Immunoglobulin G4-related kidney disease involving the renal pelvis and perirenal fat: A case report.

BACKGROUND: Immunoglobulin (Ig) G4-related disease (IgG4-RD) is an autoimmune disease associated with chronic and progressive inflammation and fibrosis. It is difficult to differentiate IgG4-RD involving the kidney from infectious diseases and malignancy on imaging. CASE SUMMARY: We report the case of a 51-year-old Chinese man whose abdominal computed tomography scan showed diffuse bilateral enlargement of the kidneys and perirenal fat, thickening of the renal pelvic walls, and hydronephrosis of the right kidney. Relevant laboratory test results showed a serum creatinine level of 464 μmol/L.

The pelvis-kidney junction contains HCN3, a hyperpolarization-activated cation channel that triggers ureter peristalsis.

Peristaltic waves of the ureteric smooth muscles move urine down from the kidney, a process that is commonly defective in congenital diseases. To study the mechanisms that control the initiation and direction of contractions, we used video microscopy and optical mapping techniques and found that electrical and contractile waves began in a region where the renal pelvis joined the connective tissue core of the kidney. Separation of this pelvis-kidney junction from more distal urinary tract segments prevented downstream peristalsis, indicating that it housed the trigger for peristalsis. Moreover,

Socioeconomic inequalities in kidney and renal pelvis cancer mortality in Canada: Trends over three decades.

BACKGROUND: Kidney and renal pelvis cancer (KCa) presents significant health challenges that require investigation. This study measured and examined trends in socioeconomic inequalities in the mortality of KCa in Canada over the period 1990-2019. METHODS: We constructed a census division level dataset pooled from the Canadian Vital Death Statistics Database (CVSD), the Canadian Census of the Population (CCP), and the National Household Survey (NHS) to measure income and education inequalities in the mortality rate of KCa in Canada over the study period. The age-standardized Concentration inde

IgG4-related kidney disease from the renal pelvis that mimicked urothelial carcinoma: a case report.

BACKGROUND: IgG4-related kidney disease is a comprehensive term for renal lesions associated with IgG4-related disease, which mainly manifests as plasma cell-rich tubulointerstitial nephritis with increased IgG4+ plasma cells and fibrosis. IgG4-related kidney disease in the renal pelvis is rare. CASE PRESENTATION: We describe a 53-year-old Asian woman who was referred to our hospital with a space-occupying renal lesion discovered by medical examination. A physical examination and laboratory evaluation revealed no significant abnormalities. Computed tomography scans showed a soft-tissue mass w

Ovulatory dysuria: a bizarre presentation of crossed non-fused ectopic kidney with extra renal pelvis.

Ectopic kidneys are a unique form of congenital anomaly that in most cases remain asymptomatic. However, depending on the location and the associated anatomical variations, they can mimic certain more common conditions, posing a challenge in differential diagnosis. They could cause secondary symptoms by virtue of their location or could falsely mimic a primary urologic problem when the etiology might be from a closely opposed adjacent structure. We present a unique case of ovulatory dysuria in a young female with a crossed non-fused ectopic kidney. The patient's symptoms were recurrent and cor

Nephron-sparing surgery for primary mucinous adenocarcinoma of renal pelvis in horseshoe kidney: A case report.

RATIONALE: Mucinous adenocarcinoma of the renal pelvis is extremely rare in malignancy of the renal pelvis, and <100 cases have been reported so far. However, horseshoe kidneys appear to be more prone to this mucinous gland metaplasia. Due to the lack of a specific diagnostic basis, it is difficult to confirm the diagnosis before surgery, which is easy to cause misdiagnosis. PATIENT CONCERNS: We report a 64-year-old male patient who found a cystic mass in the left kidney during physical examination. CT examination showed a horseshoe kidney with a cystic mass 9.5 × 8.0 cm in front of the left

Characterization of renal pelvis compliance and outflow resistance during ureteroscopy of the ex-vivo human kidney.

INTRODUCTION: Elevated intrarenal pressure during ureteroscopy has been linked to complications during recovery from stone removal surgery. Intrarenal pressure depends on irrigation flow rate, inflow and outflow resistance, and kidney compliance. These have been examined for porcine models in the literature. In this study, we report on the experimental method and results of characterizing compliance and outflow resistance in human cadaveric kidneys. METHODS: To characterize compliance a catheter is inserted retrograde into the renal collecting system and sealed at the UPJ. Pressurized irrigat

Complex case of ureteropelvic junction obstruction with bifid renal pelvis in ectopic kidney: A rare combination.

INTRODUCTION: Renal pelvis bifidity, ectopic pelvic kidney, and ureteropelvic junction obstruction are rare urinary tract anomalies resulting from embryological developmental variations. Their coexistence in one kidney is exceedingly uncommon. CASE PRESENTATION: An 18-year-old male with no prior medical history presented with right-sided lumbar pain. A CT scan revealed bilateral hydronephrosis, with the left kidney being malrotated and ectopically positioned in the pelvis. Dynamic renal scintigraphy confirmed bilateral ureteropelvic junction obstruction. Surgical management involved laparosco

[IgG4-related kidney disease: a long-term follow up case of pseudotumor of the renal pelvis].

