Infantile tinea faciei caused by Trichophyton indotineae successfully treated with topical ketoconazole: A case report.

BACKGROUND: Dermatophyte infections are increasing worldwide, with shifts in the distribution of causative species. Trichophyton indotineae has gained attention due to its antifungal resistance. Infections caused by Trichophyton indotineae in infants are rarely reported. CASE PRESENTATION: We report a case of tinea faciei caused by Trichophyton indotineae in a 5-month-old Vietnamese infant, likely infected via maternal transmission. Fungal species identification was confirmed by ITS1-5.8S-ITS2 sequencing. Antifungal susceptibility testing revealed susceptibility to terbinafine (MIC = 0.0313 µ

Topical MSC-Derived Extracellular Vesicles Accelerate Healing of Second-Degree Burns: A Case Report.

Burn injuries are challenging to treat due to wound progression and long-term scarring. Mesenchymal stem cell (MSC)-derived extracellular vesicles (EVs) have regenerative and immunomodulatory properties that may mitigate these complications. In this Phase 1/2a, first-in-human, open-label study, a 47-year-old man with deep second-degree burns received a single topical dose of allogeneic bone marrow MSC-derived EVs within 48 hours of injury. The treated wound achieved >99% closure within one week, with durable healing for over one year. Scar outcomes improved over 52 weeks, with POSAS scores ind

Multimodal Approach for Recalcitrant Melasma Using Picosecond Laser and Topical JAK Inhibition: A Case Report.

BACKGROUND: Melasma is a chronic hyperpigmentation disorder with high relapse rates, particularly in Fitzpatrick skin types III-V. Increasing evidence implicates cytokine-driven inflammation and visible light-induced melanogenesis as key contributors to disease persistence. These mechanisms support the rationale for exploring a multimodal therapeutic approach. CASE PRESENTATION: A woman in her early 30s with phototype IV presented with a 5-year history of recalcitrant centrofacial melasma. Previous treatments-including hydroquinone, oral tranexamic acid, and superficial chemical peels-produce

Successful Addition of Topical Tapinarof for Residual Psoriatic Lesions During Apremilast Therapy: A Case Report.

In some patients with psoriasis, residual skin lesions persist despite systemic treatment with apremilast. However, switching to biological agents may be challenging due to comorbidities, or dose reduction may be required due to side effects, leading to relapses. We report the case of an 86-year-old male with plaque psoriasis and psoriatic arthritis who achieved complete clearance after the addition of topical tapinarof to his apremilast regimen. Although his skin and joint symptoms initially improved with apremilast, mild residual cutaneous lesions recurred after three years (psoriasis area a

Short-Term Topical Minoxidil Use Associated with Acute Central Serous Chorioretinopathy: A Case Report.

INTRODUCTION: Minoxidil is a topical, over-the-counter, FDA-approved drug used to treat male and female pattern hair loss. Three case reports were published associating long-term use of topical minoxidil to central serous chorioretinopathy (CSCR) after 4, 6, and 8 months of use. In this report, we will present a case of CSCR after 1 week of topical minoxidil use that resolved with discontinuation of medication and observation 6 weeks later. CASE PRESENTATION: A 34-year-old Hispanic male presented with a 2-week history of central visual loss and flashes in the left eye. His only medication use

Epinastine Eyelid Cream as a Practical Option for Patients With Glaucoma on Multidrug Topical Therapy: A Case Report.

Patients with glaucoma often require long-term use of multiple ophthalmic medications, which can make adherence to additional treatments, such as anti-allergic eye drops for seasonal allergic conjunctivitis, particularly challenging. Moreover, corticosteroid eye drops or ointments, commonly used to manage ocular allergy, are generally avoided in these patients due to the risk of increasing intraocular pressure. We report a case in which a patient with glaucoma, already undergoing multiple topical therapies, experienced recurrent allergic conjunctivitis and blepharitis each spring. The use of c

Topical Insulin Application in the Management of Resistant Leg Ulcers in a Patient With Prolidase Deficiency: A Case Report.

Leg ulcers are a significant cause of morbidity and mortality and can be caused by vascular, neuropathic, infectious, and traumatic factors, as well as rare metabolic diseases like prolidase deficiency. Despite various wound care methods and systemic treatments, managing ulcers can be challenging. This case presents a male patient with prolidase deficiency for 35 years whose leg ulcers were resistant to standard treatments such as wound dressings, topical treatments, and hyperbaric oxygen therapy. Considering the ulcers' resistant nature, we applied topical insulin to ulcers as an add-on thera

Cerebral Venous Thrombosis With Topical Estrogen-Progesterone Use in a Young Heterosexual Male: A Case Report.

