The nonword-reading deficit of disabled readers: a developmental interpretation.

This article presents empirical evidence challenging the received wisdom that a nonword-reading deficit is a characteristic trait of disabled readers. On the basis of 2 large-scale empirical studies using the reading-level match design, we argue that a nonword-reading deficit is the consequence of normal developmental differences in word-specific knowledge between disabled readers and younger normal readers (both groups being matched on real-word reading). The first study shows that the nonword-reading deficit varies as a function of age and reading level and that this deficit is not typical f

The clinical effectiveness of a multisensory therapy on clients with developmental disability.

Many clients in Hong Kong with developmental disabilities stay in mental hospitals because of mental disorders and behavioural problems. There is a need to identify strategies that promote psychological well-being and reduce problem behaviours in this group of clients. This study evaluates the impact of multisensory therapy on participants' emotional state, level of relaxation, challenging behaviour, stereotypic self-stimulating behaviour (SSB) and adaptive behaviour (AB). Using an experimental design, 89 participants were recruited from a developmental disability unit in a hospital in Hong Ko

Self-injurious behavior in the developmentally disabled: pharmacologic treatment.

The current status of pharmacological treatments of self-injurious behavior (SIB) and aggression in persons with mental retardation and autism was reviewed in the literature. Much of the existing literature is derived from anecdotal clinical experience, with a relative lack of well-controlled studies to determine the efficacy of different treatments. Although all psychotropics have been used to manage SIB and aggression, particularly promising are the data on the use of opioid antagonists like naltrexone. Beta-blockers may also have some role, but more controlled, systematic studies are needed

Toe walking. A marker for language disorders in the developmentally disabled.

Toe walking unassociated with an autistic disorder or cerebral palsy generally has been considered a normal infant gait. The incidence of toe walking in various diagnostic subgroups of 799 developmentally disabled children presenting to a tertiary-level multidisciplinary assessment clinic was reviewed to investigate the authors' clinical impression that toe walking may be a marker for language dysfunction. Toe walking was found to be more frequent in those diagnostic subgroups with more severe language disorders. Toe walking also correlated with lower IQ scores (p less than 0.0001). The sensit

Gastroesophageal reflux and Barrett's esophagus in developmentally disabled patients.

Twenty-seven patients from an institution for the developmentally disabled underwent endoscopy for evaluation of vomiting, regurgitation, rumination, or upper gastrointestinal bleeding. The presence of gastroesophageal reflux and Barrett's esophagus was determined retrospectively. Twenty-three patients had an IQ less than 20, 19 were nonambulatory, and 14 were taking at least one neuroleptic drug daily. Seven patients (26%) had histologically documented Barrett's esophagus of the specialized-columnar type. Two patients with Barrett's esophagus had benign esophageal strictures, but no cases of

Preparticipation and Sideline Evaluation of Athletes with Intellectual and Developmental Disability.

Individuals with intellectual and developmental disability face several barriers to participation in sports and physical activity. Fortunately, there are opportunities for athletes with intellectual and developmental disability to participate in sport. Special Olympics continues to be the most prominent opportunity. Unfortunately, many physicians and other health care providers may feel underprepared to evaluate and treat athletes with intellectual and developmental disability. While national and international events often have a large number of resources available, access to medical care for

BCS1L mutations produce Fanconi syndrome with developmental disability.

Fanconi syndrome is a functional disorder of the proximal tubule, characterized by pan-aminoaciduria, glucosuria, hypophosphatemia, and metabolic acidosis. With the advancements in gene analysis technologies, several causative genes are identified for Fanconi syndrome. Several mitochondrial diseases cause Fanconi syndrome and various systemic symptoms; however, it is rare that the main clinical symptoms in such disorders are Fanconi syndrome without systematic active diseases like encephalomyopathy or cardiomyopathy. In this study, we analyzed two families exhibiting Fanconi syndrome, developm

Genetic convergence of developmental and epileptic encephalopathies and intellectual disability.

