Complications after vertical gastroplasty with artificial pseudopylorus in the treatment of morbid obesity: a 7-year experience.

BACKGROUND: Vertical gastroplasty with artificial pseudopylorus (VGAP) was designed to reduce the complication rate of other forms of gastroplasty. The purpose of this study was to analyze the complications of this approach for 7 years of surgical practice. METHODS: A total of 156 morbidly obese patients (BMI 41-81 kg/m2) who underwent VGAP were analyzed retrospectively in terms of early and late complications and gastroplasty failure. RESULTS: The early complication rate, operative and systemic, was 6.40% (10 patients) and the late complication rate was 12.16% (19 patients). Early complicat

The place of upper gastrointestinal tract endoscopy before and after vertical banded gastroplasty for morbid obesity.

In industrialized countries, surgical gastroplasty is performed more and more frequently in patients with morbid obesity. The aims of this prospective study were to determine the incidence of upper gastrointestinal lesions in obese patients and to assess the place of digestive endoscopy in symptomatic patients after gastroplasty. A consecutive group of 159 obese patients were studied before and after vertical banded gastroplasty. In the preoperative evaluation, reflux esophagitis and gastroduodenal lesions were endoscopically observed in 31% and 37% of the patients, respectively. Interestingly

Gastroplasty with distal gastric bypass: a new and more successful weight loss operation for the morbidly obese.

Because of the unacceptable late failure rates associated with other surgical procedures for morbid obesity, the author has performed a new operation-vertical banded gastroplasty with distal gastric bypass-on 70 patients. In seven of these the gastroplasty stoma had to be debanded. The mean lowest loss of excess weight was 88% and the mean final loss of excess weight was 83% for the main group. The postoperative complication rate was acceptable and no late metabolic complications have been seen. This operation gives improved weight loss when compared with gastroplasty or gastric bypass alone a

Laparoscopic conversion of vertical banded gastroplasty with an antireflux wrap into Roux-en-Y gastric bypass.

BACKGROUND: Vertical banded gastroplasty (VBG) is associated with a significant rate of revision because of regain of weight due to staple-line disruption, gastric pouch and stoma dilation, change to sweet eating, outlet stenosis with vomiting and reflux. To avoid reflux, some surgeons added an antireflux wrap. METHODS: We report laparoscopic revision of VBG with antireflux wrap to Roux-en-Y gastric bypass (RYGBP) in 4 patients. The indication for revision was insufficient weight loss in all 4 patients, with stenosis of the stoma resistant to endoscopic balloon dilation in one and reflux esop

Long-term follow-up on the effect of silastic ring vertical gastroplasty on weight and co-morbidities.

BACKGROUND: Silastic ring vertical gastroplasty (SRVG) is a simple, effective and reproducible restrictive operation for the treatment of morbid obesity. Over the last years, it has lost its popularity due to the development of adjustable gastric banding systems performed laparoscopically. In order to evaluate the long-term effect of SRVG on weight loss and co-morbidities, we reviewed the results of SRVG operations in our institution. METHODS: We reviewed SRVG operations performed in our University-affiliated General Hospital. Data was collected from the patients' in-patient records, their ou

The influence of Helicobacter pylori infection and corpus gastritis on the postoperative outcomes of laparoscopic vertical banded gastroplasty.

BACKGROUND: Helicobacter pylori (HP) infection is linked to weight control through gastric inflammation-induced deregulation of satiety-related hormone, and eradication of HP before a weight reduction operation has been advocated. We aimed to examine the impact of HP infection and corpus gastritis on preoperative patient characteristics, postoperative complications and weight loss following laparoscopic vertical banded gastroplasty (LVBG). METHODS: A prospective cohort of 152 patients undergoing LVBG was enrolled. Gastric specimens at the corpus were obtained during LVBG operation and scored

[Billiopancreatic diversion with preservation of the duodenal bulb and transitory gastroplasty in the treatment of morbid obesity. Our experience].

We present our experience in malabsorbitive procedure in bariatric surgery based on Biliopancreatic Diversion (BPD) with transitory gastroplasty. Since 1995 we operated on 74 patients with BPD coupled with gastroplasty which is transitory due to the presence of a band in polidioxanone (PDS). The technique, proposed by Vassallo et al. in 1992, involve the respect of the duodenal bulb (5 centimeter from the pylorous) making an end-to-side duodeno-ileal isoperistaltic anastomosis. The initial excess weight loss was satisfactory (69.8% +/- 11.4% after 1 year) and it kept being stable during all th

Wedge gastroplasty and reinforced crural repair: important components of laparoscopic giant or recurrent hiatal hernia repair.

OBJECTIVE: Laparoscopic repair of a giant hiatal hernia (>50% of the stomach above the diaphragm) is associated with short-term recurrence rates of 12% to 42%. Recurrent hiatal hernias often have significantly altered anatomy, making laparoscopic repair challenging. We hypothesized that increasing intra-abdominal esophageal length by means of Collis wedge gastroplasty, complete fat-pad dissection, hernia-sac excision, and primary reinforced crural repair would minimize short-term recurrence and provide adequate symptomatic relief. METHODS: From January 1, 2001, though May 1, 2005, 61 patients

Persistent Abdominal Pain After Endoscopic Sleeve Gastroplasty: A Multicenter Case Series and Hypothesis-Generating Management Framework.

