Case Report: Ocular Tilt Reaction with Internuclear Ophthalmoplegia and Multiple Cranial Nerve Palsies.

SIGNIFICANCE: Ocular tilt reaction (OTR) is an abnormal eye-head postural reaction that consists of skew deviation, head tilt, and bilateral ocular torsion. Understanding of the pathway of the vestibulo-ocular reflex (VOR) is essential because this will help to localize the pathology. PURPOSE: The aim of this study was to report a case of OTR with contralateral internuclear ophthalmoplegia (INO) and fifth and seventh cranial nerve palsies. CASE REPORT: A 51-year-old gentleman with underlying diabetes mellitus presented with sudden onset of diplopia for 3 days. On examination, his visual acui

Is the root entry/exit zone important in microvascular compression syndromes?

OBJECTIVE: Microvascular compression syndromes such as trigeminal neuralgia, hemifacial spasm, and disabling positional vertigo involve an artery or vein compressing a cranial nerve. A cranial nerve is composed of a central nervous system (CNS) segment and a peripheral nervous system (PNS) segment separated by the root entry/exit zone (REZ). Although vascular compression can occur at any point along the cranial nerve, it has been generally assumed that only vascular contact at the REZ of the affected cranial nerve can cause symptoms. On the basis of personal surgical experience, we propose tha

Injuries to the auditory nerve: a study in monkeys.

The effect of traction injury to the eighth nerve on the compound action potentials (CAPs) recorded from the exposed nerve in response to tonebursts and click sounds was compared to responses elicited by continuous tones and noise that were amplitude-modulated with pseudorandom noise. Responses to the continuous sounds were presented in the form of cross-correlograms between the averaged response, with the averager locked to the periodicity of the noise and one period of the noise. While the wave form of these cross-correlograms was similar to the wave form of the response to the same, but unm

Acute acoustic nerve palsy associated with vincristine therapy.

A 73-year-old woman with non-Hodgkin's lymphoma had two episodes of severe, bilateral, sensori-neural hearing loss after vincristine therapy. Her hearing gradually, then completely returned 2-3 months after the vincristine therapy was discontinued. Bilateral ear infections, central nervous system lymphoma and infection, or other drug-induced neuropathies were excluded as possible etiologies for the deafness. Bilateral acoustic (VIII) nerve palsy in this patient was most likely a manifestation of vincristine neurotoxicity.

Hemifacial Microsomia in a Cat.

A 7-month-old domestic medium hair cat presented with facial asymmetry affecting the bony and soft tissue structures of the right side of the head including the maxilla, nose, eye and pinna of the ear. Additionally, neurological dysfunction of the facial and vestibulocochlear nerves on the affected side was present. A congenital malformation affecting the first and second embryologic pharyngeal arches was suspected. This is the first case of hemifacial microsomia of likely congenital origin reported in a cat.

A Small Glossopharyngeal Schwannoma Presenting with Intractable Vomiting: Case Presentation and Literature Review.

BACKGROUND: Glossopharyngeal schwannomas are rare tumors. Clinical and radiologic presentation closely resembles those of vestibular schwannomas. Their clinical presentation varies from vestibulocochlear dysfunction to glossopharyngeal and vagal nerve dysfunction. CASE DESCRIPTION: We report a case of a small glossopharyngeal schwannoma presenting with intractable recurrent sudden episodes of vomiting that subsided after tumor resection. To our knowledge, this is the only case in the literature of glossopharyngeal schwannoma presenting with nerve irritation in the form of intractable emesis.

Hypertrophic Pachymeningitis Possibly Secondary to Otitis Media.

This study describes a 46-year-old female patient who initially presented with hearing impairment and subsequently developed a headache as the predominant symptom, accompanied by the involvement of multiple cranial nerves, including the optic, vestibulocochlear, and trigeminal nerves. Cerebrospinal fluid pressure progressively increased, and imaging demonstrated progression from unilateral to bilateral involvement of the tentorium cerebelli. Clinically, the patient's hypertrophic pachymeningitis was considered associated with otitis media. After receiving high-dose corticosteroid pulse therapy

Imaging of Vascular Compression Syndromes.

Trigeminal neuralgia, hemifacial spasm, vestibulocochlear neuralgia and glossopharyngeal neuralgia represent the most common neurovascular compression syndromes. Repeated vascular pulsations at the vulnerable transitional zone of the individual cranial nerves lead to focal axonal injury and demyelination. High-resolution 3-D T2-weighted MR imaging is essential in detecting and mapping neurovascular compression for directed therapy. Knowledge of the specific nerve root exit, the transitional zones, and the adjacent vasculature is critical in proper management. Treatment options for these neurov

Early diagnosis and treatment of Ramsay Hunt syndrome: the role of magnetic resonance imaging.

We present the case of a 47-year-old woman with left otalgia, rotatory vertigo, sensorineural hearing loss and acute facial nerve palsy. An enhanced magnetic resonance imaging (MRI) scan showed discrete enhancement of the facial and vestibulocochlear nerves in the left internal auditory canal as well as of the labyrinth. This appearance was compatible with that in Ramsay Hunt syndrome and acyclovir was started prior to the appearance of any vesicular eruption. The diagnosis was subsequently confirmed serologically. She regained full facial function but the sensorineural hearing loss persisted.

Idiopathic AA amyloidosis manifested by autonomic neuropathy, vestibulocochleopathy, and lattice corneal dystrophy.

A 69-year-old Japanese woman with non-familial amyloidosis had polyneuropathy and profound autonomic neuropathy, and kappa chain monoclonal gammopathy. Immunohistopathological examination showed protein AA and protein AP in the amyloid deposits. She showed involvement of the vestibulocochlear nerve and lattice dystrophy of the cornea. Vestibulocochleopathy and corneal lattice dystrophy have been reported in familial amyloid polyneuropathy type IV, Finnish type, but never in non-familial amyloidosis.

Problems in interpreting abnormal auditory brainstem responses in comatose patients.

Auditory brainstem responses (ABRs) provide a way to evaluate central nervous system function in young, confused, uncooperative or comatose patients. Auditory brainstem responses represent activity in or around the vestibulocochlear (VIII) cranial nerve, cochlear nucleus, superior olivary complex, lateral lemniscus, and inferior colliculus. Many factors affect recording of the auditory brainstem response. These factors include technical factors, chronic alcoholism, demyelinating diseases, ototoxic drugs, barbiturates, hearing loss, otitis media, and hypothermia.

Vestibular syndrome associated with temporohyoid joint fusion and temporal bone fracture in three horses.

Acute onset of vestibulocochlear and facial nerve dysfunction due to a stress fracture of the petrous part of the temporal bone was diagnosed in 3 horses. The fracture was secondary to chronic inflammatory changes in the petrous part of the temporal bone and the proximal stylohyoid bone, with fusion of the temporohyoid joint. Bacterial meningoencephalitis was a complicating factor. Treatment resulted in reduction of severity of clinical signs in 2 of the 3 cases, but residual compensated vestibular deficits persisted.

Tumors of the nervous system and hearing loss: Beyond vestibular schwannomas.

Hearing loss is a common side effect of many tumor treatments. However, hearing loss can also occur as a direct result of certain tumors of the nervous system, the most common of which are the vestibular schwannomas (VS). These tumors arise from Schwann cells of the vestibulocochlear nerve and their main cause is the loss of function of NF2, with 95 % of cases being sporadic and 5 % being part of the rare neurofibromatosis type 2 (NF2)-related Schwannomatosis. Genetic variations in NF2 do not fully explain the clinical heterogeneity of VS, and interactions between Schwann cells and their micro
