BACKGROUND: Prolonged chest tube duration is less well studied in patients who are supported by mechanical ventilation and have acquired pneumothorax. We investigated the impact of prolonged chest tube duration on patient outcomes and the risk factors associated with prolonged chest tube duration.
METHODS: This retrospective observational study included 106 ventilated subjects who had been treated with thoracostomy for pneumothorax between May 2004 and December 2011. We analyzed 61 subjects and 63 events. The subjects were divided into a prolonged chest tube duration group (> 18 d) and a non-

BACKGROUND: Numerous studies have suggested an inverse relationship between hospital volume and short-term mortality after various major operations. However, the volume-outcome relationship after lung cancer surgery remains controversial. We investigated the effects of hospital volume on various outcomes after lobectomy for lung cancer, including chest tube duration, postoperative length of stay, and in-hospital mortality.
METHODS: From a total of 5.85 million inpatients in the Japanese Diagnosis Procedure Combination database, we identified 19,831 patients who underwent lobectomy for lung ca

BACKGROUND: Tube thoracostomy (TT) is central in the management of chest trauma sufficing in over 80% of cases. As a result the procedure is commonly performed in most emergency departments.
OBJECTIVE: The aim of this study was to assess the efficacy and complications of TT using Advanced Trauma Life Support (ATLS) technique in chest trauma.
METHODS: This prospective study was done at the Trauma Unit of the University of Benin Teaching Hospital in Nigeria. All patients with chest trauma who needed tube thoracostomy between February 2006 and February 2009 were studied. Data recorded for each

Aortic perforation caused by the friction of a chest tube is a rare but life-threatening complication of coronary artery bypass grafting (CABG). We report a case involving CABG for triple-vessel disease and unstable angina. Reexploration was necessary 24 hours after the operation because of the sudden onset of pronounced bleeding. Perforation of the ascending aorta caused by friction of the chest tube was found. Bleeding was controlled by means of a purse string suture with 4-0 Prolene. Use of pericardium membrane between the heart and the chest tube and the choice of smaller and more flexible

BACKGROUND: Little evidence exists to describe expected volumes of chest tube (CT) drainage after coronary artery bypass grafting (CABG).
OBJECTIVES: The study objective was to map the trajectory of CT drainage volumes from insertion to removal after CABG.
DESIGN: This was a retrospective, descriptive study.
PATIENTS: The study included 239 patients who underwent CABG at a single metropolitan hospital in Melbourne, Australia.
RESULTS: The sample (N = 234), aged 68.7 years (standard deviation [SD] 9.9), was predominantly male (N = 185, 79.1%). The mean duration of CT insertion was 45.2 hour

OBJECTIVE: A chest tube is usually placed in the pleural cavity after wedge resection of the lung, even after thoracoscopic procedures. The aim of this study was to determine the validity and safety of postoperative management without chest tube placement for patients undergoing thoracoscopic wedge resection of the lung.
METHODS: Between 1998 and 2002, 93 patients underwent thoracoscopic wedge resection of the lung. In January 2000, we established the following criteria for avoiding chest tube placement: (1) absence of air leaks during intraoperative alternative sealing test, (2) absence of b

Intercostal artery aneurysms can be of congenital, posttraumatic, or mycotic type. Intercostal arterial aneurysmal bleeding can be brisk enough to lead into shock or sudden death. Bleeding through chest tubes has been reported due to various causes; it is commonly due to injury to intercostal or pulmonary vessels and is occasionally due to leaking aortic aneurysms. We describe a patient who had development of repeated episodes of brisk bleeding through an empyema chest tube leading to shock episodes requiring resuscitations. After extensive search, the bleeding source was found to be an interc

OBJECTIVES: "Enhanced recovery after thoracic surgery" is increasingly implemented following surgery for primary spontaneous pneumothorax (PSP). Historically, postoperative chest tubes are left in place for several days to ensure adequate pleurodesis and prevent recurrence. Prompt chest tube removal upon cessation of air leakage may enhance recovery but potentially increases risk of recurrence. We evaluated progressive chest tube management following surgical pleurodesis for PSP.
METHODS: Retrospective multicentre study of patients undergoing surgical pleurodesis for PSP between 2020 and 2023

