Histological study of the thin replacement membrane of human tympanic membrane perforations.

A histological study was done on the thin, nearly transparent replacement membrane of tympanic membrane perforations. Human tympanic membranes that were rejected for transplantation, were studied by light and electron microscopy. The abrupt reduction in thickness at the margin of the covered perforation, is entirely due to the reduction of the lamina propria. Even in the thinnest parts of the replacement membrane, a lamina propria is present, separated by continuous basement membranes from the epithelium and mucosa, and measuring no more than some 2-3 microns in thickness. This lamina propria

Clinical characteristics of patients showing tympanic membrane pulsation.

BACKGROUND: Visible tympanic membrane (TM) pulsation is an uncommon otoendoscopic finding that may reflect middle ear or intracranial pathology. OBJECTIVE: To characterize clinical features and etiologies of TM pulsation in an outpatient otology setting. METHODS: We retrospectively reviewed 34 consecutive adults (mean age 56 ± 13 years) with TM pulsation identified on routine otoendoscopy (January 2022-September 2024). All patients underwent audiologic testing and temporal bone computed tomography; internal auditory canal magnetic resonance imaging was performed when indicated. Patients with

A transcytotic transport mechanism across the tympanic membrane.

Drug treatments for middle ear diseases are currently delivered systemically, or locally after opening the impermeable tympanic membrane (TM). We previously used bacteriophage display to discover novel peptides that are actively transported across the intact TM, with a variety of transport rates. Peptide structures were analyzed for evidence regarding the mechanism for this unexpected transport, which was then tested by the application of chemical inhibitors. Primary sequences indicated that trans-TM peptides share one of two amino acid motifs. Secondary structures revealed that linear configu

Management of tympanic membrane retractions: a systematic review.

IMPORTANCE: Tympanic membrane retraction (TMR) is a relatively common otological finding. However, no consensus on its management exists. We are looking especially for a treatment strategy in the military population who are unable to attend frequent follow-up visits, and who experience relatively more barotrauma at great heights and depths and easily suffer from otitis externa from less hygienic circumstances. OBJECTIVE: To assess and summarize the available evidence for the effectiveness of surgical interventions and watchful waiting policy in patients with a tympanic membrane retraction. E

Permeation of polyethylene glycols across the tympanic membrane.

BACKGROUND: Integrating pre-exposure prophylaxis (PrEP) delivery in family planning (FP) settings may offer an efficient platform to reach women at elevated risk for HIV. METHODS: We conducted a stepped-wedge cluster randomized trial to integrate systematic screening for HIV risk and PrEP delivery in twelve public FP clinics in Kisumu County, Kenya. Clients were HIV-negative women 15-49 years accessing the participating clinics. Each clinic represented a cluster, and four clinics sequentially transitioned from usual care (control) to the intervention period in a randomized staged order every

Blast-Induced Cholesteatomas After Spontaneous Tympanic Membrane Healing.

OBJECTIVES: To characterize blast-induced cholesteatomas (BIC) in terms of symptoms, presentation, and location within the middle ear cleft (MEC). DESIGN: A search for all English language articles in "MEDLINE" via "PubMed" and "Google Scholar" was conducted. RESULTS: A total of 67 ears with BIC were included. Fifty-eight ears in which the traumatic perforation failed to spontaneously close were excluded, leaving seven case reports (eight patients, nine ears) for statistical analysis. Time between blast exposure to spontaneous tympanic membrane (TM) closure was 16 days to 10 months. Time bet

[Advances of chronic tympanic membrane perforation animal model].

Chronic tympanic membrane perforation in an animal model has been widely used. The ideal model is a lasting and stable perforation without infection. For half a century, several physical modalities have been evolved, such as thermal injury, infolding technique, tympanostomy tube, re-myringotomy, and laser myringotomy. Chemical methods using chemical substance inhibiting growth and repair of cells as well as using gene defect animals are also applied. We have found the success rate is lower when using only one method and it would be better to combine physical and chemical approaches. In this ar

Biomechanical Changes of Tympanic Membrane to Blast Waves.

