PURPOSE: Multiple studies have shown that spectral analysis of tissue autofluorescence can be used as a live indicator for various pathophysiological states of cardiac tissue, including ischemia, ablation-induced damage, or scar formation. Yet today there are no percutaneous devices that can detect autofluorescence signals from inside a beating heart. Our aim was to develop a prototype catheter to demonstrate the feasibility of doing so.
METHODS AND RESULTS: Here we summarize technical solutions leading to the development of a percutaneous catheter capable of multispectral imaging of intracar

BACKGROUND: Cardiac resynchronization therapy (CRT) device implantation is hampered by difficult placement of the left ventricular (LV) lead. We have routinely used a steerable electrophysiology catheter to guide coronary sinus (CS) cannulation and facilitate LV lead positioning. The aim of this prospective study is to present our results with this approach in 138 consecutive patients receiving a CRT device over 10 years.
METHODS: The study included 120 men and 18 women, aged 64.8±11.4 years, with coronary disease (n=63), cardiomyopathy (n=72), or other disease (n=3), and mean ejection fracti

OBJECTIVE: The long-term outcomes of catheter ablation of atrial fibrillation (AF) developing post-cardiac valve replacement (VR) remain undefined.
METHODS AND RESULTS: Eighty-nine post-VR patients with AF (44% longstanding persistent AF, LSP-AF) were enrolled. Cumulative success rate of circumferential pulmonary vein ablation (CPVA for paroxysmal AF) and bidirectional block of lines and disappearance of complex fractionated atrial electrograms (CFAEs for persistent and LSP-AF) as index and repeat procedural endpoints reached 57% (mean, 1.3 procedures) during the first year, and dropped to 42

INTRODUCTION: Multiple cardiac ruptures after radiofrequency catheter ablation that requires surgical repair are uncommon.
METHODS AND RESULTS: We describe a 64-year old male patient with paroxysmal atrial fibrillation who had a cardiac tamponade following radiofrequency ablation. Surgical exploration demonstrated two ruptures in the left atrium, one in the right atrium, and one hematoma in the right atrium. MEDLINE, the Cochrane Library, and related databases were searched up to June 2011 without language restrictions, and related literature was reviewed and discussed. The patient has surviv

We report a case of pulmonary hemorrhage suspected to be caused by a pulmonary artery catheter (PAC) after cardiac surgery. An 80-year-old woman underwent aortic valve replacement, mitral valve replacement, and tricuspid annuloplasty. After anesthesia induction, a PAC was inserted from the right jugular vein and fixed at 48 cm, but it could not be wedged. Therefore, the PAC was fixed at 59 cm to measure the pulmonary capillary wedge pressure during the operation. The catheter showed a postoperative bending. The chest X-ray radiograph obtained at the first postoperative day showed a shadow in t

BACKGROUND: Catheter ablation is assuming a larger role in the management of patients with cardiac arrhythmias. Conventional fluoroscopic catheter mapping has limited spatial resolution and involves prolonged fluoroscopy. The non-fluoroscopic electroanatomic mapping technique (CARTO) has been developed to overcome these drawbacks.
OBJECTIVES: To report the early and late outcome in patients with different arrhythmias treated with radiofrequency ablation combined with the CARTO mapping and navigation system.
METHODS: The study cohort comprised 125 consecutive patients with different cardiac a

The use of pulmonary artery catheters (PACs) during cardiac surgery varies considerably depending on local policy, ranging from use in 5-10% of the patient population to routine application. However, as in other clinical fields, recent years have witnessed a progressive decline in PAC use. One of the reasons for this is probably the increasing use of transoesophageal echocardiograpy, even though careful analysis of the information provided by PAC and transoesophageal echocardiograpy indicates that the two tools should be considered subsidiary rather than alternatives. The principal categories

INTRODUCTION: Cardiac tachyarrhythmias present as supraventricular or ventricular tachycardia. Catheter ablation has completely revolutionised the treatment of patients with these arrhythmias.
METHOD: We reviewed the experience of radiofrequency catheter ablation in a single centre.
RESULTS: A total of 1,022 patients underwent radiofrequency catheter ablation from October 1991-December 1997. There were 480 patients who had AV nodal re-entrant tachycardia, 429 patients with accessory pathways, 7 patients with both AV nodal re-entrant tachycardia and accessory pathways, 4 patients with both AV

