The use of topical retinoids for inflammatory acne: a review of the pharmacoeconomic considerations.

Few pharmacoeconomic studies exist regarding the use of topical retinoids for inflammatory acne vulgaris. Those pharmacoeconomic studies that do exist predictably involve new entrants (topical tazarotene in various formulations). The use of topical retinoids in combination with antimicrobial agents constitutes cost-effective therapy for inflammatory acne. The growing U.S. trend toward use of isotretinoin for less-severe forms of acne instead of topical retinoid-based combination therapy is problematic from clinical and cost perspectives. More pharmacoeconomic studies that consider the importan

Childhood granulomatous periorificial dermatitis: lupus miliaris disseminatus faciei in children?

BACKGROUND: Childhood granulomatous periorificial dermatitis (CGPD) is a granulomatous disease characterized by monomorphous, small papular eruptions around the mouse, nose and eyes, and is thought to be closely related to perioral dermatitis. This condition has always been believed to occur more commonly in dark-skinned patients; however, recent observations have revealed CGPD to occur also in white patients. OBSERVATION: We report an 11-year-old Japanese boy with the characteristic features of CGPD. Although sarcoidosis and acne/granulomatous rosacea could be differentiated from CGPD, we co

Rethinking Lifetime Abuse in Old Age.

Research on abuse is often piecemeal and focused on specific forms, populations, and social situations. As a result, these studies tend to ignore the web of abuse resulting from the interactive effects of various forms of abuse over the life course, together referred to as "lifetime abuse." An examination of abuse from the point of view of older adults is likely to provide a lifetime perspective based on linking child abuse to elder abuse over the life course and an assessment of its consequences. Despite the growing awareness and resulting body of knowledge regarding this phenomenon, proposed

Safety evaluation of topical applications of ethanol on the skin and inside the oral cavity.

Ethanol is widely used in all kinds of products with direct exposure to the human skin (e.g. medicinal products like hand disinfectants in occupational settings, cosmetics like hairsprays or mouthwashes, pharmaceutical preparations, and many household products). Contradictory evidence about the safety of such topical applications of the alcohol can be found in the scientific literature, yet an up-to-date risk assessment of ethanol application on the skin and inside the oral cavity is currently lacking. The first and foremost concerns of topical ethanol applications for public health are its car

Prevalence of technology facilitated and other gender-based violence among adolescent girls in Gqeberha, South Africa and its association with probable common mental disorders.

BACKGROUND: Emerging evidence is raising alarms that technology facilitated gender-based violence (TF-GBV) is a growing public health concern with impacts on child wellbeing, yet little research on the topic has been conducted in middle-income country settings. This study aimed to investigate the prevalence of TF-GBV, other GBV, and their association with common mental disorder (CMD) symptoms among adolescent girls in South Africa. METHODS: Trained enumerators facilitated surveys on exposure to physical and sexual violence with adolescent girls aged 10-19 from 14 low-income primary and second

Ocular immunization of guinea pigs.

The experiments described were undertaken to determine which cells of guinea pigs immunized by different ocular routes produce the IgG1 and IgG2 antibodies detected in the serum. Guinea pigs were immunized intravitreally, topically, or by intravitreal immunization followed by topical conjunctival challenge. An indirect plaque assay was used to detect antibody producing cells in the cervical lymph nodes and ocular tissues. Passive hemagglutination, passive cutaneous anaphylaxis and ELISA assays were used to detect serum antibody. Both the topical and intravitreal methods initiated a primary ant

Epidermolysis bullosa dystrophica pretibialis - Clinical snapshot and management of a rare orphan disease.

If blistering occurs in childhood, the possibility of hereditary epidermolysis bullosa should be considered even if the symptoms are mild. Besides clinical and histological examination, molecular genetic screening is diagnostically relevant. For localized forms, symptomatic, topical therapy options are currently still the primary choice. Of particular interest is the new option of topical therapy with diacerein 1 % cream. In the case of a pronounced clinical picture with extracutaneous organ involvement, multidisciplinary management is required. In the future, new forms of therapy such as auto

[Oral lichen planus. Insight into the pathogenesis and therapeutic options].

Oral lichen planus is a common, chronic mucosal disease associated with T-cell-mediated immunological dysfunction. The disease is classified in a painful red form with erosions and ulcerations and a less painful to painless white form with reticular, papular and plaque lesions. In a small proportion of cases, the lesions may develop into a squamous cell carcinoma. Corticosteroids are the mainstay of the therapy; the combination of systemic and topical corticosteroids is often very effective. Topical calcineurin inhibitors, especially tacrolimus, have produced long-term responses in clinical st

Use of Topical Corticosteroids in the Treatment of Noninfectious Inflammatory Dermatoses of the Scalp: A Survey of Practicing Dermatologists and Dermatology Residents Using Delphi Methodology.

BACKGROUND: Noninfectious inflammatory dermatoses of the scalp are common, and their symptomsin particular, those affecting appearance, can have a psychological effect that may be disproportionate to their clinical severity. Effective, cosmetically acceptable treatments are important to manage these conditions. Topical corticosteroids form the cornerstone of acute treatment for many of these conditions. We surveyed practicing dermatologists and dermatology residents to determine the current clinical practice in prescribing topical corticosteroids for these disorders in their various preparatio

Treatment of rosacea-like demodicidosis with oral ivermectin and topical permethrin cream.

A 32-year-old man presented with a chronic rosacea-like dermatitis of the facial skin and the eyelids. The skin disorder had been present for 4 years and was unresponsive to multiple previous treatment attempts. Skin scrapings and a histologic examination of a biopsy specimen from the affected area revealed the presence of numerous Demodex mites. The patient was treated with oral ivermectin and subsequent topical permethrin resulting in complete and rapid clearing of the folliculitis. We believe that this case supports the view that Demodex mites may be pathogenic when they are present in larg

Shortcomings of protocols of drug trials in relation to sponsorship as identified by Research Ethics Committees: analysis of comments raised during ethical review.

BACKGROUND: Submission of study protocols to research ethics committees (RECs) constitutes one of the earliest stages at which planned trials are documented in detail. Previous studies have investigated the amendments requested from researchers by RECs, but the type of issues raised during REC review have not been compared by sponsor type. The objective of this study was to identify recurring shortcomings in protocols of drug trials based on REC comments and to assess whether these were more common among industry-sponsored or non-industry trials. METHODS: Retrospective analysis of 226 protoco

Topical mechlorethamine for psoriasis. An attempt to avoid the development of sensitization by the use of a topical tolerogenic schedule.

The topical application of mechlorethamine is associated with a high incidence of irritant contact dermatitis. An attempt was made to avoid this complication by the use of a topical tolerogenic schedule. Seven of ten patients developed contact dermatitis to the medication after they had completed the schedule, The use of this medication in its present form is unlikely to be practical value in the treatment of psoriasis.
