Coronary arterial dominance associated with congenital bicuspid aortic valve.

Gross examination was done of 186 hearts, principally from adults, for the nature of the aortic valve and the source of the posterior descending coronary artery (origin from the right coronary artery indicated as right dominance, origin from the left circumflex artery as left dominance). Exclusive of cases with coarctation of the aorta, the overall incidence of left coronary dominance in cases with congenital bicuspid aortic valves was 24.4%, while in cases with tricuspid aortic valves the incidence was 9.5%. There appears to be a greater tendency for left coronary arterial dominance in coarct

Exercise in Patients With Bicuspid Aortic Valve and Aortic Dilation.

BACKGROUND: Vigorous exercise is discouraged among individuals with aortic dilation (≥4.0 cm) or aneurysm (≥4.5 cm). Although bicuspid aortic valve (BAV) is associated with aortic dilation, the absolute incidence of acute aortic syndrome is low (3.1/10,000 patient-years). We examined the association of exercise with aortic growth in BAV patients with aortic dilation. METHODS: Data were collected from a prospectively enrolled cohort at Northwestern University including BAV patients with aortic dilation on computed tomography/magnetic resonance imaging and without prior aortic intervention. Progression of aortic stenosis in patients with bicuspid aortic valve.

BACKGROUND: Bicuspid aortic valve is the most common congenital heart defect and predisposes patients to developing aortic stenosis more frequently and at a younger age than the general population. However, the influence of bicuspid aortic valve on the rate of progression of aortic stenosis remains unclear. METHODS: In 236 patients (177 tricuspid aortic valve and 59 bicuspid aortic valve) matched by initial severity of mild or moderate aortic stenosis, we retrospectively analyzed baseline echocardiogram at diagnosis with latest available follow-up echocardiogram. Baseline comorbidities, annua

Polyamines and microbiota in bicuspid and tricuspid aortic valve aortopathy.

Polyamines are small aliphatic cationic molecules synthesized via a highly regulated pathway and involved in general molecular and cellular phenomena. Both mammalian cells and microorganisms synthesize polyamines, and both sources may contribute to the presence of polyamines in the circulation. The dominant location for microorganisms within the body is the gut. Accordingly, the gut microbiota probably synthesizes most of the polyamines in the circulation in addition to those produced by the mammalian host cells. Polyamines are mandatory for cellular growth and proliferation. Established evide

Aortic Diameter Growth in Children With a Bicuspid Aortic Valve.

Knowledge of aortic growth in patients with bicuspid aortic valve (BAV) is essential to identify patients at risk for dissection, but data on children remain unclear. We retrospectively evaluated the aortic diameters of all pediatric BAV patients, identified through an echocardiographic database (2005 to 2013). Medical records were reviewed and aortic diameters re-measured on echocardiographic images at diagnosis and if available on variable mid- and endpoints follow-up. Dilatation (z-score >2) was based on 2 different z-score equation methods (Gautier/Campens). In 234 of the total 250 BAV pat

Moving a Blocked-Out Bicuspid into the Arch Using the"Sleeze".

Dental crowding can frequently result in a tooth becoming palatally blocked-out. When the tooth is displaced too far, tying the tooth to the archwire can become unfeasible. A modified "singshot" technique is presented to bring a blocked-out bicuspid into the arch. The lastomeric chain acted as a "sling shot" squeezing the bicuspid in, hence the name "sleeze."

A Bicuspid Aortic Valve Imaging Classification for the TAVR Era.

OBJECTIVES: This study sought to evaluate transcatheter aortic valve replacement (TAVR) in bicuspid aortic valve (BAV) aortic stenosis (AS), with a particular emphasis on TAVR-directed bicuspid aortic valve imaging (BAVi) of morphological classification. BACKGROUND: TAVR has been used to treat BAV-AS but with heterogeneous outcomes and uncertainty regarding the relevance of morphology. METHODS: In 14 centers in the United States, Canada, Europe, and Asia, 130 BAV-AS patients underwent TAVR. Baseline cardiac computed tomography (CT) was analyzed by a dedicated Corelab. Outcomes were assessed

Evolution of the Z-score in size-reduced bicuspid homografts.

BACKGROUND AND AIM OF THE STUDY: Human homografts are frequently used to establish an anatomic continuity between the right ventricular outflow tract (RVOT) and the pulmonary artery. Their limited availability, especially in small sizes, has encouraged the use of alternative strategies, such as size-reduced bicuspid homografts. The study aim was to analyze the follow up of patients who had received a standard tricuspid or size-reduced bicuspid homograft in the RVOT position, and to investigate modifications of the patients' Z-scores over the years. METHODS: A consecutive series of 107 patient

Are bicuspid aortic valves a limitation for aortic valve repair?

