Cardiac computed tomography (CT) has become indispensable in contemporary electrophysiology, providing rapid, non-invasive, high-resolution imaging of the heart with dramatically reduced radiation exposure compared to previous techniques. This review synthesizes the current and expanding roles of cardiac CT across a broad spectrum of electrophysiology procedures, focusing on the management of cardiac implantable electronic devices (CIEDs)-including transvenous lead extraction, leadless pacemakers, and extravascular implantable cardioverter-defibrillators-as well as atrial fibrillation therapie

Tin-lead (Sn-Pb) mixed perovskite solar cells (PSCs) are promising for high-efficiency tandem photovoltaics. However, substantial temperature fluctuations during device operation generate mechanical stresses that, coupled with insufficient adhesion at the perovskite/hole transport layer (HTL) interface, induce perovskite cracking and interfacial delamination. These structural degradations hinder efficient hole extraction and compromise long-term device stability. In this study, we introduce 1,2-ethylenediphosphonic acid (EDPA) as an interfacial molecular bridge to reconstruct the contact inter

Carbon electrodes (CEs) have emerged as promising electrodes for perovskite solar cells (PSCs) due to their effective hole extraction capability, excellent stability, and low cost, which are conducive to improving the device stability, simplifying the device structure, and reducing fabrication costs. However, their porous structure, limited conductivity, and interfacial energy level mismatch with perovskites lead to severe charge recombination. To address these issues, we designed β-cyclodextrin-grafted multiwalled carbon nanotubes (β-CD/MWCNTs) to modify a CE, in which β-CD can chelate Pb2+ i

Frozen leads are a rare but clinically relevant complication during cardiac device replacement. A 77-year-old man undergoing pacemaker upgrade had atrial and ventricular leads firmly adhered to the generator header despite set screw removal and ethanol application. As specialized extraction tools were unavailable, an orthopedic nipper and forceps were used as a locking mechanism to dismantle the header, successfully freeing the leads without damage. This is the first reported case in which fully exposed lead pins could not be extracted. While adhesion limited to the pin port may be addressed w

BACKGROUND: Optimal management of advisory implantable cardioverter defibrillator (ICD) leads has not been established. Several studies were reported concerning the lead extraction of advisory ICD leads, but the implant duration of those studies was short. We estimated the efficacy of lead extractions of advisory ICD leads with a relatively longer duration in Japanese patients.
METHODS: We retrospectively analyzed 28 patients who underwent a lead extraction at Kokura Memorial Hospital and Tokyo Medical and Dental University Hospital [Fidelis (Medtronic, Minneapolis, MN, USA): n=19, Riata (St.

BACKGROUND: Delayed lead perforation is a rare complication of cardiac implantable electronic device (CIED). Clinical presentations range from completely asymptomatic to pericardial tamponade. Surgical lead extraction is recommended and transvenous lead extraction (TLE) with surgical backup is an alternative method.
CASE PRESENTATION: A male with paroxysmal atrial fibrillation and sick sinus syndrome implanted a dual-chamber pacemaker with two passive fixation lead. He was on oral anticoagulants and played golf for almost 1 h every day after implantation. However, he complained of thoracic st

OBJECTIVES: The number of cardiac implantable electronic devices is growing worldwide because they play a relevant role in improving the survival rate in patients with specific heart diseases. Cardiac implantable electronic devices complications including infection, dysfunction or venous stenosis increase the need for the least traumatic way to explant leads. Our goal was to report the successes and procedural complication rates of transvenous lead extraction (TLE) in a consecutive series of patients.
METHODS: From 2010 to 2016, 108 patients underwent TLE of 227 leads due to endocarditis (n =

AIMS: Systemic cardiac implantable electronic device (CIED) infection is a serious complication, especially in patients with pacemaker (PM) dependency. In those patients after infectious device removal temporary pacing is necessary, to obtain adequate haemodynamics. In this study, a new therapy option with temporary pacing by ipsilateral implantation of a new active-fixation right ventricular (RV)-lead was evaluated.
METHODS AND RESULTS: Between September 2009 and November 2011, infectious lead removal was performed in 17 patients with systemic CIED infection. Temporary pacing was achieved by

