Cardiac implantable electronic device infections: Emerging paradigms in precision prevention and personalized management.

Cardiac implantable electronic device (CIED) infections remain a major source of morbidity, mortality, and healthcare expenditure despite continuing advances in device technology. Increasing procedural complexity, repeat interventions, and an aging population with multiple comorbidities have shifted the focus from treatment alone toward comprehensive infection prevention. Biofilm formation is now recognized as the central pathogenic mechanism underlying CIED infection, explaining both the limited efficacy of antimicrobial therapy in the presence of retained hardware and the need for complete s

Surgical site infections after cesarean delivery.

Surgical site infections represent a persistent and clinically relevant complication of cesarean delivery, contributing substantially to postpartum morbidity, hospital readmission, and pregnancy-related mortality. As cesarean delivery has escalated to the most common major operation worldwide-now comprising 21% of global births and 32% in the United States-the burden of postcesarean surgical site infections, reported in 3% to 15% of cases, remains unacceptably high despite advances in perioperative antisepsis, antibiotic prophylaxis, and prevention bundles. This review provides an updated comp

A Lip Fixation Device Facilitates Labial Salivary Gland Biopsy for Sjögren's Disease.

The protocol aims to standardize and simplify labial salivary gland biopsy (LSGB) in suspected Sjögren's disease (SjD) using a lip fixation device, enabling safe, efficient, and operator-independent procedures with reduced technical difficulty, especially for junior rheumatologists. We developed a step-by-step protocol encompassing preoperative assessment, antisepsis, local anesthesia, device-assisted lip stabilization, precise vertical incision (5-7 mm), blunt dissection, gland harvesting (minimum 4-6 glands), and standardized closure. The device consists of two clamp arms, an oval pressure r

[Historical notes on the management of infection during the development of surgery].

Surgery and medicine have not evolved in parallel. There have been discrepancies, bellicosity, contempt and even separate university studies during a long time. The Saint Cosme Brotherhood, founded to supervise the professional practice of barbers (short robe surgeon-barbers) in France in 1260, was opposed by the Faculty of Medicine in Paris. The conflicting interests of the university, Brotherhood and Barbers, that persisted until the 18 th century, impaired the progress of surgery. In the first half of the 19 th century, the advance of surgery continued facing pain, hemorrhage and infection.

Use of antimicrobials to prevent postoperative infection in patients with cataracts.

Despite the high frequency of cataract surgery, there are still no definitive studies on antibiotic prophylaxis against endophthalmitis after cataract surgery. Most surgeons follow an empiric approach. The results of several recent studies shed additional light on this complex topic. There is better knowledge of the usual source of the infection and the specific organisms incriminated. This review presents some principles of prophylactic treatment and selected literature that addresses the effectiveness of antisepsis, topical preoperative antibiotics, intracameral antibiotics, subconjunctival

A case report of anaphylaxis to chlorhexidine during urinary catheterisation.

Chlorhexidine gluconate is a chemical antiseptic that is effective against Gram positive and negative bacteria and on certain viruses and fungi. A bacteriocidal and bacteriostatic agent, this cationic drug is absorbed on to negatively charged cell surfaces of organisms, disrupting the cell membrane, which results in increased permeability. Its use is mainly topical as a surgical hand antisepsis, site preparation/cleansing and for pre-genitourinary procedures such as urethral catheterisation. Like any drug, caution should be employed with its use as hypersensitivity reactions are being document

Chlorhexidine--pharmacology and clinical applications.

Chlorhexidine is a widely used skin antisepsis preparation and is an ingredient in toothpaste and mouthwash. It is an especially effective antiseptic when combined with alcohol. Its antimicrobial effects persist because it is binds strongly to proteins in the skin and mucosa, making it an effective antiseptic ingredient for handwashing, skin preparation for surgery and the placement of intravascular access. Catheters impregnated with chlorhexidine and antimicrobial agents can reduce the incidence of catheter-related bloodstream infections. Contact dermatitis related to chlorhexidine is not com

The Utility of Perioperative Products for the Prevention of Surgical Site Infections in Total Knee Arthroplasty and Lower Extremity Arthroplasty: A Systematic Review.

