Fostering Ambulation for a Preschool Child with Rett Syndrome: A Case Report.

UNLABELLED: Children with Rett Syndrome (RS) have neuromotor impairments that impact their mobility. Poor hand function among children with RS limits the selection of an assistive device for ambulation. PURPOSE: The purpose of this case report is to describe the process of selecting an assistive device for a child with RS to promote ambulation. METHOD: This single subject case reports on a 5-year-old girl with RS at a suburban mid-western early childhood special education setting. RESULTS: The child in this case was able to walk the farthest distances with a metal toy shopping cart and then

Orthostatic intolerance ambulation in patients using patient controlled analgesia.

BACKGROUND: Opioid analgesics are widely used to reduce postoperative pain and to enhance post-operative recovery. However, orthostatic intolerance (OI) induced by opioid containing intravenous patient controlled analgesia (IPCA) may hinder postoperative recovery. This study investigated factors that affect OI in patients receiving IPCA for postoperative pain control. METHODS: OI was instantly evaluated at the time of first ambulation in 175 patients taking opioid containing IPCA after open and laparoscopic subtotal gastrectomies. Patients were classified as having OI if they experienced dizz

The relationship between manual ability and ambulation in adolescents with cerebral palsy.

This study examined the relationship between gross motor function and manual ability in 120 adolescents with cerebral palsy (CP) (15.2, SD 2.1 years, 59.8% male). Adolescents were evaluated using the Gross Motor Function Classification System (GMFCS) and the Manual Ability Classification System (MACS). A neurologist classified CP subtype. Most adolescents were ambulatory with or without utilization of aids (GMFCS level I: 35.0%, II: 30.0%, III: 5.8%, IV: 10.8%, and V: 18.3%). MACS levels were I: 34.2%, II: 25.8%, III: 16.7%, IV: 7.5%, and V: 15.8%. Correlations between GMFCS and MACS were stro

Prospective study of ambulation after open and laparoscopic colorectal resection.

PURPOSE: Open and laparoscopic surgical approaches each have specific advantages. This study compares ambulation, hospital length of stay (LOS), and incision length after open and laparoscopic colorectal resection. METHODS: All consecutive patients undergoing colorectal resection over a 2 year period ending August 2002 were followed prospectively. Ambulation, LOS, and incision length were recorded. Hybrid low anterior resection (LAR) patients had laparoscopic splenic flexure takedown, vessel ligation, and proximal rectal mobilization followed by planned inferior laparotomy to complete the cas

Early ambulation after acute deep vein thrombosis: is it safe?

The number of thrombotic events in children, although significantly less than that in adults, is increasing as a result of therapeutic advances in primary illnesses that were previously fatal. When a patient, adult or pediatric, develops a deep vein thrombosis and anticoagulation therapy is initiated, many health professionals ask, "When should this patient have physical therapy and/or ambulate?" Fear of causing a pulmonary embolism with increased activity drives this question. Often, an order for bed rest is prescribed based more on tradition than on evidence-based medicine. A review of the l

Ambulation and compression after deep vein thrombosis: dispelling myths.

The traditional dogma of putting mobile patients with acute deep vein thrombosis into bed for several days has been challenged by some studies that showed a better clinical outcome with walking exercises under good compression. Repeated lung scans did not show an increased risk of new pulmonary embolism. There was a faster and more intense reduction of pain and swelling and a clear quality-of-life benefit. Immediate ambulation with compression reduces the propagation of thrombi and has a positive impact regarding development of postthrombotic syndrome. Patients selected for home therapy should

Enhancement of the ambulation-increasing effect of opioid analgesics by ethanol in mice.

The interaction between opioid analgesics (morphine and buprenorphine) and central depressants (ethanol, pentobarbital and diazepam) was investigated by means of ambulatory activity in mice. The ambulation-increasing effect of both morphine (10 mg/kg, s.c.) and buprenorphine (1 mg/kg, s.c.) was enhanced by the combined administration of ethanol (0.8-3.2 g/kg, p.o.) in a dose-dependent manner. Naloxone (0.1 mg/kg, s.c.) was effective for reducing the enhanced ambulatory activity. The pretreatment with Ca-cyanamide (5 mg/kg, p.o., 30 min before) reduced the enhancement of the ambulation-increasi

Foregoing prehospital care: should ambulance staff always resuscitate?

Approximately 400,000 people die outside US hospitals or chronic care facilities each year. While there has been some recent movement towards initiating procedures for prehospital Do Not Resuscitate (DNR) orders, the most common situation in the US is that emergency medical systems (EMS) personnel are not authorized to pronounce patients dead, but are required to attempt resuscitation with all of the modalities at their disposal in virtually all patients. It is unfair and probably unrealistic for EMS personnel to have to make a determination of the validity of a non-standard prehospital DNR or

Ambulation in the adolescent with spina bifida. II. Oxygen cost of mobility.

This study was designed to determine the energy cost (measured as oxygen use) of walking and wheelchair propulsion in children aged 10 to 15 with myelomeningocele of thoracic to sacral levels, and to determine whether energy cost of mobility could be estimated from clinical measures. Oxygen consumption (measured with open circuit spirometry) and heart rate were measured during treadmill walking by 21 children, wheelchair use by eight children, and, for five children, in both modes. Speeds ranged from 27 to 134 m/min, with slopes up to 15%. Energy consumption for walking was linearly related to

Energy cost of three-point crutch ambulation in fracture patients.

The energy consumption associated with unilateral nonweight-bearing (three-point) crutch ambulation was evaluated in a group of 25 newly injured fracture patients immobilized in a long or short leg cast. After 5 min of crutch ambulation, the rate of oxygen uptake was 32% greater than the value for normal walking (averaging 15.7 ml/kg-min); the heart rate was 53% greater than normal (averaging 153 beats/min); and the respiratory quotient was markedly elevated (averaging 1.03). In those subjects able to ambulate for 10 min, these values averaged 17.6 ml/kg-min and 173 beats/min, approaching the

Effect of short leg casting on ambulation in children with cerebral palsy.

The purpose of this study was to compare and analyze changes in ambulation between two groups of children with cerebral palsy, with and without short leg casting. Sixteen children with spastic cerebral palsy, aged 10 to 108 months, were assigned randomly to either a short leg casted group or an uncasted group. All children were tested by me, before and after 10 weeks of neurodevelopmental treatment, by recording an ink print ambulation pattern on a length of paper fastened to the floor. Measurements of stride length, stride width, foot angle, and footprint clarity were taken to quantify the am

Bacterial Contaminants in Ambulances from a Tertiary Care Hospital as Potential Threats to Patients and Medical Staff in Al-Qassim Region, Saudi Arabia-Effect of Decontamination.

Bacterial contaminants in ambulances could have a major impact on morbidities, mortalities, and healthcare resources, especially if these bacteria are antimicrobial-resistant. As far as we know, this is the first study in Al-Qassim region to evaluate the prevalence of bacterial contaminants in swab samples obtained from ambulances from Alqwarah General Hospital, Al-Qassim region, Saudi Arabia as an indicator for evaluation of the implemented infection control measures, and screen the antibiotics profiles of the isolates against the most regularly used antimicrobials. In total, 204 samples were
