Sonographic detection of radiographically occult fractures in pediatric ankle and wrist injuries.

BACKGROUND: In some pediatric ankle and wrist injuries, the clinical picture is suggestive of a fracture despite negative standard radiographs. OBJECTIVE: The goal of this prospective study was to determine the effectiveness of high-resolution ultrasound (US) in differentiating radiographically occult fractures from sprains. METHODS: During a period of 3 years children aged 2 to 16 years, who sustained an acute ankle and wrist injury suggestive of being a fracture on clinical examination, but with negative radiograph, were referred for high-resolution US. Follow-up radiographs were obtained

Suppression of immobilisation device on wrist radiography to improve fracture visualisation.

OBJECTIVES: This study validates the use of CycleGAN-generated wrist radiographs with digitally removed splints, specifically assessing their impact on fracture visualisation. MATERIALS AND METHODS: We retrospectively collected wrist radiographs from 1748 patients who had imaging before and after splint application at a single institution. The dataset was divided into training (1696 patients, 5353 images) and testing sets (52 patients, 965 images). A CycleGAN-based model was trained to generate splint-free wrist radiographs (generated "splint-less") from the original "splint" images. A pre-tr

Arthroscopic Management of an Extremely Rare Wrist Fracture-Dislocation Pattern.

BACKGROUND: Axial dislocation involves the displacement of carpal bones along the longitudinal axis of the wrist joint. There are three major groups: (1) axial ulnar, (2) axial radial, and (3) combined. On the other hand, radiocarpal dislocation entails disruption of the articulation between the radius and carpal bones. There are two groups: (group 1) it is either a pure radiocarpal dislocation or a minor avulsion of the radial styloid and (group 2) a fracture-dislocations characterized by a significant radial styloid fragment. CASE DESCRIPTION: The patient, a 66-year-old female involved in a

Wrist angle measurements in predicting the presence of a displaced scaphoid fracture.

PURPOSE: Early diagnosis and treatment of scaphoid fractures is critical to achieve union and prevent wrist degenerative arthritis. Plain radiographic measurements can guide the early approach to suspected fractures. Specifically, the ability to predict fracture or fracture stability from plain radiographs may allow the traumatologist to expand initial referral to three-dimensional imaging. We evaluated the ability of four measured angles to predict presence of a scaphoid fracture and stability. METHODS: Fifty patients with a scaphoid fracture and 50 patients without fracture were evaluated f

Patients with wrist fractures are less likely to be evaluated and managed for osteoporosis.

BACKGROUND: Although osteoporosis is being evaluated and treated increasingly in elderly patients with fragility fractures, some studies have suggested that physicians may be missing important opportunities, especially in patients with nonvertebral fractures. The purpose of the present study was to determine whether specialists responsible for treating fractures at various locations have different propensities for evaluating and treating osteoporosis after a fracture in female patients over the age of fifty years. METHODS: A retrospective nationwide cohort study was performed with use of data

Concurrent fractures of the hip and wrist: a matched analysis of elderly patients.

INTRODUCTION: Osteoporotic fractures involving the hip and wrist are common in the elderly. The incidence of coincident hip and wrist fractures in the same patient is small but may represent a unique subpopulation of elderly with osteoporotic fragility fractures and little information has been published about these patients. MATERIALS AND METHODS: We performed a retrospective review studying a series of 33 elderly patients who were admitted with concurrent hip and wrist fractures and matched them with patients of similar age, race, gender and co-morbidities with isolated hip fractures analysi

[Perilunate dislocations and fracture-dislocations of the wrist, a review of 14 cases].

OBJECTIVES: We report a retrospective series of 14 dislocations or perilunate fracture-dislocations. The results of our series are compared with the data of the literature and we discuss epidemiology, types of lesions, surgical treatment, complications and prognosis of this pathology. METHODS: The series included seven pure dislocations and seven fracture-dislocations including three trans-scapho-lunate forms (including one Fenton's syndrome). The displacement of all these lesions was posterior. The mean age was 35 years. Sixty-four percent were manual workers. All 14 patients had undergone s

Unusual combined scaphoid and lunate fracture of the wrist: a case report.

All perilunate fracture-dislocations combine ligament ruptures, bone avulsions, and fractures in a variety of clinical forms. The most frequent is the dorsal trans-scaphoid perilunate dislocation. In rare cases, however, these dislocations also have been associated with capitate fractures, triquetral fractures, or lunate fracture. We report a combined scaphoid and lunate fracture of the wrist that was not associated with perilunate dislocation.

Ruptures of flexor tendons at the wrist as a complication of fracture of the distal radius.

We report two cases of rupture of flexor tendons after fracture of the distal radius. The first case was a rupture of the flexor digitorum profundus and superficialis tendon to the index finger that happened 20 years after the fracture. The second was a rupture of the flexor pollicis longus tendon that occurred two years after, and the flexor profundus tendon to the index finger that occurred four years after the fracture. In the first case, the ruptures were caused by the bony protuberance of the radius after long interval without interference of the ulnar head.

Ultrasound attenuation of the calcaneus in normal subjects and in patients with wrist fracture.

We determined broadband ultrasound attenuation (BUA) of the calcaneus, and bone mineral density (BMD) of the spine, proximal femur and radius in 137 healthy subjects (79 women and 58 men) and in 56 women with Colles' fracture. The repeated measurements on 9 healthy subjects indicated a short-term reproducibility (coefficient of variation) of 4 percent for BUA. There was a small (7 percent) but significant difference in BUA between normal men and women. The age-dependence in normal subjects was weak. When all the study subjects were pooled, modest correlations between BUA of the calcaneus and B

Cineradiographic study of wrist motion after fracture of the distal radius.

Residual dorsal tilt is one of the main deformative factors in the malunion of fractures of the distal radius. We investigated the effect of such tilt on the range of wrist motion and on carpal alignment in a cineradiographic study of wrist motion in 30 patients with extra-articular Colles' fracture. With increased dorsal tilt, the range of wrist motion became more restricted and abnormalities of carpal alignment during wrist motion became apparent. When the dorsal tilt was less than 10 degrees, the effects on the range of motion and on the dynamic carpal alignment were small.

Complications of smooth pin fixation of fractures and dislocations in the hand and wrist.

A retrospective review of patients treated with internal fixation of fractures or dislocations of the hand or wrist over a four-year period was undertaken to determine the complication rates of pin fixation for stabilization of these injuries. One hundred thirty-seven patients who received 422 pins were studied. All pins were unthreaded, measured 0.035-0.069 inches (0.9-1.8 mm) in diameter, were placed with a power drill, and were left protruding through the skin. The mean time that pins were left in place was 6.5 weeks, (range, two days to 24 weeks). Minimum follow-up time was 43 days after p

Effects of distal radius fracture malunion on wrist joint mechanics.

An experimental model using a static positioning frame, pressure-sensitive film (Fuji), and a microcomputer-based videodigitizing system was used to measure contact areas and pressures in the wrist. Contact areas and pressures were compared in a group of wrists between the normal state and with simulated distal radius fracture malunions of varying degrees. In simulated malunions, radial shortening to any degree slightly increased the total contact area in the lunate fossa, and was significant at 2 mm of shortening. By angulating the distal radius more than 20 degrees either palmar or dorsal, t
