Family caregivers' views on coordination of care in Huntington's disease: a qualitative study.

BACKGROUND: Collaboration between family caregivers and health professionals in specialised hospitals or community-based primary healthcare systems can be challenging. During the course of severe chronic disease, several health professionals might be involved at a given time, and the patient's illness may be unpredictable or not well understood by some of those involved in the treatment and care. AIM: The aim of this study was to explore the experiences and expectations of family caregivers for persons with Huntington's disease concerning collaboration with healthcare professionals. METHODS:

Co-designing approaches to enhance falls management after spinal cord injury in an Australian spinal injuries unit.

INTRODUCTION: Persons with spinal cord injury (SCI) are at risk of falls because of changes in sensation and motor function. While research exists on the consumer and clinician perspective of falls and fall prevention, these groups have not been brought together to determine how to effectively address this issue. The study aimed to co-design approaches to enhance falls management including prevention of falls and post fall management, after SCI in inpatient spinal rehabilitation. METHODS: Using an experience-based co-design process, six consumers and eight clinicians, including occupational t

Setting up a nurse-led model of care for management of hypertension and diabetes mellitus in a high HIV prevalence context in rural Zimbabwe: a descriptive study.

BACKGROUND: In the light of the increasing burden of non-communicable diseases (NCDs) on health systems in low- and middle-income countries, particularly in Sub-Saharan Africa, context-adapted, cost-effective service delivery models are now required as a matter of urgency. We describe the experience of setting up and organising a nurse-led Diabetes Mellitus (DM) and Hypertension (HTN) model of care in rural Zimbabwe, a low-income country with unique socio-economic challenges and a dual disease burden of HIV and NCDs. METHODS: Mirroring the HIV experience, we designed a conceptual framework wi

Embedded Psychological Services and Lower 60-Day Acute-Care Utilization in High-Need, High-Cost Adults: A Target Trial Emulation.

In safety-net care management, high-need, high-cost (HNHC) adults often receive bundled services. The independent contribution of embedded psychological-services engagement is unclear. This retrospective cohort study used target trial emulation under the ICH E9(R1) estimand framework to evaluate an opt-out embedded psychological-services pathway nested within a manualized social-determinants-of-health-focused care-management program at four Phoenix Federally Qualified Health Centers. Eligible HNHC adults had ≥ 1 inpatient admission or ≥ 2 emergency department visits in the prior 12 months. Eac

Experiences of adults with intellectual disabilities accessing acute hospital services: A systematic review of the international evidence.

Adults with intellectual disabilities experience significant physical and mental health needs when compared to their typically developing peers. Previous research evidences that many people with intellectual disabilities have negative encounters within acute hospitals. The aim of this systematic review was to identify the specific views and experiences of adults with intellectual disabilities when accessing acute hospital services arising from the available literature. The review commenced in June 2019 and was updated in May 2020. A systematic search of five electronic databases including CINA

Policy frameworks for promoting public hospital development in major economies: implications for China.

BACKGROUND: As the core providers of medical and health services, public hospitals play an irreplaceable role in ensuring basic medical care, safeguarding health equity, and fulfilling social responsibilities. Their operational efficiency and institutional design not only influence the national health status but also relate to the stability and development of the entire healthcare system. Therefore, continuously optimizing support policies and management mechanisms for public hospitals has become a crucial issue for governments worldwide in advancing healthcare system reforms. METHODS: This p

Patterns and characteristics of 'calls of despair': a service evaluation using Yorkshire Ambulance Service data.

INTRODUCTION: 'Deaths of despair' (DoD) - encompassing fatalities from suicide, drug overdoses and alcohol-related causes - represent a growing public health crisis. Socioeconomic vulnerability and healthcare disparities are well-documented drivers of DoD. While healthcare contacts preceding despair-related deaths have been studied extensively, the role of ambulance services is underexplored. This study aimed to address this gap by utilising ambulance service data to provide insights into 'calls of despair' received by a UK ambulance service over a 12-month period. METHODS: This exploratory,

The Recovering Together Initiative: Integrating Psychosocial Care into ICUs.

More than 5 million patients are admitted to ICUs each year in the United States alone. ICUs are stressful environments given the patients' medical severity, family emotional experience, and staff burnout. However, psychosocial services are rarely offered and sustained in these settings. Multidisciplinary partnerships and innovative frameworks and services are needed to successfully integrate psychosocial care into these complex settings. To address this need, Massachusetts General Hospital developed the Recovering Together (RT) Initiative (2015-present) in its Neurosciences ICUs (Neuro-ICU).

Availability of equipment and medications for non-communicable diseases and injuries at public first-referral level hospitals: a cross-sectional analysis of service provision assessments in eight low-income countries.

CONTEXT AND OBJECTIVES: Non-communicable diseases and injuries (NCDIs) comprise a large share of mortality and morbidity in low-income countries (LICs), many of which occur earlier in life and with greater severity than in higher income settings. Our objective was to assess availability of essential equipment and medications required for a broad range of acute and chronic NCDI conditions. DESIGN: Secondary analysis of existing cross-sectional survey data. SETTING: We used data from Service Provision Assessment surveys in Bangladesh, the Democratic Republic of the Congo, Ethiopia, Haiti, Mala

Organizing contemporary oncology care together - A scoping review on multi-hospital oncology networks.

INTRODUCTION: In oncology, hospitals increasingly collaborate in multi-hospital networks to deliver high-quality care. However, it is unclear how these networks should be organized and whether they are effective. METHODS: Following PRISMA-ScR guidelines, this scoping review included peer-reviewed studies on multi-hospital oncology networks of three or more autonomous hospitals collaborating to deliver services and improve quality of care in high income economies, published in English. RESULTS: The search yielded 4.013 hits, of which 47 articles were included. Results revealed that the resear

Procedural Portfolio Planning in Plastic Surgery, Part 2: Collaboration Between Surgeons and Hospital Administrators to Develop a Funds Flow Model for Procedures Performed at an Academic Medical Center.

INTRODUCTION: Although plastic surgeons make important contributions to the clinical, educational, and research missions of academic medical centers (AMCs), determining the financial value of a plastic surgery service can be difficult, due to complex cost accounting systems. We analyzed the financial impact of plastic surgery on an AMC, by examining the contribution margins and operating income of surgical procedures. METHODS: We collaborated with hospital administrators to implement 3 types of strategic changes: (1) growth of areas with high contribution margin, (2) curtailment of high-risk

Structure and Distribution of Health Care Costs across Age Groups of Patients with Multimorbidity in Lithuania.

BackgroundPatients with multimorbidity account for ever-increasing healthcare resource usage and are often summarised as big spenders. Comprehensive analysis of health care resource usage in different age groups in patients with at least two non-communicable diseases is still scarce, limiting the quality of health care management decisions, which are often backed by limited, small-scale database analysis. The health care system in Lithuania is based on mandatory social health insurance and is covered by the National Health Insurance Fund. Based on a national Health Insurance database. The stud
