis of a controlled trial examined heterogeneity of outcomes with CBT and LDX, alone and combined, for BED. METHODS: One hundred and forty-one patients with BED with co-occurring obesity were randomized to CBT, LDX, or combined CBT+LDX 3-month treatments. Changes in binge-eating frequency, eating-disorder psychopatholo

Quantitative electroencephalogram differentiation between treatment-resistant and responsive groups following deep transcranial magnetic stimulation in obsessive-compulsive disorder.

OBJECTIVE: To reliably assess biomarkers for treatment response, including clinical factors such as comorbid depression, and evaluate quantitative electroencephalography (qEEG) changes in treatment-resistant obsessive-compulsive disorder (OCD) after deep transcranial magnetic stimulation (dTMS). METHODS: Treatment-resistant OCD sixty patients (35 F, 25 M), after dTMS were separated into two groups based on the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) score, decreased by 35% or more (Treatment-responsive (TRP)) and less than 35% (Treatment-resistant (TRS)). QEEG recordings were compared

Trial-History Effects During WCST in Treatment-Resistant Schizophrenia.

AIM: Treatment-resistant schizophrenia (TRS) is a clinically important subtype of schizophrenia, but its cognitive characteristics remain incompletely understood. The Wisconsin Card Sorting Test (WCST) is widely used to assess executive dysfunction in schizophrenia, although conventional summary analyses may overlook trial-by-trial behavioral adaptation. This study aimed to examine whether recent trial outcome modulates subsequent responding differently between TRS and non-TRS patients during the WCST. METHODS: We analyzed WCST data from 41 outpatients with schizophrenia, including 13 patient

Improvement and deterioration trajectories in psychotherapy for depression: results from an RCT with cognitive behavioural and psychodynamic therapy.

OBJECTIVE: Depression is a heterogeneous disorder with varying trajectories. The aim of this study was to characterize patterns of change in patients with major depressive disorder receiving cognitive behavioural therapy (CBT) or psychodynamic therapy (PDT) and examine baseline factors associated with each trajectory. METHODS: Patients with major depressive disorder (n = 100) were randomized to one of two treatments over 28 weeks: CBT with weekly sessions for 16 weeks followed by three monthly booster sessions or PDT with weekly sessions. Patients were assessed for depressive symptoms with th

Symptom Trajectories in Patients With Anxiety Disorders During Transdiagnostic Cognitive Behavioral Therapy.

Growing research confirms the efficacy of transdiagnostic cognitive behavioral psychotherapy, that is treatments applicable across multiple diagnoses within a disorder class or spectrum, by examining group-level changes from pre- to posttreatment. However, less is known about interindividual differences and dynamic changes during therapy. Understanding different symptom trajectories and their associations with treatment outcome would allow early detection of nonresponders and treatment adjustments. Using data from the treatment group of a randomized controlled trial, we aimed to investigate tr

Age Is an Effect Modifier of Thrombectomy Benefit in a Socioeconomically Vulnerable Stroke Population.

OBJECTIVE: Randomized trials from high-income countries have shown that endovascular treatment (EVT) yields similar benefits in elderly and non-elderly patients with large vessel occlusion stroke (LVOS). However, evidence from low/middle-income countries remains scarce. We aimed to assess the impact of age on outcomes and determine whether age modifies the effect of EVT under the resource-limited conditions of a public healthcare system in a developing country. METHODS: Pre-specified subgroup analysis of the RESILIENT trial, a multicenter, randomized, controlled trial in Brazil comparing EVT

Personality Pathology and Functional Outcomes During Pharmacological Treatment of Adult ADHD.

Adults with ADHD show substantial variability in response to pharmacological treatment, and comorbid personality pathology may help explain why symptom improvement often fails to translate into meaningful functional improvement. This naturalistic longitudinal study examined whether dimensional personality pathology predicts symptomatic and functional outcomes during routine stimulant treatment. A total of 246 adults with ADHD (66% women; mean age = 33.5) contributed repeated assessments over irregular follow-up intervals, with substantial attrition across waves. Linear mixed-effects models wer

Varying Treatment Effects in Subgroups: A Unified Framework for Longitudinal Data Analysis.

