Urban-rural and racial/ethnic disparities in modes of preconception cannabis use among insured patients receiving prenatal care at an integrated healthcare system in Northern California from 2020 to 2023.

OBJECTIVES: To examine joint associations between race/ethnicity and urbanicity in any cannabis use and modes of cannabis use during preconception. METHODS: In this cross-sectional study, insured pregnant patients (N = 146,166) were screened for cannabis use at prenatal care entry between February, 2020 and September, 2023 at a large integrated healthcare system in Northern California and answered questions about preconception cannabis use (any cannabis use and modes of use [smoking, blunts, edibles, vaping, dabs and topicals]). Multivariable Poisson regression models examined associations be

Dissecting racial disparities in multiple myeloma.

Multiple myeloma (MM) is a fatal plasma cell dyscrasia with a median overall survival of 5 to 10 years. MM progresses from the more common but often subclinical precursor states of monoclonal gammopathy of undetermined significance (MGUS), and smoldering multiple myeloma (SMM) to overt MM. There are large racial disparities in all stages of the disease. Compared with Whites, Blacks have an increased MGUS and MM risk and higher mortality rate, and have not experienced the same survival gains over time. The roots of this disparity are likely multifactorial in nature. Comparisons of Black and Whi

Disparities in Headache Care and Research in Children and Adolescents: A Narrative Review.

PURPOSE OF REVIEW: Headache disorders are highly prevalent in children and adolescents and represent a significant source of disability during developmental years. Despite this, disparities in pediatric headache care and research remain insufficiently characterized. This narrative review synthesizes current evidence on inequities across epidemiology, diagnosis, treatment access, and research participation. RECENT FINDINGS: Migraine and tension-type headache contribute substantially to years lived with disability in the pediatric population, with rising absolute burden over time. Important gaps

Exploring what works, for whom, under what circumstances to transform systems: realist synthesis protocol of four ongoing studies and literature addressing health inequalities.

INTRODUCTION: Health inequalities remain resistant to interventions that primarily target individual behaviour. Although systems approaches are increasingly promoted, their application in practice is often not well grounded in real-world settings. In this protocol paper, we present the approach we will take in an overarching project that synthesises the combined insights of four ongoing systems-based research projects on system-based approaches for reducing health inequalities in the Netherlands. By bringing together and comparing findings across diverse contexts, populations and interventions

Understanding inequities in the malaria landscape of Madagascar: a scoping review of current evidence.

BACKGROUND: Malaria remains a significant public health challenge in Madagascar, affecting vulnerable populations including children under five and pregnant women. Despite global progress in reducing malaria cases and deaths, Madagascar continues to experience a high burden due to inequities in access to prevention, diagnosis, and treatment. METHODS: A comprehensive literature review was conducted using major academic databases including PubMed, Web of Science, and Google Scholar. A comprehensive search of literature was conducted in PubMed, Web of Science, Google Scholar, and EBSCOhost. The

Disparities in Pancreatic Oncology Care.

This article presents an overview of disparities in pancreatic cancer care using previous collated reviews and gathered evidence from national databases, prospective cohort studies, and genetic analyses to examine how these disparities manifest throughout the cancer care continuum. The article explores how both traditional multivariate and novel analytical frameworks are being used to characterize disparities at patient, provider, and health care system levels, concluding with evidence-based interventions to address these persistent inequities in clinical practice.

Roll Back Stroke: The Way Forward for Physicians and Patients.

The 6-lettered word "Stroke" is an ancient disease, likened Biblically to "666" in Revelation 13 verse 18. Globally, it is the commonest cause of acquired long-term disability. While its incidence has been declining in high-income countries, low/middle-income countries (LMICs) have witnessed an increase. This increase is driven by epidemiologic-transition/modernization-complex, healthcare disparities, and increasing prevalence of vascular risk-factors. Despite the disproportionate burden of stroke in LMICs, these countries have the lowest rate of access to acute reperfusion-interventions leadi

Socioeconomic disparities in vestibular schwannoma diagnosis and management: A systematic review.