INTRODUCTION: Endoscopic sleeve gastroplasty (ESG) is an established minimally invasive treatment for patients with obesity, with a favorable safety profile. Although transient abdominal pain is common in the early post-ESG period, persistent pain remains poorly characterized. We aimed to describe the clinical presentation, diagnostic evaluation, management strategies, and outcomes of patients with persistent abdominal pain after ESG, and to propose a pragmatic management pathway. METHODS: We conducted a retrospective multicenter case series across six expert bariatric endoscopy centers. Pati

Endoscopic Sleeve Gastroplasty After Solid Organ Transplantation: First Reported Case Worldwide in a Kidney Transplant Recipient.

Obesity adversely affects graft and cardiovascular outcomes in kidney transplant recipients, with limited effective therapies. Endoscopic sleeve gastroplasty (ESG) is established for obesity but not well studied post-transplant. We report ESG in a 33-year-old kidney transplant recipient with class III obesity. A structured search (PubMed, Embase, Scopus; January 2025) found no prior cases. ESG was completed without complications. At 6 months, meaningful weight loss and metabolic improvement occurred, with stable graft function. Semen parameters improved, without proven causality. ESG appears f

Feasibility of a fully endoscopic one-anastomosis gastric bypass procedure after endoscopic sleeve gastroplasty in pigs.

BACKGROUND AND AIMS: Endoscopic sleeve gastroplasty (ESG) has become an adopted endoscopic gastric volume-reduction procedure, whereas endoscopic gastric bypass is a recent experimental procedure. We assessed the feasibility and safety of combining ESG with a recently developed fully endoscopic one-anastomosis gastric bypass (OAGB) procedure. METHODS: This was a 14-week prospective follow-up of 4 growing pigs using a dedicated gastrojejunal lumen-apposing metal stent (GJ-LAMS) to create a gastrojejunal anastomosis and a duodenal exclusion device (DED). ESG at baseline was followed by the OAGB

Implementation of an Enhanced Recovery After Surgical Endoscopy protocol to improve outcomes following endoscopic sleeve gastroplasty.

BACKGROUND AND AIMS: An endoscopic sleeve gastroplasty (ESG) is a minimally invasive, transoral approach to weight loss surgery. One in 5 patients who have undergone ESG reportedly return to the emergency department (ED) within 30 days, with the majority (70%) presenting with postoperative nausea and vomiting (PONV). Implementing an Enhanced Recovery After Surgery (ERAS) protocol has been shown to reduce the incidence of PONV and lower the risk of readmission. The aim of this study was to develop and implement an ERAS protocol for adult patients undergoing ESG to prevent PONV and reduce readmi

Peroral endoscopic myotomy (POEM) for achalasia developing after vertical banded gastroplasty with asymptomatic gastro-gastric fistula.

Video 1At preoperative esophagram, a typical bird's beak image is shown at the gastroesophageal junction. A gastro-gastric fistula, opening from cardia to fundus, is also shown. A scope fitted with a distal clear cap is introduced. At the cardia, we see the proximal opening of the fistula. Here, we see the gastric fundus. As we go down, the gastric pouch is regular, and further down we reach the pylorus. In the retroflexed view, we recognize the neo-pylorus and the distal opening of the fistula. After submucosal injection on the anterior wall of the esophagus, a longitudinal mucosal incision i

Gastroesophageal reflux after intact vertical banded gastroplasty: correction by conversion to Roux-en-Y gastric bypass.

Symptomatic gastroesophageal reflux disease is common in our experience after vertical banded gastroplasty. Our aim was to determine the safety and efficacy of Roux-en-Y gastric bypass in the treatment of symptomatic gastroesophageal reflux disease complicating vertical banded gastroplasty. We evaluated prospectively collected data on 25 patients who underwent revisional bariatric surgery because of severe gastroesophageal reflux disease after vertical banded gastroplasty. Only 4 of 25 patients had gastroesophageal reflux disease symptoms prior to vertical banded gastroplasty. Endoscopic findi

Endoscopic sleeve gastroplasty video annotation: analysis of the learning curve of three annotators to achieve procedure proficiency.

INTRODUCTION: Endoscopic interventional skills acquisition generally relies on apprenticeship-based training. Limitations of this learning model include the availability of trainers and human-factor biases. Increased routine surgical and endoscopic video recording and storage promote video-based learning (VBL) methods. VBL could shorten the learning curve by increasing trainer availability and democratizing training access for procedures such as endoscopic sleeve gastroplasty (ESG), which is carried out by a few expert operators. The aim of this work was to evaluate video annotation as a learn

Intragastric Nonabsorbable Sutures From Prior Endoscopic Sleeve Gastroplasty Discovered During Robotic Hiatal Hernia Repair: A Case Report.

Endoscopic sleeve gastroplasty (ESG) has emerged as a minimally invasive bariatric procedure that utilizes endoscopic suturing to reduce gastric volume. While generally considered safe, delayed complications related to retained suture material are incompletely characterized in the literature. We report a case of intragastric nonabsorbable sutures discovered during revisional hiatal hernia surgery in a patient with a remote history of prior foregut intervention. A 58-year-old woman with a history of hiatal hernia repair with fundoplication approximately 10 years prior presented for surgical eva

Impact of gastroplasty on salivary characteristics, dental health status and oral sensory aspects: A controlled clinical study.

BACKGROUND: Saliva is a non-invasive source of biomarkers useful in the study of different pathophysiological conditions. The qualitative and quantitative study of saliva, as well as the assessment of oral health, can be particularly useful for a better understanding of obesity due to its importance in the food oral perception and ingestion. OBJECTIVES: To evaluate the effect of treatment of morbid obesity (dietary counselling versus gastroplasty) on salivary characteristics, oral sensory perception and dental health in a controlled study. METHODS: Seventy-three adults (60 females; 19-59 yea