AIM: To determine whether genes that cause developmental and epileptic encephalopathies (DEEs) are more commonly implicated in intellectual disability with epilepsy as a comorbid feature than in intellectual disability only. METHOD: We performed targeted resequencing of 18 genes commonly implicated in DEEs in a cohort of 830 patients with intellectual disability (59% male) and 393 patients with DEEs (52% male). RESULTS: We observed a significant enrichment of pathogenic/likely pathogenic variants in patients with epilepsy and intellectual disability (16 out of 159 in seven genes) compared wi

Priority vaccination for mental illness, developmental or intellectual disability.

Coronavirus vaccines have made their debut. Now, allocation practices have stepped into the spotlight. Following Centers for Disease Control and Prevention guidelines, states and healthcare institutions initially prioritised healthcare personnel and elderly residents of congregant facilities; other groups at elevated risk for severe complications are now becoming eligible through locally administered programmes. The question remains, however: who else should be prioritised for immunisation? Here, we call attention to individuals institutionalised with severe mental illnesses and/or development

Sleeve Gastrectomy for Youth With Cognitive Impairment or Developmental Disability.

BACKGROUND AND OBJECTIVES: Youth with cognitive impairment or developmental disability (CI/DD) face higher rates of obesity and secondary medical issues. Bariatric surgery may be a helpful tool for health improvement because it has been shown efficacious for adolescents. We aim to contribute to literature regarding bariatric surgery for adolescents with CI/DD and explore the association between cognitive functioning and weight loss outcomes. METHODS: Adolescents (N = 64) received a preoperative psychological evaluation, including cognitive assessment, and bariatric surgery at 1 weight loss pr

Risperidone for the treatment of fecal smearing in a developmentally disabled adult.

PURPOSE: The use of risperidone in decreasing fecal smearing behavior as an expression of nonverbal aggression in a developmentally disabled adult is discussed. SUMMARY: A 36-year-old Caucasian man had a history of fecal smearing when he did not get his way or was upset. The patient had profound mental retardation, had an IQ in the range of less than 20 to 25, and was nonverbal. For this patient, fecal smearing was not only a behavioral issue but also a medical concern because he was hepatitis B positive with noncarrier status. Serology also confirmed previous infection with hepatitis A. Risp

Dual diagnosis: developmental disability complicated by mental illness.

When mental retardation is complicated by mental illness, the clinical result is "dual diagnosis." Though recognized as such only since the 1970s, these patients have long been the subset of the developmentally disabled most resistant to integration into the community. The maladaptive behavior or mental illness often prevents training for the retardation or other developmental disability. Because these patients are no longer placed in the back wards of state institutions, the modern expectation is that even this most difficult group can be influenced by positive behavior support directed by a

Developmental language disability as a consequence of prenatal exposure to ethanol.

Two pre-school-aged patients with a history of prenatal exposure to ethanol had abnormal head size and developmental delay. Both children were strikingly similar in physical appearance, behavior, and cognitive dysfunction. Facial features were typical of fetal alcohol syndrome. Head circumference greater than 97th percentile without hydrocephalus and no evidence of prenatal or postnatal growth failure were unusual for ethanol teratogenicity. Each child had a similar pattern of verbal and behavioral dysfunctions characterized by (1) marked hypervigilence, (2) distractability, and (3) cognitive

Validity Data for the Developmental Profile-4 in Individuals With Profound Intellectual and Multiple Disabilities and Developmental Epileptic Encephalopathies.

Valid neurodevelopmental assessments for individuals with profound intellectual and multiple disabilities (PIMD) and/or developmental and epileptic encephalopathy (DEE) are critical for clinical trials and clinical care as families report functional abilities are a priority for improvement. However, most extant tools have limited ability to measure relevant skills and meaningful change for individuals with severe to profound impairment. We evaluated the psychometric properties of the Developmental Profile, Fourth Edition (DP-4), a measure of neurodevelopmental functioning, in individuals with