Disparities in neurosurgical care are increasingly recognized, yet how they affect the diagnosis and management of vestibular schwannoma (VS) remains unclear. Differences in tumor size at diagnosis, treatment selection, and outcomes may be influenced by race, insurance status, income, and geography. To date, no comprehensive synthesis has addressed these inequities in patients with sporadic VS. To systematically evaluate the literature for evidence of social determinants of health (SDoH) in the diagnosis, treatment, and outcomes of patients with sporadic vestibular schwannoma. We conducted a P

New Developments in Hodgkin's Lymphoma: Outcomes and Disparities in Classic Hodgkin Lymphoma.

Despite continued advances, survival among patients with Hodgkin lymphoma differs by age, race and ethnicity, socioeconomic status, insurance, and treatment location. Inequities are evident across the cancer care continuum, from timely diagnosis and treatment initiation and clinical trial access, to posttreatment survivorship care. As the therapeutic landscape for Hodgkin lymphoma evolves, studies examining the impact of new treatments on disparities are needed to ensure that treatment advances benefit all populations equitably.

From Tragedy To Opportunity: Hospital-based Violence Intervention Programs May Address Root-Cause Health Disparities for Violent Traumatic Injury Patients.

Violence is a growing public health issue that disproportionately affects low-income and racialized communities across North America. While trauma centers appropriately respond to acute violent injuries, many patients are discharged back into the same environments that put them at risk. Hospital-based Violence Intervention Programs (HVIPs) offer opportunities for trauma care to address the upstream root causes of violence. These programs engage patients during critical "teachable moments," connecting them to staff with lived experience, along with social workers and community partners, providi

Health financing inequality in fragmented health systems: evidence from Mexico, 2000-2023.

BACKGROUND: Universal health coverage (UHC) depends, among other things, on fair and efficient public financing. In many low- and middle-income countries (LMICs), fragmented health systems entrench inequalities by linking entitlements to employment status. In Mexico, persistent segmentation and institutional fragmentation have shaped the structure and distribution of public health financing, exemplifying this challenge through a dual public system that separates the population with social security from those without. While several policy reforms have sought to improve coverage and financial pr

When is health adequate for older adults? Optimal standards, patient-defined thresholds, and the weight of inequality.

This perspective distinguishes two standards that are often used as if they were interchangeable. Optimal health is a standard built by medicine and expressed through guideline targets and biomarkers. Adequate health is the threshold of functioning that allows a person to live in a way they themselves experience as sufficient and dignified. Building on the concept of homeodynamic space as the biological substrate of adequate independence, on the older salutogenic tradition that asked what sustains health rather than only what causes disease, and on clinical evidence that aligning care with pat

Understanding Inequalities in Mobile Health Utilization Across Phases: Systematic Review and Meta-Analysis.

BACKGROUND: Mobile health (mHealth) holds promise for enhancing patient care, yet attrition in its use remains a major barrier. Low retention rates limit its potential impact, while barriers to accessing or adopting mHealth vary across populations and countries. These differences in utilization of mHealth may exacerbate health inequalities, contributing to the digital health divide. OBJECTIVE: We aimed to conduct a systematic review and meta-analysis to investigate the factors associated with inequalities in mHealth utilization across different implementation phases, including access, adoptio

Disparities in Diagnosis of and Management of Narcolepsy and Idiopathic Hypersomnia.

This article examines disparities in the diagnosis, treatment, and lived experience of narcolepsy and idiopathic hypersomnia (IH). It highlights prolonged diagnostic delays, especially in women, children, and marginalized groups, compounded by limited access to essential sleep tests and insurance-related barriers. Treatment inequities stem from high medication costs, restrictive coverage, and a lack of global drug availability, leaving many patients with persistent symptoms. IH poses unique challenges due to ambiguous diagnostic criteria and poor test reliability. Both disorders impose severe
