Using the experiences achieved by experimental studies on cauliflower ear, protruding ears have been operated on with a technique involving the neochondrogenic potential of the perichondrium. The underdeveloped or absent conchoscaphal angle is restored to a normal antihelix in a posterior approach by a perichondrioplasty, gently folding the cartilage. The mobilized perichondrium is used to generate new cartilage, molding the folded cartilage in the desired position. The method is described in detail and is a modification of the original technique, which, in a follow-up study, was found to give

BACKGROUND: Ear framework fabrication with autologous costal cartilage is considered to be one of the important factors for successful auricular reconstruction. Congenital microtia in adults has many particular physiological characteristics different from that in children. The authors report an effective ear framework-fabricating technique in congenital microtia in adults. Technical points regarding the reduction of the amount of costal cartilage harvested and the increase of the flexibility of the cartilage are discussed.
METHODS: One hundred three adult patients diagnosed with unilateral co

OBJECTIVE: Compare hearing benefit of incus preservation in primary cholesteatoma surgery versus cartilage-myringostapediopexy.
METHODS: Prospective cohort study in a tertiary referral center. Tympanoplasty utilizing cartilage or other grafts, with or without intact incus was performed in 195 ears (187 children) with intact stapes. Outcome measures were pre and post-operative four-tone air conduction (AC) threshold (0.5, 1, 2, 4 kHz) and proportion with normal hearing (AC ≤ 30 dB HL) at 12 months.
RESULTS: Ears with intact ossicles had better post-operative AC thresholds than those with incu

The aim of this article is to report the application of expanding retroauricular skin fascia flap, and autogenous costal cartilage for congenital microtia reconstruction. Microtia reconstruction was generally completed in 3 surgical stages. In the first surgical stage, a 50 or 80 mL kidney-shaped tissue expander was inserted subcutaneously in the retroauricular mastoid region. Inflation of saline volume increased up to 60 to 80 mL, and skin flap was expanded for 2 to 3 months postoperatively. In the second surgical stage, removal of tissue expander, formation of retroauricular skin flap, eleva

OBJECTIVE: To investigate a method for total reconstruction of auricle.
METHODS: 90 patients (101 ears) with congenital microtia underwent two-stage operations for auricular reconstruction. The first stage involved fabrication and grafting of autologous costal cartilage, removing the remnant ear cartilage, embedding the framework into local flap of the mastoid region, transferring the remnant ear lobule flap to link to the inferior framework. The second stage was creating an auriculocephalic sulcus. The reconstruction was performed 4 - 12 months after the first surgery. Skin incision was made

Relapsing polychondritis is a cartilage tissue disease characterized by inflammatory and destructive episodes. Elastic cartilage of the ear and nose, hyaline cartilage of the joints, hyaline cartilage of vertebra and trachea are the main involvement areas. Also organs having proteoglycan structure such as eyes, heart, blood vessels, and the inner ear can be affected. In this article otologic findings of a 54-year-old male patient with a diagnosis of relapsing polychondritis were presented and discussed in the light of the literature.

Four features, manifested in various combinations, characterise deformity in the burned ear: (i) the presence of scarred skin at the site of and surrounding the ear, with dramatic loss of skin elasticity; (ii) the presence of longitudinal scars of the pinna due to previous drainage of the perichondritis as an initial trial for saving the ear; (iii) absence of different components of the framework of the ear, mostly the helix/antihelix complex (the cartilage-containing part) with or without the ear lobule; (iv) scarred chest wall due to associated burns of the skin of the chest. Large full thickness helical ear defects are a challenge to reconstruct. A 25-year-old woman presented to us with loss of a large portion of the helical rim after an assault. A successful 2-stage reconstruction was performed, incorporating the principles of perforator flaps, otoplasty techniques, and use of diced cartilage more commonly used for rhinoplasty. There was a 5 × 0.5-cm central helical defect of the right ear. In the first stage, the original defect was recreated and a 7 × 1.5-cm inferiorly based postauricular artery perforator flap was raised from the right postauricular sulcus. Di