BACKGROUND AND PURPOSE: Subcutaneous implantable cardioverter defibrillators (S-ICDs) have emerged in recent years as a valid alternative to traditional transvenous ICDs (TV-ICDs). Therefore, the number of S-ICD implantations is rising, leading to a consequent increase in S-ICD-related complications sometimes requiring complete device removal. Thus, the aim of this systematic review is to gather all the available literature on S-ICD lead extraction (SLE), with particular reference to the type of indication, techniques, complications and success rate.
METHODS: Studies were identified by search

Repeat implantable cardioverter defibrillator (ICD) procedures are increasing and may be associated with higher risks for complications. To provide more information for clinical decision making, especially in light of recent defibrillator advisories, we examined a large national cohort to characterize repeat ICD procedural outcomes. Using data from the National Cardiovascular Data Registry (ICD Registry), we compared patient characteristics, reasons for ICD implantation, and associated in-hospital adverse events among 92,751 patients receiving their first device and 81,748 patients who underwe

AIMS: Indications for cardiovascular implantable electronic devices continue to evolve, which has led to an increasing requirement for transvenous lead extraction. We explore the indications, complications, and success rates involved in the removal of pacemaker and implantable cardioverter-defibrillator (ICD) leads in a high-volume centre, over 20 years.
METHODS AND RESULTS: We performed a retrospective analysis of all consecutive patients undergoing transvenous lead extraction by a single operator at a single centre between 1993 and 2012. Patient characteristics, indications, and outcomes we

INTRODUCTION: We hypothesized that an accurate assessment of preoperative venography could be useful in predicting transvenous lead extraction (TLE) difficulty.
METHODS AND RESULTS: A dedicated preoperative venogram was performed in consecutive patients with cardiac implantable electronic device who underwent TLE. The level of stenosis was classified as without significant stenosis, moderate, severe, and occlusion. The presence of extensive lead-venous wall adherence (≥50 mm) was also assessed. A total of 105 patients (median age: 71 years; 72% male) with a median of 2 (1-2) leads to extract

BACKGROUND: The rate of cardiac implantable electronic device (CIED) implantation in low- and middle-income countries is increasing. Patients recieving these devices are frequently older and with multiple co-morbidities, which may later lead to complications requiring CIED removal. CIED removals are associated with life-threatening complications. However, high sucesss rates are reported in high-income countries. The purpose of this study was to report on the experience of CIED removal in a resource-constrained setting.

The current design of an innovative left ventricular assist device (LVAD) makes use of magnetic levitation technology, which enables the rotors of the device to be completely suspended by magnetic force, reducing friction and blood or plasma damage. However, this electromagnetic field can result in electromagnetic interference (EMI), which can interfere with proper functioning of another cardiac implantable electronic device (CIED) in its direct proximity. Approximately 80% of patients with an LVAD have a CIED, most frequently an implantable cardioverter-defibrillator (ICD). Several device-dev

BACKGROUND: Cardiac implantable electronic device (CIED)-related tricuspid regurgitation (TR) is increasingly recognized, necessitating heart team assessment. Transcatheter tricuspid valve replacement (TTVR) is emerging as an option for inoperable patients with TR, requiring decisions on right ventricular lead management.
CASE SUMMARY: A 76-year-old woman developed moderate to severe TR after pacemaker implantation, which was exacerbated by a second RV lead 8 years later, leading to torrential TR and recurrent heart failure hospitalizations. TR persisted despite lead extraction, and multimoda

BACKGROUND: Percutaneous transcatheter tricuspid valve replacement (TTVR) has emerged as an effective treatment for severe tricuspid regurgitation (TR). Management of TR in the setting of transvenous leads is complex, requiring a multidisciplinary team-based approach. Transvenous lead extraction (TLE) may improve or worsen lead-related TR and is important when considering TTVR to avoid "jailing" leads between the transcatheter and native tricuspid valve. Data and guidance in these settings are limited.
OBJECTIVE: This study aimed to describe the outcomes of a strategy using TLE in patients wi