BACKGROUND: Peripherally inserted central catheters (PICCs) are commonly used in neonatal intensive care units (NICUs) to provide prolonged intravenous access for preterm and critically ill infants. They are generally preferred over surgically placed central venous catheters because of easier insertion and, in some settings, lower infection rates. However, rare but life-threatening complications may occur. Cardiac tamponade, resulting from catheter malposition or delayed migration with perforation of the superior vena cava (SVC) or cardiac wall, is one such complication. This case highlights t

AIMS: The diagnosis of infiltrative cardiomyopathies has increased over last years. Catheter ablation is becoming the preferred approach for managing atrial fibrillation (AF) in these patients. This study aims to characterize differences in procedural findings during AF ablation in patients with and without infiltrative cardiomyopathies.
METHODS AND RESULTS: Patients with cardiac amyloidosis and cardiac sarcoidosis undergoing first-time AF ablation were propensity score matched in 1:4 ratio to separate reference groups that received trigger provocative manoeuvres (isoproterenol infusion and/o

BACKGROUND: Cardiac sarcoidosis (CS) is a rare, potentially life-threatening condition associated with ventricular tachycardia (VT). Outcomes of catheter ablation for VT in patients with histologically diagnosed sarcoidosis and those with suspected or clinically diagnosed sarcoidosis have not been well studied. This study addressed this knowledge gap.
METHODS AND RESULTS: We conducted an observational retrospective chart review of patients with CS who underwent VT ablation between 2007 and 2024 at Mayo Clinic Hospital. The cohort was divided into 2 groups: those with histologically diagnosed

A woman in her 70s presented to accident and emergency (A&E) with shortness of breath and fever following atrial flutter ablation. Initial investigations revealed a new onset of pleural and pericardial effusion with raised inflammatory markers. After systematically ruling out chest infection and heart failure, a diagnosis of post-cardiac injury syndrome (PCIS) was made. After a short course of steroids and colchicine, she showed significant improvement in her symptoms, and subsequent follow-up showed resolution of her pleural and pericardial effusion.

Iatrogenic ventricular perforation of the myocardial wall is a rare but life-threatening complication. It has been described using pulmonary artery catheter, pigtail catheter, and Judkins catheter. Straight wires and catheters can be used to cross the aortic valve for left ventriculogram; however, the risk of perforation is higher compared with J-tip wires. Prompt recognition of cardiac tamponade and pericardial drain insertion is vital, but surgical patch repair may be required for definitive treatment. This case highlights the importance of increased vigilance and prompt management of cardia

Ventriculoperitoneal shunting is the most common neurosurgical procedure for treatment of hydrocephalus. Shunt-related complications are relatively common and associated with a high rate of shunt revision. However, migration of the distal ventriculoperitoneal shunt, especially into the cardiac and intravascular regions, has rarely been reported. Awareness of this rare but potentially hazardous complication is important owing to its significant morbidity, which can be prevented by prompt management. Here, we introduce a case of a 23-year-old male with migration of the distal shunt catheter thro

BACKGROUND: Catheter ablation (CA) is an effective technique for the management of atrial fibrillation (AF). Cardiac computed tomography (CCT) is a non-invasive imaging modality that is used as a crucial part of planning before CA procedures which can detect other incidental findings and require further diagnostic investigations.
OBJECTIVES: We sought to assess the prevalence and distribution of incidental CCT findings in patients with AF undergoing CA.
METHODS: Retrospective analysis over a three-year period (2013-2016) of 218 patients undergoing CCT prior to AF CA. CCT findings were analyz

Cardiac catheterization associated with central vein cannulation can involve potential thrombotic and infectious complications due to multiple cannulation trials or improper placement. To minimize the risks, medical simulators are used for training. Simulators are also employed to test medical devices such as catheters before performing animal tests because they are more cost-effective and still reveal necessary improvements. However, commercial simulators are expensive, simplified for their purpose, and provide limited access sites. Inexpensive and anatomical cardiovascular simulators with ce