BACKGROUND: Articular cartilage has been the focus of multiple strategies to improve its regenerative/ repair capacity. The Murphy Roths Large (MRL/MpJ) "super-healer" mouse demonstrates an unusual enhanced regenerative capacity in many tissues and provides an opportunity to further study endogenous cartilage repair. The objective of this study was to test whether the super-healer phenotype could be transferred from MRL/MpJ to non-healer C57Bl/6 mice by allogeneic bone marrow transplant.
METHODOLOGY: The healing of 2mm ear punches and full thickness cartilage defects was measured 4 and 8 week

OBJECTIVE: To explore the fabrication and application of three-dimensional autogenous cartilage framework in auricular reconstruction.
METHODS: The process of fabrication of three-dimensional cartilage framework consisted of cartilage harvesting, carving and assembling the cartilage. The rib cartilage was harvested separately. The three-dimensional framework was composed of three main parts:the helix, the base and the pad, at different layer. The framework was fabricated according to the development of rib cartilage and contour and height of the reconstructed ear.
RESULTS: From January 1992

OBJECTIVES: Prominent ear abnormalities affect 5% of the population. A prospective, double-blind study of patients who underwent otoplasty procedures to correct these abnormalities was conducted to compare two surgical techniques. They diverge by the preservation or not of conchal cartilage.
METHODS: The two techniques were compared by measuring the cephalo-auricular and scapho-conchal angles. Measurements were performed in pre and 6-months post-operative periods using alginate molding. Twenty patients were randomly assigned to two groups (with and without cartilage preservation) with 10 part

OBJECTIVE: One of the greatest challenges in facial plastic surgery is total auricular reconstruction, as it demands precise technique as well as artistic creativity. The crucial step to reconstruct satisfactory auricles is sculpting a three-dimensional cartilaginous framework, which determines the success of surgery. In this paper we describe our new techniques in fabricating the three-dimensional auricular framework.
METHODS: To assemble satisfactory auricular frameworks, we designed four different methods according to the number of rib cartilages used. (1) Method I, the framework was made

BACKGROUND: An unusual complication of cutaneous surgery and its management is presented. Aspergillus flavus was identified from a nonhealing surgical wound of the ear undergoing cartilaginous necrosis in an immunocompetent host.
OBJECTIVE: We wish to alert clinicians that Aspergillus may infect surgical wounds of the ear causing significant morbidity.
METHOD: A healthy man underwent Mohs micrographic surgery for invasive Bowen's disease of the ear. Due to the size and location of the defect it was allowed to heal by secondary intent. The patient developed inflammation and subsequent destruc

The absence of ears in children is a global problem. An implant made of costal cartilage is the standard procedure for ear reconstruction; however, side effects such as pneumothorax, loss of thoracic cage shape, and respiratory complications have been documented. Three-dimensional (3D) printing allows the generation of biocompatible scaffolds that mimic the shape, mechanical strength, and architecture of the native extracellular matrix necessary to promote new elastic cartilage formation. We report the potential use of a 3D-bioprinted poly-ε-caprolactone (3D-PCL) auricle-shaped framework seede

Sculpting a tridimensional autologous rib cartilage framework is essential to restore a natural ear shape and becomes routine with preoperative training, but management of the skin is the key to minimizing complications. Here the authors provide a classification scheme to manage auricular skin: Type 1 is a Z-plasty with transposition of the lobule; type 2 is a transfixion incision of the microtic ear; type 3 exposes the cartilage remnants through a cutaneous incision. They also explain how to choose between the three types, depending upon the auricular skin potential. With training and method,

External canal reconstruction of extensive tissue defects, including auricle or ear canal defects, is challenging. We describe the case of a 28-year-old man who underwent staged skin and nasal septum cartilage grafting for ear canal reconstruction after resectioning a giant arteriovenous malformation of the auricle. We performed arteriovenous malformation resection and reconstruction of the significant tissue defect using an anterolateral femoral skin flap. Simultaneously, a Nelaton catheter stent was placed to temporarily form an external auditory canal. Subsequently, split-thickness skin gra