Eardrum or tympanic membrane (TM) is a multilayer soft tissue membrane located at the end of the ear canal to receive sound pressure and transport the sound into the middle ear and cochlea. Rupture of the TM is one of the most frequent injuries of the ear after blast exposure in military service members. The TM mechanical property changes induced by blast waves also affect progressive hearing loss in veterans. This chapter describes the biomechanical measurements and modeling of blast wave transduction through the ear and the TM mechanical property changes after blast exposure. The human TM ru

Laser-induced tissue remodeling within the tympanic membrane.

The tympanic membrane (TM) separates the outer ear from the tympanic cavity. Repeated pathologies can permanently decrease its tension, inducing conductive hearing loss and adhesive processes up to cholesteatoma. The current main therapy is its surgical reconstruction. Even though lasers have been proposed to tighten atrophic TMs, details of this effect, specifically histological analyses, are missing. We therefore used laser pulses to induce TM collagen remodeling in an animal model to compare the histological and electrophysiological effects of different applied laser intensities before ente

Acute otitis media with spontaneous tympanic membrane perforation.

The principal aim of this review is to present the current knowledge regarding acute otitis media (AOM) with spontaneous tympanic membrane perforation (STMP) and to address the question of whether AOM with STMP is a disease with specific characteristics or a severe case of AOM. PubMed was used to search for all studies published over the past 15 years using the key words "acute otitis media" and "othorrea" or "spontaneous tympanic membrane perforation". More than 250 articles were found, but only those published in English and providing data on aspects related to perforation of infectious orig

Otoscope fogging: examination finding for perforated tympanic membrane.

The author reports a recently recognised physical examination finding, otoscope fogging, for perforated tympanic membrane. Otoscope fogging is defined as condensation forming in the view field of the otoscope while inspecting the ear. In the setting of occult perforation secondary to the inability to visualise the entire tympanic membrane, otoscope fogging may provide the clinician with valuable information since medical management may differ if perforation is present.

Chronic tympanic membrane perforation in an animal model.

OBJECTIVE: The aim of this study was to revalidate and reproduce a chronic tympanic membrane perforation animal model. STUDY DESIGN: Prospective, animal study. METHODS: Eight female chinchillas underwent bilateral thermal myringotomy. The edges of the perforation were folded inward using microflaps. The perforations were followed over time to monitor the course of closure. RESULTS: Two animals were excluded from the study because of ear infection. None of the other tympanic membrane perforations remained open. The closing time varied from 4 to 6 weeks. CONCLUSIONS: Our findings demonstrate

Total tympanic membrane reconstruction: AlloDerm versus temporalis fascia.

BACKGROUND AND OBJECTIVE: Patients who require surgery for chronic otitis media with perforation and cholesteatoma frequently provide no residual tympanic membrane that is usable in grafting procedures. A novel technique of total tympanic membrane reconstruction (TTMR) is described that maximizes perforation closure rate in these situations while minimizing mucosalization, incomplete healing, and anterior blunting. The specific aim of this report is to assess the safety and efficacy of TTMR and to compare the results obtained with AlloDerm compared with temporalis fascia as a grafting material

Tympanic membrane perforation repair with acellular porcine submucosa.

OBJECTIVES: To evaluate the efficacy of acellular porcine small intestine submucosa in the repair of chronic tympanic membrane perforations. Although tympanoplasty with autologous temporalis fascia and cartilage is common practice in the repair of chronic tympanic membrane perforations, these materials are associated with increased operative time and have variable availability and quality in individual patients. Recently, new materials for tympanoplasty have been explored, including acellular human dermis. Small intestine submucosa (Surgisis) is an inexpensive and readily available alternative

Effectiveness of adenoidectomy and laser tympanic membrane fenestration.

OBJECTIVE: Adenoidectomy alone or with tonsillectomy (A+/-T) is an effective surgical intervention in the management of otitis media in children, especially when it is performed in conjunction with insertion of pressure equalization tubes (PETs). Otorrhea and persistent tympanic membrane (TM) perforation are frequent complications. This study evaluates the effectiveness of intermediate duration middle ear ventilation using laser tympanic membrane fenestration (LTMF) without tube insertion and as an adjunct to adenoidectomy in resolving middle ear disease within the first 90 days after surgery.
