Characteristics and treatment of elemental mercury intoxication: A case series.

BACKGROUND AND AIMS: Elemental mercury toxicity is a rare condition which can be difficult to diagnose due to its nonspecific signs and symptoms. The purpose of this investigation is to describe the presenting characteristics and treatment of adult and pediatric patients with elemental mercury poisoning. METHODS: A retrospective review was performed in six patients with elemental mercury exposure or intoxication who were treated in an outpatient medical toxicology clinic. Clinical signs and symptoms, laboratory assessments, and public health responses were reviewed. RESULTS: Headache, anorex

Methyl mercury exposure at Niigata, Japan: results of neurological examinations of 103 adults.

BACKGROUND: Large-scale poisonings caused by methyl mercury (MeHg) have occurred in Japan (Minamata in the 1950s and Niigata in the 1960s) and Iraq (in the 1970s). The current WHO neurological risk standard for adult exposure (hair level: 50 μg/g) was based partly on evidence from Niigata which did not consider any cases who were diagnosed later and/or exposed to low level of MeHg (hair mercury level less than 50 μg/g). METHODS: Early in the Niigata epidemic in June 1965 there were two extensive surveys. From these two surveys, we examined 103 adults with hair mercury measurement who consulte

Ammoniated mercury ointment: outdated but still in use.

Although severe mercury-associated side effects, such as poisoning and high irritant and allergic potential, as well as the weak potency of mercury as an antiseptic or antipsoriatic agent, are well-known, topical mercury preparations are still listed in several pharmacopoeias, including that of the United States, UK and several other European countries. Thus, topical mercurials are still in use, even though more effective and less toxic drugs are now available. We report 2 cases of severe allergic reaction to ammoniated mercury and hope, in agreement with the pertinent literature, to contribut

Plasma mercury during prolonged acute renal failure after mercuric chloride ingestion.

Serial measurements of plasma mercury were made in a patient with severe and prolonged acute renal failure due to poisoning with mercuric chloride. An initial mercury concentration in whole blood of 1200 micrograms/l (6 mumol/l) was recorded, and recovery of renal function coincided with a fall in plasma mercury concentration to below 100 micrograms/l (0.5 mumol/l). The case demonstrates that survival and recovery of renal function is possible despite very high concentrations of mercury in the blood and oliguric renal failure of nearly six weeks' duration.

Overview of the clinical toxicity of mercury.

Mercury is ubiquitous in the environment and therefore every human being, irrespective of age and location, is exposed to one form of mercury or another. The major source of environmental mercury is natural degassing of the earth's crust, but industrial activities can raise exposure to toxic levels directly or through the use or misuse of the liquid metals or synthesized mercurial compounds. The aim of this review is to survey differences in human exposure and in the toxicology of different forms of mercury. It covers not only symptoms and signs observed in poisoned individuals by a clinician

Mercury and health care.

Mercury is toxic heavy metal. It has many characteristic features. Health care organizations have used mercury in many forms since time immemorial. The main uses of mercury are in dental amalgam, sphygmomanometers, and thermometers. The mercury once released into the environment can remain for a longer period. Both acute and chronic poisoning can be caused by it. Half of the mercury found in the atmosphere is human generated and health care contributes the substantial part to it. The world has awakened to the harmful effects of mercury. The World Health Organization and United Nations Environm

Is shellfish consumption safe?

Louisiana provides nearly 40% of domestic seafood production. America's commercial fisheries, especially coastal shellfish fisheries, now face crippling economic and environmental pressures from seafood imports, over-fishing, urban and agricultural wastewater runoff, harmful algal blooms, and coastal wetlands loss. As a result of these ecosystem stresses, seafood-borne disease now causes 37% of all foodborne illness in the United States. Louisiana and other coastal-state physicians can effectively curtail the rising threat of local shellfish-borne disease outbreaks by supporting responsible co

Strictly-controlled techniques are required in laparoscopic appendectomy for elemental mercury sequestration in the appendix: a case report.

Elemental mercury impaction in the appendix can cause subsequent local and systemic complications. We present a case of a teenage boy who ingested approximately 10 mL of elemental mercury, resulting in residual mercury sequestration in the appendix after conservative management. We performed laparoscopic appendectomy to remove the residual mercury. The patient made a complete clinical recovery without adverse events related to mercury poisoning over the 6-month follow-up. We highlight the advantages of laparoscopic appendectomy, abdominal computed tomography (CT), negative pressure operating r

Inorganic mercury intoxication: A case report.

