Neurotologic considerations in the treatment of advanced clival tumors.

Head and neck manifestations of advanced clival tumors result from subtle, yet progressive mass effect or direct involvement of multiple cranial nerves. Misinterpretation of clinical or radiographic findings in these patients may result in inappropriate treatment planning, increased patient morbidity, and probable tumor recurrence. Our combined experience in managing 21 patients with advanced clival tumors has shown that preoperative loss of vision, diplopia, or facial hypesthesia suggests superior parasellar disease. Facial twitching or neurotologic symptoms result from posterior central tumo

Pediatric trans-oral submandibular gland excision: A safe and effective technique.

INTRODUCTION: In the pediatric population the submandibular gland requires removal in a number of conditions including, refractory recurrent sialoadenitis, sialolithiasis, salivary gland neoplasms and debilitating sialorrhea. In comparison to the traditional trans-cervical approach, the trans-oral route avoids a cervical scar, potential keloid formation and decreased risk of injury to the marginal mandibular branch of the facial nerve. This approach also eliminates the potentiality of remnant duct disease since the entire duct and papillae are removed. The article demonstrates the appropriaten

Cutaneous angiosarcoma clinically presenting as progressive solid facial edema in a 43-year-old male.

Cutaneous angiosarcoma of the head and neck is a rare, highly malignant neoplasm; prognosis is heavily influenced by tumor size, resectability, and stage at initial diagnosis. Most patients present with one to several erythematous to violaceous patches, plaques, or nodules. However, the clinical presentation is highly variable and leads to delayed diagnosis. We report cutaneous angiosarcoma in a 43-year-old man who presented with an 11-month history of progressive solid (non-pitting) edema involving his entire face, scalp, eyelids, and neck without characteristic clinical features of cutaneous

[Incidence of salivary gland malignancies in 12 year time period in ENT Department Medical University in Poznan].

Neoplasm of salivary gland constitutes about 3% of all tumours of head and neck. Within the category we can differentiate tumours of a very different histological structure. What lies behind such great differences in the changes within the salivary glands is complex embryogenesis of the glands. This work aims at the assessment of the frequency of occurrence of malignant neoplasm in parotid gland and submandibular gland based on the material collected at the ENT Department of the Medical University in Poznan in the years 1995-2006. In the 12-year period, 103 patients suffered from malignant neo

Challenges in the Cytodiagnosis of Intraparotid Schwannoma.

Clear cell tumors of parotid gland encompass a wide spectrum of neoplasms, including benign and malignant epithelial neoplasms. Additionally, tumors from adjacent structures such as paraganglioma, and metastatic neoplasms may also show clear cells. Overlapping cytological features may cause difficulty in diagnosis. This 50-year-old female presented with inability to close the right eye for 4 years, and with an infra-auricular swelling. Fine-needle aspiration cytology (FNAC) done outside was submitted for review with a diagnosis of neuroendocrine tumor. Smears showed clusters of tumor cells wit

Large congenital facial teratoma.

This case report discusses a presentation of a giant facial teratoma, a rare congenital neoplasm. Head and neck locations of the tumour uncommonly distort the face and may be associated with functional problems. We present a case of teratoma arising from the right parotid extending to the extracranial regions, successfully managed by surgical resection. Reviewing this case with the supporting body of literature anticipates further investigation to address patients' needs more thoroughly.

A finding of granuloma faciale associated with basal cell carcinoma.

The clinical differential diagnosis of violaceous plaques on the face is broad and includes both neoplasms and inflammatory dermatoses. We report the first case of a basal cell carcinoma (BCC) superficial to an underlying granuloma faciale (GF). This is a single case report with clinicopathological correlation. Histological examination of a skin biopsy from the cheek revealed superficial BCC and GF. This case constitutes the first reported finding of coincident GF and BCC.

Pseudolymphomatous Cutaneous Angiosarcoma Presenting With Persistent Firm Facial Edema in a Patient With Systemic Sclerosis.

Pseudolymphomatous cutaneous angiosarcoma (cAS) is a rare subtype characterized by a prominent lymphocytic infiltrate, posing diagnostic challenges due to its resemblance to lymphoid neoplastic processes. We present a novel case highlighting the clinical and histopathological features, notably its association with persistent firm facial edema in a patient with systemic sclerosis (SSc). A 47-year-old woman with a 21-year history of SSc presented with firm palpebral edema evolving to involve the entire face and cervical region over six months. Diagnostic imaging revealed inflammatory changes in

Evaluation of the Efficacy of the Combined Therapy of Botulinum Toxin and Hyaluronic Acid Compared to Conservative Intervention in the Treatment of Chronic Peripheral Facial Paralysis of Oncologic Etiology.

BACKGROUND: Neoplasms in the head and neck often lead to peripheral facial paralysis (PFP), impacting facial expression and causing psychological distress. Standard treatments involve physiotherapy, but botulinum toxin type A (BTA) and hyaluronic acid (HA) injections have shown promise in promoting facial symmetry. AIM: To assess the effectiveness of combined BTA and HA intervention in PFP patients, comparing it to conventional treatment (physiotherapy). METHOD: A single-blind, randomized clinical trial at Hospital de Amor de Barretos included 18 patients, with the control group receiving st

Ossifying Fibroma of Non-odontogenic Origin: A Fibro-osseous Lesion in the Craniofacial Skeleton to be (Re-)considered.

