BACKGROUND AND PURPOSE: The heterogeneous outcomes used in lower-limb lengthening surgery (LLLS) complicate evidence synthesis, weakening systematic reviews and clinical recommendations, and reducing research impact. This scoping review maps the outcomes and outcome measurement instruments (OMIs) used in LLLS.
METHODS: This pre-registered review systematically identified studies reporting outcomes in adults or children who underwent LLLS. Outcomes and OMIs were extracted verbatim, and experts grouped outcome terms under headings using the COMET taxonomy.

CASE: A 20-year-old man with a history of right lower extremity fibular hemimelia previously treated with PRECICE femoral nail lengthening presented with a broken magnetic nail and a displaced fracture through an ununited distraction osteogenesis site. Using a combination of techniques, we removed the broken implant while maintaining the achieved limb length and preserving the native biology without bone grafting.

Femoral shaft fracture in children is a frequently encountered problem. Residual leg length discrepancy persisting at skeletal maturity is the most common complication. In this study, a positive correlation was found between the amount of bone fragments overlap and the overgrowth phenomenon. When comparing final leg-length discrepancy with initial fragments overlap, 9.3 mm was found to be the maximal overlap to be tolerated in order to avoid significant leg length discrepancy.

We measured discrepancy in the length of the tibiae in cases of unilateral congenital dislocation of the hip (CDH) in 10 patients. The mean shortening of the tibia on the affected side was 1 cm and was not related to the severity of the dislocation. Patients who are about to undergo total hip arthroplasty for CDH should be warned that the operation is unlikely to restore normal leg length.

The authors have presented a preliminary report on the results of elongation of the lower extremities by the bloodless method of slow disruption of the long bones epiphyses based on the observation of 6 cases. The treatment was applied in the children just before bone growth has been finished. Bone rebuilding at the site of elongation was obtained at 4 to 6 months after the start of distraction. In that period, union of the epiphyseal cartilage was present in 5 from 6 treated patients. The authors believe that the presented method of elongation of the lower extremities can be recommended.