Surgical site infections (SSIs) are among the most prevalent and devastating complications following lower extremity total joint arthroplasty (TJA). Strategies to reduce the rates can be divided into preoperative, perioperatives, and postoperative measures. A multicenter trial is underway to evaluate the efficacy of implementing a bundled care program for SSI prevention in lower extremity TJA including: (1) nasal decolonization; (2) surgical skin antisepsis; (3) antimicrobial incise draping; (4) temperature management; and (5) negative-pressure wound therapy for selected high-risk patients. Lack of effect on blood alcohol level of swabbing venepuncture sites with 70% isopropyl alcohol.

OBJECTIVE: It is standard practice to clean the skin using a non-alcohol-containing swab before forensic blood alcohol sampling, because of the belief that the use of an alcohol-containing swab will contaminate the sample. The present study aimed to determine whether cleaning the skin with 70% isopropyl alcohol swabs, before venepuncture, alters measured blood alcohol level (BAL). METHODS: Volunteers aged >18 years had paired venous blood tests, which were drawn within 2 min of each other. One arm was swabbed with a 70% isopropyl alcohol swab and allowed to dry before venepuncture. The other

[Pandemic influenza: rapid sanitary-hygienic measures for initial containment of diffusion].

Viral respiratory diseases may be characterized by rapid diffusion in population, that often cause epidemic outbreaks or pandemic. Besides, typical high mutations of involved virus (almost always influenza virus) can reduce the validity of the up to date available vaccine. The achievement of new vaccines can require prolonged period. In addition, the availability and efficacy of antiviral drugs against new viruses should be evaluated before their uses. Influenza virus replication occurs in the epithelial cells of the respiratory system, and viruses, present in contaminated secretions, spread m

Photosensitised versus conventional infection control: the local fight continues.

The worsening problem of antimicrobial drug resistance requires a nuanced approach. Since the conventional drug pipeline is unlikely to be sufficient to avoid massive increases in mortality by the mid-twenty-first century, other methods of antisepsis will be required. These might be used either in place of (allowing conservation) or together with conventional agents. Of such approaches, locally applied protocols involving photo-antimicrobials suggest themselves, particularly as early intervention, e.g. in bacterial tonsillitis, would be curative without recourse to conventional drugs, and woul

Oral and maxillofacial myiasis associated with epidermoid carcinoma: a case report.

Myiasis is a term derived from the Greek word "myia", meaning invasion of vital tissue of humans or other mammals by fly larvae. The deposited eggs develop into larvae, which penetrate deep structures causing adjacent tissue destruction. It is an uncommon clinical condition, being more frequent in underdeveloped countries and hot climate regions, and is associated with poor hygiene, suppurative oral lesions, alcoholism and senility. Its diagnosis is made basically by the presence of larvae. This paper reports a case of oral and maxillofacial myiasis involving 273 larvae in a patient with epide

Reduced Bacterial Burden of the Skin Surrounding the Shoulder Joint Following Topical Protocatechuic Acid Application: Results of a Pilot Study.

BACKGROUND: Reducing the rising health-care burden associated with shoulder surgical site infection (SSI) is of paramount importance. The purpose of this study was to investigate the antimicrobial efficacy of protocatechuic acid (PCA) as a topical reagent for surgical skin antisepsis surrounding the shoulder joint. METHODS: This was a 2-phase skin-disinfection trial involving the human shoulder. The shoulders of healthy volunteers were randomized to topical treatment with PCA (a 10% concentration of PCA in Phase I [11 subjects] and a 17% concentration in Phase II [12 subjects]), with a contro

Post-surgical complications of median nerve release at the wrist level.

Carpal tunnel syndrome is a set of signs and symptoms caused by compression of the median nerve as it travels through the wrist at the carpal tunnel. The diagnosis is clinical and based on the presence of characteristic signs and symptoms. Proper nonsurgical treatment can stop the progression of this disorder and prevent the development of permanent disability. Surgical treatment may be indicated to patients with complications rated as moderate to severe. Although the surgery is relatively simple, basic antisepsis care before, during, and after the procedure, and guidance of patients for the m