OBJECTIVE: To compare the mid-term results after aortic valve (AV) repair in bicuspid AVs with those in tricuspid AVs. METHODS: Between 2000 and 2010, 100 patients (mean age 47.2 years) underwent AV repair procedures for insufficient bicuspid AV (n=43) and tricuspid AV (n=57). Aortic regurgitation (AR) more than moderate was present in 31/43 and 21/57 patients in the bicuspid AV and the tricuspid AV group, respectively. Concomitant root replacement by either the reimplantation or the remodeling technique was performed in 42 patients (bicuspid AV 17/43, tricuspid AV 25/57). All patients were p

[Recurrent syncope in a male patient with bicuspid aortic valve].

We present a case of a 31 year-old male patient with a congenital heart disease - bicuspid aortic valve with secondary aortic valve insufficiency, in whom recurrent syncope associated with episodes of complete atrioventricular block was observed. After implantation of a pacemaker syncope did not reccur. Due to risk of complications with bicuspid aortic valve it was decided that surgical treatment should be considered in this case.

The bicuspid aortic valve and its relation to aortic dilation.

BACKGROUND: A bicuspid aortic valve (BAV) is a common congenital heart disease, which affects 1-2% of the population. However, the relationship between BAVs and aortic dilation has not been sufficiently elucidated. METHODS: A total of 241 BAV patients who were referred to this hospital for cardiac surgery over a 4.75-year period were included in this study. In addition to the clinical characteristics of the included patients, the morphological features of the aortic valve and aorta, the length of the left main coronary artery, and the laboratory findings (the coagulation and hematological par

Aortopathy is prevalent in relatives of bicuspid aortic valve patients.

OBJECTIVES: This study aimed to determine the prevalence of dilation and abnormal elastic properties of aortic root in first-degree relatives (FDRs) of bicuspid aortic valve (BAV) patients. BACKGROUND: Evidence indicates that BAV is a genetic disorder. Although FDRs of affected individuals have an increased prevalence of BAV, their risk of aortic root abnormalities is unknown. METHODS: We studied dimensions as well as the elastic properties of the ascending aorta in 48 FDRs with morphologically normal tricuspid aortic valves, 54 BAV patients, and 45 control subjects using 2-dimensional echoc

Repair of bicuspid aortic valves in patients with aortic regurgitation.

BACKGROUND: Bicuspid aortic valve regurgitation can be caused by a defect in the valve itself or by dysfunction of one or more components of the aortic root complex. A successful repair thus requires correction of all aspects of the problem simultaneously. We review our experience addressing both the valve and the aortic root when correcting bicuspid valve regurgitation. METHODS AND RESULTS: Between 1996 and 2004, we treated 68 patients for aortic regurgitation. Thirty patients had isolated aortic regurgitation, and 38 had an associated ascending aortic aneurysm. All patients were treated usi

In vivo ultrasonic debridement forces in bicuspids: a pilot study.

BACKGROUND: Hand instruments lead to application of excessive forces, resulting in extensive root substance removal. Ultrasonic debridement primarily removes accretions from root surfaces. However, there is a lack of information on procedural aspects of ultrasonic debridement. The purpose of the present study was to assess the forces applied by different therapists during ultrasonic root surface debridement of bicuspids in periodontal patients in vivo. METHODS: Ten dentists and 10 dental hygienists each debrided 1 aspect of a bicuspid with moderate chronic periodontitis using an ultrasonic cu

Remodeling of the aortic root and reconstruction of the bicuspid aortic valve.

BACKGROUND: Currently, isolated reconstruction of a regurgitant bicuspid aortic valve can be performed with adequate early results. Dilatation of the proximal aorta is known to be associated with this valve anomaly and may be partially responsible for the development of primary regurgitation or secondary failure of valve repair. We have used repair of the bicuspid valve with remodeling of the aortic root as an alternative to insertion of a composite graft. METHODS: Between October 1995 and May 1999, 16 patients (12 men, 4 women, aged 35 to 73 years) were seen with a regurgitant bicuspid aorti

[Clinicopathologic study of congenital bicuspid aortic valve in the aged].

Congenital bicuspid aortic valve was found in 25 patients(18 men, 7 women, mean age 78 years) of 7,000 consecutive autopsy cases over 60 years old(0.36%). Nine cases showed aortic stenosis(Group AS), 7 cases showed aortic stenosis and regurgitation(Group ASR) (4 cases with stenosis dominancy, 3 cases with regurgitation dominancy), 9 cases showed no aortic stenosis or regurgitation(Group Non-ASR) and no cases showed pure aortic regurgitation. The groups were compared clinically and pathologically. Cardiac death was frequently found in Groups AS and ASR. The aortic valve was the thickest in Grou