In the cranio-facial skeleton, a heterogeneous group of well characterized fibro-osseous lesions can be distinguished. Whereas fibrous dysplasia can affect any skeletal bone, ossifying fibroma and cemento-osseous dysplasia exclusively develop in the cranio-facial region, with most subtypes restricted to the tooth bearing areas of the jaws. Herein we present a series of 20 fibro-osseous lesions that developed mostly in the frontal bone and in the mandible, presenting as expansile intramedullary tumors with a unique histologic appearance and an indolent clinical course. We provide evidence that

Clinical features, evaluation, and management of ophthalmic complications of facial paralysis: A review.

The purpose of this review was to study the evaluation, diagnosis, and management of ophthalmic complications associated with facial nerve palsy and to discuss the current and future interventions. The ophthalmic complications of facial paralysis include lagophthalmos, ectropion, exposure keratopathy, ocular synkinesis, and crocodile tears. Evaluation by an ophthalmologist skilled in recognizing and managing complications of facial paralysis shortly after its initial diagnosis can help identify and prevent long-term complications. Several types of grading scales are used to evaluate, measure t

Giant cranial osteoma: successful staged excision of the largest reported.

Craniofacial osteomas are benign, slow-growing neoplasms of the craniofacial region that are usually asymptomatic. Uncommonly, giant craniofacial osteomas may be symptomatic and cause serious morbidity including ophthalmologic problems, cerebral compression, pneumocephalus, and seizures. We present a case of a 15-year-old Asian adolescent girl with a giant cranial osteoma (17.5 × 13.2 × 5 cm: significantly larger than previously reported). She also had multiple other synchronous giant osteomas of the face causing facial asymmetry. These osteomas were resected in a multiple-staged approach with

Parotid defects.

Parotidectomy is a widely performed procedure for various indications, including benign and malignant conditions. For malignant neoplasms of the parotid gland or metastatic disease, it may be performed in conjunction with cheek or temple skin resection, facial nerve sacrifice, or composite resection of a portion of the mandible. There are various options for reconstruction depending on the extent of resection. This article discusses various reconstructive options following parotidectomy and other ablative procedures that are often performed in conjunction. Reconstruction of parotidectomy defec

[Dubowitz syndrome with immunodeficiency and solid malignant tumor in two siblings (author's transl)].

The main features of Dubowiz syndrome are bird headed dwarfism and a typical facial configuration. Including two own patients 13 cases among 8 siblings are known in the literature. Two own cases of Dubowitz syndrome in two sisters are described, one of them with hypogammaglobulinemia and neuroblastoma, the other one with complete Ig A deficiency and malignant lymphoma. A propable relationship between immundeficiency and malignancies is discussed. Until 1973 151 malignant tumors with primary immundeficiency had been registrated. This paper is the first description of two cases of Dubowitz syndr

A Rare Presentation of a Bi-Maxillary Brown Tumour due to Secondary Hyperparathyroidism.

Brown tumours are localized bone lesions, seen in patients with high parathyroid hormone levels. This can be due to primary hyperparathyroidism, which occurs more often in neoplasms of the parathyroid gland or due to secondary hyperparathyroidism more often seen as a result of renal impairment. Facial involvement is rare, with most reports focusing on long and axial bones. However, the mandibular bone is often the only bone affected. Here, we report a rare case of a bi-maxillary attainment of brown tumour in a patient with secondary hyperparathyroidism due to chronic kidney disease.

Nevus sebaceus.

A 35-year-old woman presented with cosmetically disturbing facial plaques, which had been present since birth and were consistent with nevus sebaceus on histopathologic examination. Nevus sebaceus is a congenital, hairless plaque, which is found most commonly on the scalp and face and typically becomes thicker and more verrucous in appearance as the patient enters adulthood. There is an association with benign and malignant neoplasms. Typically, these lesions are either removed surgically or the patients are followed clinically. Other therapeutic modalities utilized for cosmesis include carbon

[Eyelid and cutaneous lesions as the sole indicators of tuberous sclerosis].

CASE REPORT: A 53-year old woman presented a tuberous sclerosis complex (Bourneville's disease) characterised by facial cutaneous angiofibromas, periungual fibromas and cutaneous leaf-shaped macules in the thorax. She also presented two neoplasms in the left lower eyelid which were histopathologically diagnosed as angiofibromas. There were no other ophthalmologic or systemic findings. DISCUSSION: Tuberous sclerosis complex is an autosomical dominant neurocutaneous phakomatosis in which up to 80% of cases represent new mutations, characterized by mental retardation, epilepsy and angiofibromas.

Metastatic basal cell carcinoma originating in the supra-parotid region.

This case illustrates that massive tumor size, ulceration, or history of multiple recurrences are not absolute prerequisites for metastatic BCC. It supports the hypothesis that tumor proximity to major lymphatic systems or large-caliber blood vessels may be of significance. BCCs originating in skin overlying the parotid gland have the potential to penetrate to a rich vascular and lymphatic plexus as well as compromise the integrity of the facial nerve. Early treatment of these neoplasms utilizing accurate methods of microscopic control appears warranted to prevent the serious consequences of r

Unicentric Castleman disease in intraparotid lymph node: a case report.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder characterized by non-neoplastic lymph node hyperplasia. Unicentric Castleman disease (UCD), the most common clinical form, typically presents as a solitary, asymptomatic mass. Involvement of intraparotid lymph nodes is rare and often mimics salivary gland neoplasms, complicating preoperative diagnosis. CASE PRESENTATION: A 35-year-old female presented with a painless, enlarging mass in the left parotid region. Contrast-enhanced CT revealed a well-demarcated, homogeneously enhancing mass with additional smaller lesions i