The rise of precision medicine has shifted statistical analysis from population-averaged treatment effects to subgroup-specific effects, highlighting the importance of tailoring treatments to distinct patient subgroups. Longitudinal data, in particular, provides an opportunity to study how treatment effects evolve over time. However, methods for analyzing subgroup-specific, time-varying treatment effects in longitudinal settings remain limited. Most current approaches rely on two-step procedures with restrictive assumptions, such as a fixed number of subgroups or prespecified correlation struc

Heterogeneity of pathological tumour response in patients with multiple colorectal liver metastases undergoing resection after preoperative chemotherapy.

BACKGROUND: Pathological tumour response to preoperative chemotherapy is a key prognostic factor in patients with colorectal liver metastases (CRLM). Multiple CRLM may present different responses, but the prognostic significance of such heterogeneity remains uncertain. The present study aims to elucidate this issue. PATIENTS AND METHODS: We conducted a retrospective analysis of patients undergoing liver resection for multiple CRLM after neoadjuvant chemotherapy. We analysed up to five metastases per patient. Pathological response was assessed for each lesion using tumour regression grade (TRG

Proteomic profiling of brain organoids and extracellular vesicles identifies early Alzheimer's disease biomarkers and drug response heterogeneity.

INTRODUCTION: Alzheimer's disease (AD) exhibits high genetic and clinical heterogeneity that limits therapeutic success. Patient-derived brain organoids and their extracellular vesicles (EVs) provide physiologically relevant models to study disease mechanisms and individualized drug responses. METHODS: We generated the largest brain organoid cohort to date, derived from 30 independent induced pluripotent stem cell (iPSC) lines from AD and control individuals. Comparative proteomic profiling was performed on both organoids and their secreted EVs to capture molecular diversity and treatment eff

Mechanisms of mindfulness-based cognitive therapy in difficult-to-treat depression: moderation and mediation analyses from the RESPOND trial.

BACKGROUND: Mindfulness-based cognitive therapy (MBCT) was developed for relapse prevention in people with remitted depression but is increasingly used for those with difficult-to-treat depression (DTD). A key question regarding this broader application is whether ongoing depressive symptoms constrain therapeutic responsiveness or disrupt MBCT's proposed mechanism, decentering. We explored whether baseline depressive severity moderates clinical outcomes, whether changes in decentering mediate treatment effects, and whether this mediation varies by baseline severity. METHODS: Secondary moderat

Association between anxious depression and clinical outcomes in subjects with treatment-resistant depression: a comparative effectiveness research trial for antidepressant incomplete and non-responders with treatment-resistant depression (ASCERTAIN-TRD).

OBJECTIVE: Anxious depression is a negative prognostic factor linked to worse outcomes in major depressive disorder (MDD). However, its role in treatment-resistant depression (TRD) remains unclear. This secondary analysis of the ASCERTAIN-TRD trial investigates the role of anxious depression as a predictor or moderator of treatment response. METHODS: A Hamilton Depression Rating Scale Anxiety and Somatization Subscale score of at least 7 identified subjects with the anxious depressive subtype. Treatment response was evaluated based on changes in Montgomery-Asberg Depression Rating Scale score

Targeted Sequencing-Based Re-Evaluation of Candidate Immunogenetic Variants for Biologic Treatment Response and Difficult-to-Treat Rheumatoid Arthritis.

Genetic determinants of response to biologic disease-modifying antirheumatic drugs (bDMARDs) in rheumatoid arthritis (RA) remain insufficiently characterized. We developed a targeted sequencing panel of candidate immunogenetic variants identified through a literature review and evaluated associations with response to TNF-α inhibitors, response to IL-6 inhibitors, and difficult-to-treat RA (D2TRA). Whole-blood DNA was sequenced using custom hybridization capture on a DNBSEQ-G400 platform (2 × 150 bp). Reads were aligned to hg38 using BWA-MEM, and variants were called with GATK HaplotypeCaller.

Anxiety Trajectories during Electroconvulsive Therapy and Their Role in Predicting Depression Treatment Response: Towards Personalized Therapy.

INTRODUCTION: Current predictors of electroconvulsive therapy (ECT) effects rely on static measures. The prognostic value of anxiety evolution during ECT remains unestablished, which impedes personalized treatment. This study aims to investigate the dynamic evolution of anxiety during ECT and its predictive role in treatment effects. METHODS: We collected 1,053 data points from 117 patients with depression who were undergoing ECT, measuring both the Hamilton Anxiety Rating Scale (HAMA) and the 17-item Hamilton Depression Rating Scale (HAMD-17) at baseline and at each of the first eight ECT se
