BACKGROUND: Zygomatic arch (ZA) fractures are a common facial fracture, and reduction is typically performed blind via a Gillies or Keen approach. Postoperative confirmation of reduction thus requires advanced imaging, which may not be readily available in all settings. Thus, there exists a need for an effective, low-cost imaging paradigm to employ in these clinical scenarios. Herein, we introduce the ultrasonic arch reduction (USA Reduction) for ZA fractures.
METHODS: All consecutive patients with ZA fractures undergoing a USA Reduction at a single public hospital were reviewed. Patients wer

The aim of this prospective follow-up study was to clarify the incidence and characteristics of dysfunction of the temporomandibular joint (TMJ) in patients treated surgically for fractures of the zygomatic complex. Patients were evaluated on presentation and six months after injury to assess the function of the masticatory system using the Helkimo index, which incorporates two complementary subindices: the subjective symptomatic (anamnestic) index (Ai) and the objective clinical dysfunction index (Di). Forty-five patients (12 women and 33 men, mean (range) age 44 (21-83)years) completed the s

The infraorbital nerve (ION) is often involved in trauma to the zygomatic complex (ZC), resulting in sensory disturbance of the area innervated by it. The purpose of the study was to compare the incidence of persistent sensory disturbance after recovery from isolated simple fractures of the ZC, with four treatment methods: 1) closed reduction via subcutaneous approach without fixation; 2) open reduction via subcutaneous approach without fixation; 3) open reduction via oroantral approach and support of the complex by an intraantral Foley catheter; and 4) open reduction via Gillies approach with

A randomized controlled trial evaluated the effect of intraoperative computed tomography (CT) on zygomaticomaxillary complex (ZMC) fractures. Participants, aged 16 and older, were recruited from the Royal Brisbane and Women's Hospital and included if they required surgical management for their fractures. The study compared outcomes between operations using intraoperative CT and those that did not, hypothesizing that the CT group would have better reductions and lower revision surgery rates. The primary outcome variable was quality of radiographic reduction. The secondary outcome variables incl

Displacement of an intact mandibular coronoid process lateral to the zygomaticomaxillary complex (ZMC) is a rare complication of ZMC fractures. We report a case of a woman in her early 30s with malar flattening and inability to achieve full mouth closure following a fall from an electric scooter. Her CT scan revealed a comminuted right ZMC fracture with the ipsilateral intact coronoid process displaced laterally to the ZMC. This rare injury pattern produced mechanical restriction of mandibular movement in both opening and closure. Urgent operative management of the ZMC fracture allowed for spo

The zygomatic bone is one of the most prominent bones in the facial region. It forms the most anterolateral projection on each side of the middle face and is articulated with the maxilla, frontal, and temporal bones. Isolated zygomatic arch fractures can occur when a direct force is applied to the zygoma. A variety of intraoral and extraoral techniques have been used as closed reduction techniques for isolated fractures of the zygomatic arch. In this case report, we aim to present our approach for the treatment of a 40-year-old patient with an isolated right zygomatic arch fracture. We used th

BACKGROUND: The deep temporal fascia (DTF) is known to separate into two layers that descend to attach to the zygomatic arch. When surgeons reduce an isolated fracture of the zygomatic arch through a temporal approach, the temporal incision site needs to be superior to the split line of the DTF.
MATERIALS AND METHODS: Sixty-seven hemifacial cadavers were investigated after removing the skin, subcutaneous tissue, and superficial temporal fascia. The superficial layer of the DTF was exposed. We cut the superficial layer along the line along, which it adhered to the deep layer inseparably. The h

Midfacial fractures rank first concerning maxillofacial traumatology. Collisions of two heads or head to object are the main causes for these fractures. An investigation based on a transient simulation using the finite element method was performed. A biomechanical head model was created and tested. A transient collision of two heads was simulated. The results were compared to a typical real patient case. This comparison revealed an identical fracture pattern, which can be interpreted as a clinical match of the simulation. The results of this study show the validity of biomechanical investigati

Collapse of the zygomatic arch following trauma results in inadequate anteroposterior projection of the zygomatic body and an increase in facial width. Accurate assessment of the position of the zygomatic arch in relation to the cranial base posteriorly and the midface anteriorly is the key to the acute repair of complex midfacial fractures and the secondary reconstruction of posttraumatic deformities of the orbitozygomaticomaxillary complex. Loss of projection of the zygomatic arch may occur with injuries confined to the orbitozygomaticomaxillary region or in association with complex midfacia

BACKGROUND: Zygomatico-maxillary complex (ZMC) fractures are prevalent facial injuries with significant functional and aesthetic implications. Computer-assisted surgery (CAS) offers precise surgical planning and outcome evaluation. The study aimed to evaluate the application of CAS in the analysis of ZMC fracture outcomes and to propose a reproducible workflow for surgical outcome assessment using cephalometric landmarks.
METHODS: A retrospective cohort study was conducted on 16 patients treated for unilateral ZMC fractures at the Maxillofacial Surgery Unit of Siena University Hospital (2017-

Tapia syndrome is known as a rare complication of airway manipulation, which can occur after any type of surgery under endotracheal general anesthesia. This syndrome is characterized by neurologic deficits involving the hypoglossal (XII) and recurrent laryngeal branch of the vagal nerve (X) that result in the tongue muscle's unilateral paralysis associated with the vocal cord's unilateral palsy. This article describes the first case of Tapia syndrome, which occurred after repair of a fractured zygomatic complex. In this article, we discuss the diagnosis, possible causes, clinical manifestation

A rare case of fracture of the coronoid process, sphenoid bone, zygoma, and zygomatic arch caused by a firearm is described. A 25-year-old man was hit in the face with a bullet, resulting in restricted mouth opening, difficulty chewing, and pain when opening the mouth. The clinical examination revealed a perforating wound in the right parotid region. A computed tomographic scan revealed a comminuted fracture of the left coronoid process with the bullet stopping in the intact left coronoid process. Treatment was bilateral coronoidectomy associated with speech therapy and was successful. Details

The ultimate goal in treating zygomatic complex fracture is to obtain an accurate, stable reduction while minimizing external scars and functional deformity. The present authors present our experiences with a single transconjunctival incision and two-point (inferior orbital rim and frontozygomatic suture) fixation in 53 patients with zygomatic complex fracture which were not comminuted. All patients had transconjunctival approaches with lateral canthal extensions, and six out of 53 patients also had an additional small (about less than 2 cm) gingivobuccal incision to achieve an accurate reduct

A case of osteomyelitis of the zygomatic arch with complete resorption has been presented. The cause of osteomyelitis of the facial bones was discussed. Infection of the soft tissue after intraoral elevation of fractured zygomas does occur, but rarely leads to osteomyelitis and subsequent bony resorption of the underlying bone. Antibiotics have reduced the incidence of osteomyelitic infections in the past 20 years; however, a vigorous regimen of preoperative and postoperative attention to aseptic technique, proper antibiotics, and close follow-up is required to control these problems.