INTRODUCTION: Our report sheds light on the risk of topical hormonal use in relation to cerebral venous sinus thrombosis. We diagnosed our patient with cerebral venous thrombosis (CVT) using computed tomography venogram, then detailed history and examination were obtained, and thorough blood tests and imaging were done to exclude other causes of CVT like thrombophilias, infections, and malignancies. CASE REPORT: Our patient is a 37-year-old heterosexual male, presented with headache only. The computed tomography venogram showed extensive CVT in the right internal jugular vein, sigmoid, transv

Combined Therapy with Laser and Autologous Topical Serum for Facial Rejuvenation: A Multiple Case Series Report.

BACKGROUND: Lasers require several sessions to achieve significant results and may lead to adverse reactions. Platelet-rich plasma (PRP) therapy is a good adjuvant to laser therapies; however, repeated blood extractions and invasive injections are needed. A 100% autologous topical formulation based on the patient's proteins has been recently developed, known as Endoret-Serum (ES). Unlike other PRPs, ES provides a home care, storable, and topical needle-free application. OBJECTIVE: Preliminarily assess the clinical performance of a single session of the combined therapy with nonablative laser

Application of Topical Lidocaine for Pain Relief in the Setting of Targeted Therapy-Induced Onycholysis: A Case Report.

Selective pan fibroblast growth factor receptor (FGFR) inhibitors have been linked to severe onycholysis, the uncomfortable separation of the nail plate from the nail bed. Recommendations to assist with FGFR inhibitor onycholysis vary based on the severity. We hypothesized that the application of topical lidocaine to mimic a digital nerve block would be beneficial in addition to traditional supportive care interventions and subsequently report its immediate and continued efficacy for targeted therapy-induced onycholysis.

Resolution of Recurrent Oro-facial Herpes Simplex Using a Topical Botanical Gel: A Case Report.

Oral herpes labialis, more commonly known as cold sores, are a common encountered viral infection involving herpes simplex virus-1 (HSV-1). Although relatively benign, these lesions can be both unsightly and clinically difficult to manage. Prescription standards of care and over-the-counter agents, such as docosonal, have often shown only limited efficacy in both decreasing lesional pain and reducing duration of lesional symptomology and are not without potential side effects. Despite some success with acute remediation, recurrent episodes often occur, with seemingly no imparted protection or

Topical tetracycline in the treatment of a 7-year-old child with necrotizing ulcerative gingivitis: a case report.

A 7-year-old girl presented with necrotic gingival tissue manifesting as large ulcers in the labial and lingual gingival tissue along the region of the mandibular central incisors and right canine. The severe symptoms associated with the gingival lesions were an indication for the use of a topical tetracycline solution. The patient's parent was instructed to prepare the antibiotic solution by crushing a 250-mg tablet of tetracycline in 10 mL of distilled water. Initially, the patient was instructed to use the tetracycline solution 3 times daily for 7 days by swishing the solution around her mo

Unilesional plantar mycosis fungoides treated with topical photodynamic therapy - case report and review of the literature.

BACKGROUND: Unilesional mycosis fungoides (UMF) and pagetoid reticulosis (PR) are variants of mycosis fungoides. Conventional therapy comprises surgical excision or radiotherapy, which may be associated with long-term side effects, especially when the lesion is located at a special site like the palms and soles. Therefore, alternative treatment options are needed to treat solitary lesions in the case of UMF or PR. Recently, topical photodynamic therapy (PDT) has been described to be an efficient and non-invasive therapeutical option with excellent clinical outcome. OBJECTIVE: The objective of

Granuloma gluteale adultorum associated with use of topical benzocaine preparations: case report and literature review.

BACKGROUND: Granuloma gluteale infantum is a skin disorder of controversial etiology manifested clinically by oval reddish-purple granulomatous nodules on the gluteal surfaces and groin areas of infants. Similar granulomas are noted in adults and the elderly and are referred to as granuloma gluteale adultorum and diaper area granuloma of the aged, respectively. Occlusion from diapers, paper napkins, plastic pants, detergents, starch, powder, halogenated steroids, candidal infection, and urine and feces are postulated as possible etiologies. OBJECTIVE: We report a case of a 40-year-old woman p

Severe methemoglobinemia from topical anesthetic spray: case report, discussion and qualitative systematic review.

Few health care professionals realize that topical anesthetic spray can cause methemoglobinemia. We describe a 56-year-old woman who was transferred to our emergency department when severe cyanosis and chest pain developed after administration of topical oropharyngeal benzocaine and lidocaine during outpatient endoscopy. Investigations revealed a methemoglobin level of 51%. Despite rapid diagnosis and treatment with methylene blue, pulmonary edema consistent with adult respiratory distress syndrome developed, endotracheal intubation was required, and the patient suffered a lengthy course in th