A 69-year-old man had undergone left ureteronephrectomy because of a left renal pelvic tumor, however the pathological diagnosis was inflammatory pseudotumor. About 1 year later, computed tomography showed a mass at the right kidney near the hilar. Ureterorenoscopy and urine cytology were performed, and their results showed no evidence of malignancy. He had been followed closely without therapy. The mass increased in size during follow-up, and we reviewed the surgical specimen of the left ureteronephrectomy. Immunohistochemical studies revealed diffuse infiltration by IgG4 positive plasma cell

Fused Supernumerary Kidney with Single Pelvis and Ureter; Presenting with Stones: A Case Report and Literature Review.

INTRODUCTION: A supernumerary kidney is an extremely rare congenital anomaly, defined as the presence of one or more extra kidneys. Thus far, there have been less than 100 cases reported in the medical literature. It has its capsule, vascular supply, and collecting system. It frequently causes diagnostic challenges in clinical practice. The supernumerary kidney can be symptomatic due to the presence of stone. CASE PRESENTATION: We present a case of a 19-year-old male patient who came to our clinic with the complaint of abdominal pain. On computed tomography urography (CTU), he was found to ha

Pelvi-ureteric junction obstruction in autosomal-dominant polycystic kidney disease: an association yet to be reported.

Autosomal-dominant polycystic kidney disease (ADPKD) is the most common inherited renal cystic disease. It is characterised by the development of renal parenchymal cysts and a variety of other extrarenal manifestations. Pelvi-ureteric junction (PUJ) obstruction has not been described in association with ADPKD in the literature. We present a case of a 23-year-old man presenting with bilateral flank pain. On evaluation he was diagnosed to have ADPKD with bilateral renal calculi and left-sided PUJ obstruction. He underwent successful right percutaneous nephrolithotomy and left laparoscopic dismem

Characterizing changes in kidney and renal pelvis cancer incidence from 1998 to 2006 in the United States.

In this study, we characterize the changes in kidney and renal pelvis cancer (RCC) from 1998 to 2006 in the United States. The goal is to examine variations in stage and treatments. In addition, we explore changes in risk factors that have occurred over the same period. Data on over 20,000 patients diagnosed with RCC in National Cancer Data Base were used for the study. We investigated the changes in stage and treatment rates based on race, gender and age at diagnosis from 1998 to 2006. Changes in smoking, obesity and hypertension rates over the period were examined for comparison. The increas

[Evaluation of the contractile capacity of the ureter and kidney pelvis in tuberculosis by the method of infusion dynamic uretero- scintigraphy].

The radionuclide method--infusion dynamic ureteroscintigraphy--was used to examine 123 patients with tuberculosis of the urinary organs and 51 patients of the control group. Contractility parameters of various portions of the ureters and pelves in varying urinary tuberculosis were established. The contractility of the pelves and ureters was proved to be impaired even prior to the appearance of visible X-ray changes in the ureters. A specific increase in contractility in the area from the pelvis to the middle third of the ureter and further reduction in the lower third were seen.

Instillation of bacillus Calmette-Guerin into the renal pelvis of a solitary kidney for the treatment of transitional cell carcinoma.

Intravesical bacillus Calmette-Guerin is effective in the treatment and prophylaxis of superficial urothelial cancer of the bladder. There have been few reports of its efficacy and toxicity when instilled into the upper urinary tract. We elected to use intracavitary bacillus Calmette-Guerin in a patient with recurrent high grade transitional cell carcinoma in the renal pelvis of a solitary autotransplanted kidney. The patient required hospitalization and triple-drug therapy after 5 instillations. She was free of tumor 1 year after bacillus Calmette-Guerin instillations.

[Transposition of a lower pole kidney pedicle to the anterior side of the pelvis in the treatment of certain types of hydronephrosis].

Reviewing 8 cases, the author describes the technique and gives the results from surgical treatment of certain types of hydronephrosis involving a transposition of the lower pole vascular pedicle into the anterior side of the pelvis, preventing thus the former from compressing the pyeloureteral junction. The author underlines the difficulty residing in the proper selection of the surgical indications. Success depends upon the quality of such indications. This method characteristically combines simplicity and reliability.

Laparoscopic Nephroureterectomy with Heminephrectomy for Urothelial Carcinoma of the Upper Renal Pelvis on the Left Side of the Horseshoe Kidney.

A 70-year-old male was diagnosed with urothelial carcinoma of the upper renal pelvis on the left side of the horseshoe kidney. Preoperative thin-slice contrast-enhanced CT with three-dimensional reconstruction of the images revealed that two arteries arising from the aorta supplied the left moiety of the horseshoe kidney. He underwent laparoscopic transperitoneal nephroureterectomy with heminephrectomy on the left side of the horseshoe kidney visualized by indocyanine green fluorescence system. The histopathological findings of the renal pelvic tumor revealed invasive urothelial carcinoma with
