Laboratory test of an automated cell analysis system for cervical screening.

An automated cell analysis system (Autoplan-MIAC) for the early detection of precancerous lesions of the cervix was tested under semi-routine conditions in a clinical cytology laboratory. A set of 1500 specimens, highly enriched with abnormal cases, was analysed. Cervical scrapings were collected in suspension and processed by cytocentrifugation for microscopy. Two slides were prepared from each sample: one for staining according to Papanicolaou for the visual reference diagnosis and one for Feulgen staining for automated analysis. The specimens were evaluated in two ways: the first one, which

Unlocking the puzzle of primary monosymptomatic nocturnal enuresis: a comprehensive study on the management of Thai children through alarm therapy and medication.

BACKGROUND: Primary monosymptomatic nocturnal enuresis (PMNE) is a common pediatric condition that can improve with age but can profoundly impact self-esteem and quality of life. Despite the availability of treatment options in Thailand, there are no standardized guidelines. This study aimed to evaluate the efficacy of three approaches: enuresis alarm, pharmacological intervention, and their combination, to provide evidence-based recommendations for culturally relevant clinical guidelines for PMNE and explore factors influencing treatment efficacy. METHODS: A 5-year retrospective study was co

Derivation and Validation of an Algorithm to Detect Stroke Using Arm Accelerometry Data.

Background Early diagnosis is essential for effective stroke therapy. Strokes in hospitalized patients are associated with worse outcomes compared with strokes in the community. We derived and validated an algorithm to identify strokes by monitoring upper limb movements in hospitalized patients. Methods and Results A prospective case-control study in hospitalized patients evaluated bilateral arm accelerometry from patients with acute stroke with lateralized weakness and controls without stroke. We derived a stroke classifier algorithm from 123 controls and 77 acute stroke cases and then valida

Panic, suffocation false alarms, separation anxiety and endogenous opioids.

This review paper presents an amplification of the suffocation false alarm theory (SFA) of spontaneous panic [Klein DF (1993). False suffocation alarms, spontaneous panics, and related conditions. An integrative hypothesis. Arch Gen Psychiatry; 50:306-17.]. SFA postulates the existence of an evolved physiologic suffocation alarm system that monitors information about potential suffocation. Panic attacks maladaptively occur when the alarm is erroneously triggered. That panic is distinct from Cannon's emergency fear response and Selye's General Alarm Syndrome is shown by the prominence of intens

[Acute loss of vestibular function: an emergency for patient and physician].

The dramatic symptomatology of sudden loss of vestibular function alarms the patient and the physician who is consulted in an emergency situation. The continuous vertigo is often accompanied by marked autonomic symptomatology, so that heart and cerebral diseases as well as acute intoxication are often primarily diagnosed. The differential diagnosis, clinical features, course of the disease and treatment are presented in this study of 54 patients treated in our department between 1980 and 1985. Aetiology and pathogenesis are discussed on the basis of the literature.

The effect of in-line infusion filtering on in-line pressure monitoring in an experimental infusion system for newborns.

AIM: To study the effect of in-line filtering on in-line pressure measurement in a simulated infusion system. METHODS: The experimental setup consisted of a syringe pump, syringe, intravenous (IV) line, and extension line. For aqueous fluids, a 0.2-μm infusion filter was used, and for lipid emulsions, a 1.2-μm filter. Infusion speed varied from 0.5 to 10.0 mL/h. First, the effect of clamping the extension line was studied on reaching alarm threshold. Second, the effect of aqueous and lipid fluids was evaluated on in-line pressure measurements. RESULTS: In-line placement of an infusion filter

Insights into the problem of alarm fatigue with physiologic monitor devices: a comprehensive observational study of consecutive intensive care unit patients.

PURPOSE: Physiologic monitors are plagued with alarms that create a cacophony of sounds and visual alerts causing "alarm fatigue" which creates an unsafe patient environment because a life-threatening event may be missed in this milieu of sensory overload. Using a state-of-the-art technology acquisition infrastructure, all monitor data including 7 ECG leads, all pressure, SpO(2), and respiration waveforms as well as user settings and alarms were stored on 461 adults treated in intensive care units. Using a well-defined alarm annotation protocol, nurse scientists with 95% inter-rater reliabilit

Full of sound and fury, signifying nothing: burden of transient noncritical monitor alarms in a trauma resuscitation unit.