Mercury is a heavy metal with unique physico-chemical properties, and it is well distributed throughout the environment, being present in soil, water and air. This non-essential element is considered by the World Health Organization (WHO) as one of the ten most troublesome chemical to public health. Its toxicity spectrum depends on the chemical form in which it presents: elemental (metallic), organic or inorganic. The known intoxications are mainly occupational (mining, agriculture, incineration) or related to the use of dental amalgams or the consumption of contaminated fish and shellfish. No

Protective effect of diethyldithiocarbamate on mercury-induced toxicity in kidney mitochondria.

The protective effect of diethyldithiocarbamate on several functions of kidney mitochondria, isolated from rats poisoned with mercury, was analyzed. The drug, administered at a concentration of 10 mg/ kg body weight to rats treated with a sublethal dose of Hg2+, for example, 2 mg/kg body weight, prevented the mercury-induced damage on: a) transmembrane potential, b) ATP synthesis, and c) Ca2+ accumulation. It is demonstrated that such a protective effect of diethyldithiocarbamate is due to its ionophoretic properties on mercury.

Congenital Minamata disease: a description of two cases in Niigata.

Minamata disease or methyl mercury poisoning from industrial pollution was first described from Minamata, Japan in the 1950s. Subsequently, a similar poisoning episode occurred at Niigata, Japan in the 1960s. This paper describes the Minamata event and then presents two case reports believed to be prenatal poisoning from consumption of contaminated fish at Niigata. Case number one is of special interest because it is the only subject with congenital Minamata disease for whom exposure was actually measured near the time of birth.

Minimal change disease following exposure to mercury-containing skin lightening cream.

A 34-year-old woman developed nephrotic syndrome after using a skin lightening cream that contained an extremely high level of mercury. Blood and urine mercury levels were elevated and a renal biopsy revealed minimal change disease. Membranous nephropathy was excluded using immunofluorescence and electron microscopy. Her proteinuria remitted 9 months after she stopped using the cosmetic cream. This is the first reported case in the English literature of proven minimal change disease secondary to mercury exposure. It is important that mercury poisoning due to cosmetic cream is considered in the

Mercury exposure and children's health.

Acute or chronic mercury exposure can cause adverse effects during any period of development. Mercury is a highly toxic element; there is no known safe level of exposure. Ideally, neither children nor adults should have any mercury in their bodies because it provides no physiological benefit. Prenatal and postnatal mercury exposures occur frequently in many different ways. Pediatricians, nurses, and other health care providers should understand the scope of mercury exposures and health problems among children and be prepared to handle mercury exposures in medical practice. Prevention is the ke

Recent advances in evaluation of health effects on mercury with special reference to methylmercury-A minireview.

Mercury is a metal that has long been used because of its many advantages from the physical and chemical points of view. However, mercury is very toxic to many life forms, including humans, and mercury poisoning has repeatedly been reported. The main chemical forms of mercury are elemental mercury (Hg(0)), divalent mercury (Hg(2+)) and methylmercury (CH(3)-Hg(+)), the toxicities and metabolisms of which differ from each other. Methylmercury is converted from divalent mercury and is a well-known neurotoxicant, having been identified as the cause of Minamata disease. It bioaccumulates in the env

Presentation of a general algorithm to include effect assessment on secondary poisoning in the derivation of environmental quality criteria. Part 1. Aquatic food chains.

Effect assessment on secondary poisoning can be an asset to effect assessments on direct poisoning in setting quality criteria for the environment. This study presents an algorithm for effect assessment on secondary poisoning. The water-fish-fish-eating bird or mammal pathway was analyzed as an example of a secondary poisoning pathway. Parameters used in this algorithm are the bioconcentration factor for fish (BCF) and the no-observed-effect concentration for the group of fish-eating birds and mammals (NOECfish-eater). For the derivation of reliable BCFs preference is given to the use of exper

Molecular mechanisms triggered by mercury.

Mercury is an ubiquitous environmental toxin that causes a wide range of adverse health effects in humans. Three forms of mercury exist: elemental, inorganic and organic. Each of them has its own profile of toxicity. Exposure to mercury typically occurs by inhalation or ingestion. Mercury can be an indoor air pollutant, however industry emission remains the most important source of inhaled mercury. Furthermore, fresh water and ocean fish may contain large amounts of mercury and dental amalgam can be another important source of inorganic and mercury vapor. The present review discusses the curre