BACKGROUND: Chest tube dislodgement is a frequent and potentially life-threatening complication after thoracic trauma. Reliable fixation is essential, particularly in emergency and combat casualty care. Several suture techniques exist, but comparative biomechanical evidence is limited.
METHODS: This prospective biomechanical cadaveric study compared three fixation techniques-Purse String (PS), Roman Sandal (RS), and Modified Johannesburg (JO) technique -across three tube sizes (20, 24, 28 Charrière) and two materials (polyvinyl chloride (PVC) and silicone). A total of 360 tests were performed

OBJECTIVES: Bleeding is a common complication of extracorporeal membrane oxygenation (ECMO) support. While chest tube treatment is infrequently linked to bleeding in non-ECMO patients, several reports suggest a markedly increased risk during ECMO support, sometimes requiring surgical intervention. The true incidence, contributing factors, and effect on outcome from chest tube related bleeding in ECMO patients is unknown.
METHODS: This was a single-centre observational study conducted between 2010 and 2024, including both paediatric and adult patients treated with chest tube for pneumothorax o

Supraventricular tachycardia (SVT) is commonly associated with intrinsic cardiac or metabolic causes. However, mechanical irritation of mediastinal structures from thoracic intervention is an uncommon and often overlooked trigger. Irritation of the pericardium or nearby autonomic fibers can disrupt normal conduction pathways and provoke reentrant arrhythmias such as SVT. We report a case of a 40-year-old male with diabetes mellitus who initially underwent right-sided intercostal (IC) tube insertion for a pneumothorax. Following accidental tube dislodgement, he developed recurrent pneumothorax

PURPOSE: Air leaks are common after pulmonary surgery. Prolonged air leaks (PALs) may persist through discharge and often are managed with one-way valve devices (OWD). We sought to determine the course and complications of patients discharged with OWDs, risk factors for complications, and to evaluate the utility of clamp trials before chest tube (CT) removal.
METHODS: Single-institution, retrospective review of patients discharged with a OWD after pulmonary surgery between 2008 and 2022. Charts were examined for the presence of complications and CT duration. Differences in CT duration were co

BACKGROUND: The current discourse within the thoracic surgical and pulmonological communities pertains to a contentious debate over the optimal selection criteria for thoracostomy tube diameters utilized in the management of pleural effusions. A comprehensive examination of the variables that inform the clinical decision-making paradigm for the determination of appropriate chest tube calibers is imperative to enhance patient management and elevate the prognostic results.
OBJECTIVES: The objective of this inquiry is to elucidate the determinants that influence thoracic surgeons and pulmonologi

BACKGROUND: The sunken flap or sinking skin flap syndrome is a complication that can be observed following decompressive craniectomy. More rare, sinking skin flap syndrome can occur as an iatrogenic complication of pleural effusion evacuation via chest tube placement in the presence of ventriculopleural shunt.
CASE PRESENTATION: We report the case of a Hispanic male patient in his 20s who presented to the emergency department after sustaining a penetrating gunshot wound to the head. In addition to undergoing an emergent decompressive craniectomy, a ventriculopleural shunt was subsequently pla

BACKGROUND: To investigate optimizing the use of different beam shaping filters (viz. small, medium and large) when using different tube voltages during the newborn chest computed tomography (CT) on a GE Lightspeed VCT scanner.
METHODS: We used pediatric anthropomorphic phantoms with a 64 detector-row CT scanner while scanning the chest. A real-time skin dosimeter (RD - 1000; Trek Corporation, Kanagawa, Japan) was positioned into the phantom center of the body, the surface of the body back, and the right and left mammary glands. We performed and compared six scan protocols using small, medium

BACKGROUND: Pleural collection is a common medical problem. For decades, the chest tube of different designs was the commonly used toll for pleural drainage. Over the past few years, small-bore catheter (SBC) has gained more popularity. We present our experience of using SBCs for the drainage of pleural collection of different etiologies.
PATIENTS AND METHODS: A total of 398 small-bore pleural catheters were inserted in 369 patients with pleural collection during the period from January 2013 to October 2019. Data were collected regarding the efficacy of drainage, experienced chest pain, durat

In many Dutch hospitals, treatment of spontaneous pneumothorax (SP) routinely consists of in-hospital chest tube drainage. Alternatives, such as ambulatory Heimlich valves or manual aspiration of the pneumothorax, have become available in recent years. Neither treatment requires patient hospitalisation and pneumothorax recurrence rate is the same as after in-hospital treatment. A recent study demonstrated the same results for conservative treatment of SP (no intervention at all, outpatient-based) compared to chest tube drainage. In the future, treatment of SP should be outpatient-based as much