We examined the types of patient monitor alarms encountered in the trauma resuscitation unit of a major level 1 trauma center. Over a 1-year period, 316688 alarms were recorded for 6701 trauma patients (47 alarms/patient). Alarms were more frequent among patients with a Glasgow Coma Scale of 8 or less. Only 2.4% of all alarms were classified as "patient crisis," with the rest in the presumably less critical categories "patient advisory," "patient warning," and "system warning." Nearly half of alarms were ≤5 seconds in duration. In this patient population, a 2-second delay would reduce alarms b

Feasibility of blood pressure telemonitoring in patients with poor blood pressure control.

We examined the feasibility of using home blood pressure (BP) telemonitoring devices for managing patients with poor BP control. We enrolled 591 subjects with a diagnosis of hypertension. Patients were randomized to usual care (n = 147) or to the intervention arm (n = 441). Those in the intervention arm were issued with a home BP telemonitoring device. The device transmitted BP readings automatically via the home telephone line. Technical alerts were generated if patients did not transmit their BP readings according to the protocol. During the first six months, 693 technical alerts were genera

Understanding the performance and reliability of NLP tools: a comparison of four NLP tools predicting stroke phenotypes in radiology reports.

BACKGROUND: Natural language processing (NLP) has the potential to automate the reading of radiology reports, but there is a need to demonstrate that NLP methods are adaptable and reliable for use in real-world clinical applications. METHODS: We tested the F1 score, precision, and recall to compare NLP tools on a cohort from a study on delirium using images and radiology reports from NHS Fife and a population-based cohort (Generation Scotland) that spans multiple National Health Service health boards. We compared four off-the-shelf rule-based and neural NLP tools (namely, EdIE-R, ALARM+, ESPR

Lead times in the early management of traumatic brain injury: relation to geographic conditions and clinical outcomes in a nationwide Swedish registry study.

BACKGROUND: Traumatic brain injury (TBI) patients are at risk of sudden deterioration, requiring timely diagnostics and treatment to prevent secondary cerebral injuries. This study investigated lead times in prehospital and early intrahospital TBI management, assessing their association with geographical conditions, hospital caseloads, and patient outcomes. METHODS: This nationwide, observational cohort study included 5036 TBI patients (during 2018-2022) from the Swedish Trauma Registry (SweTrau). Lead times from trauma to alarm, from alarm to hospital arrival, and times to first computed tom

Laboratory and in-use assessment of methicillin-resistant Staphylococcus aureus contamination of ergonomic computer keyboards for ward use.

BACKGROUND: An ideal computer keyboard for clinical use would be easily cleanable and cleaned by staff, meet acceptable levels of usability, and not attract hospital bacteria. METHODS: In vitro studies were performed to demonstrate bacterial transfer between keyboard surfaces and gloves. This was followed by a usability study and a controlled trial of keyboard contamination in an intensive care unit both with and without an alarm to indicate the need for cleaning. Eight cleanable keyboards were placed at random beds and compared with standard keyboards. RESULTS: Bacteria were most easily rem

A Novel Fluorescence-Triggered Auditory Feedback Photosensor for Precision Lymph Node Mapping.

Background: In cancer surgery, resection of the primary tumor and regional lymph nodes (LNs) is critical. Adequate LN examination is essential to detect metastasis, which determines the cancer stage. Fluorescence emission allows for visual differentiation and rapid monitoring of LNs. Methods: Cancer tissue is stained with a fluorescent dye (indocyanine green, ICG) to identify LNs. Fluorescence is induced from the stained LNs using LED light, and a photosensor coupled with a speaker detects the fluorescence signal and triggers an audible alarm. Filters are applied to prevent false alarms. Resul

In-Hospital Hepatitis C Alarm System: A Strategy to Enhance Linkage to Care for Hepatitis C Virus Infection.

BACKGROUND/AIMS: The World Health Organization aims to eliminate hepatitis C virus (HCV) by 2030; however, linkage to care rates remain suboptimal. To address this, an in-hospital HCV alarm system integrated into electronic medical records (EMRs) was implemented to increase confirmatory testing and referral rates. METHODS: This retrospective study analyzed patients with positive anti-HCV antibody results at a tertiary hospital in Korea, comparing the pre-alarm period (August 2020-July 2022) with the post-alarm period (July 2022-May 2024). HCV RNA testing rates, liver clinic visits, and direct

[Serenity and objectivity in the face of severe childhood hepatitis: the diagnostic challenge].

The appearance of acute hepatitis of unknown aetiology in children has set off alarms in the World Health Organization and in member countries; identifying it and studying it lay out a diagnostic challenge for first-contact medical personnel and especially for pediatricians of Mexico, as well as for the coordination of care in health services. The international outlook and an analysis of the clinical manifestations are described, as well as key points for the identification and an observation of the samples that are requested for their study in Mexico.
