Diagnostic potential of OCTA for early detection of diabetic retinopathy: A literature review.

BackgroundEarly detection of diabetic retinopathy (DR) is crucial for preventing blindness. Optical coherence tomography angiography (OCTA) offers a non-invasive method for visualizing retinal microvasculature and may facilitate earlier detection of DR. This systematic review evaluates the current evidence regarding OCTA-derived biomarkers for the identification of early DR.MethodsA systematic review of the literature was performed. Due to substantial heterogeneity in study designs, OCTA protocols, and outcome measures, a meta-analysis was not feasible.ResultsTwenty-one studies were included.

Laser photocoagulation of a hypofluorescent perifoveal exudative vascular anomalous complex/capillary macroaneurysm.

PURPOSE: To report successful treatment of a hypofluorescent perifoveal exudative vascular anomalous complex (PEVAC)/capillary macroaneurysm. CASE DESCRIPTION: A 63 year old healthy gentleman had a perifoveal isolated aneurysmal lesion with white rim. Optical coherence tomography (OCT) showed cystoid macular edema (CME) with neurosensory detachment. The aneurysmal lesion was seen in the inner retina as a hyperreflective intraretinal structure with a heterogenous lumen suggestive of PEVAC/capillary macroaneurysm. OCT angiography showed a capillary loop and a slightly hyperreflective lesion at

Comparison of Optical Coherence Tomography Angiography and Fluorescein Angiography for the Identification of Retinal Vascular Changes in Eyes With Diabetic Macular Edema.

BACKGROUND AND OBJECTIVE: To compare the visualization of microaneurysms (MA) and the foveal avascular zone (FAZ) area using optical coherence tomography angiography (OCTA) versus fluorescein angiography (FA) in patients with diabetic macular edema (DME). PATIENTS AND METHODS: Patients were prospectively recruited for same-day imaging on spectral-domain OCTA and FA. OCTA images were automatically segmented into superficial (sOCTA) and deep (dOCTA) capillary plexuses. The number of visible MAs and the FAZ area were compared between the two imaging modalities. RESULTS: Nineteen eyes of 10 pati

Pericytes and the pathogenesis of diabetic retinopathy.

Pericytes provide vascular stability and control endothelial proliferation. Pericyte loss, microaneurysms, and acellular capillaries are characteristic for the diabetic retina. Platelet-derived growth factor (PDGF)-B is involved in pericyte recruitment, and brain capillaries of mice with a genetic ablation of PDGF-B show pericyte loss and microaneurysms. We investigated the role of capillary coverage with pericytes in early diabetic retinopathy and the contribution to proliferative retinopathy using mice with a single functional allele of PDGF-B (PDGF-B(+/-) mice). As assessed by quantitative

Active Learning Under Expert-Budget Constraints: A Human-in-the-Loop Pipeline for Diabetic Retinopathy Lesion Detection.

Early diagnosis of Diabetic Retinopathy (DR) is critical for preventing irreversible vision loss, but precise lesion annotation by ophthalmologists is the dominant cost in building any clinical-grade DR detection model. The structural problem in real hospital settings is not labeling cost per se, but expert availability: ophthalmologists' time is bounded by clinical duties, so the active-learning (AL) cycle can iterate only a handful of times in practice. We frame this constraint explicitly and ask which AL designs work best under a tight expert budget. We propose Virtuous Cycle, a Human-in-th

Polyarteritis nodosa presenting with splenic infarction, bilateral renal infarction, and hematoma.

Polyarteritis nodosa (PAN) is a necrotizing vasculitis and may affect multiple organ systems. Due to the rupture of microaneurysms, perirenal hematomas may occur in both kidneys. Spontaneous perirenal hematoma is an infrequent complication of isolated PAN. Also, involvement of the splenic arteries is a rare occurrence in polyarteritis nodosa. Perirenal hematoma rupture is a serious clinical condition. Patients with life-threatening visceral bleeding must be treated with a selective embolization procedure or radical surgery because nonoperative management has been associated with a significant

Epidemiological study of the systemic ophthalmological effects of carbon disulfide.

A total of 123 male viscose rayon workers who were exposed to carbon disulfide, and an additional 67 workers who were not exposed to any toxic agent in the working environment, underwent a thorough ophthalmological examination. The relationship between exposure and ophthalmological results was analyzed with univariate and multivariate methods. The most striking findings were strong associations between exposure and the 100-HUE color vision score and excess of microaneurysms in the exposed group. The current threshold limit value appeared to protect against these effects.

Acquired retinal arteriovenous communications in occlusive disease of the carotid artery.

Retinal arteriovenous communications proximal to extensive areas of complete vascular closure developed in three patients with occlusive disease of the carotid arteries. In one case, the authors document the development of the arteriovenous communications through successive enlargement of small retinal vessels after progressive signs of retinal ischemia in the form of narrowed retinal arteries, venous dilation and beading, microaneurysms, retinal dot and blot hemorrhages, cotton-wool spots, and capillary nonperfusion. The development of arteriovenous communications in carotid occlusive disease

[Malabsorption caused by HBsAg-positive panareteritis nodosa].

A 36 year old man with panarteriitis nodosa (PAN) presented over a certain period of time meinly with gastrointestinal symptoms. He was HBsAg positive and a "reactive" hepatitis with histologically demonstrated. Arterial microaneurysms and stenoses were detected by angiography in the liver, kidney and small intestine. These findings were confirmed by autopsy. Vascular occlusions had caused infarctions of the small intestine as well as necroses and ulcerations of the large bowel. Chronic ischemia is considered as cause of the patient's malabsorption-syndrome. It is suggested, that cachexia of P

Macro-aneurysm in the basal ganglia region.

A 59-year-old female patient suddenly developed vomiting and gait disturbances followed by decreasing consciousness. CT scans revealed a hemorrhage within the left basal ganglia region with rupture into the ventricles and consecutive hydrocephalus. On angiography an aneurysm in the region of the caput nuclei caudati was shown to be the source of the bleeding. On repeat-angiography 4 months later the aneurysm was no longer visualized, probably due to thrombosis. This is an extraordinary case of a basal ganglia aneurysm comparable with the aneurysms of Willis' circle, but located in a region whe

[Causes and mechanisms of systemic vasculitides].

Multiple mechanisms contribute to the pathogenesis of systemic vasculitides: 1. vasculitides resulting from the deposition of circulating immune complexes, comprising polyarteritis nodosa associated with hepatitis B virus infection, cryoglobulinemia associated systemic with vasculitides, mainly the consequence of hepatitis C virus infection, and Schonlein Henoch purpura, which results from the deposition in the mesangium and vessels of IgA forming complexes; 2. vasculitides associated with anti-neutrophil cytoplasm antibodies (ANCA), comprising Wegener's granulomatosis associated with anti-pro

Histopathology of the brain vascular network in moyamoya disease.

There is an unusual vascular network at the base of the brain in patients with moyamoya disease. We detected various histologic lesions in the perforating arteries of 22 patients. Vessels showing rupture ranged from 50 to 530 microns in diameter; they were dilated, some had fibrin deposits in the wall, fragmented elastic laminae and attenuated media. Non-ruptured perforating arteries (diameter 200 to 550 microns) revealed microaneurysm formation, focal fibrin deposits and marked attenuation of the wall thickness with diminution of the elastic lamina. These changes seem to predispose to rupture

Polyangiitis overlap syndrome: cutaneous leukocytoclastic vasculitis associated with polyarteritis nodosa.

A rare case of polyangiitis overlap syndrome is described. The patient was a 25-year-old man who had palpable purpura on his legs which showed leukocytoclastic vasculitis, and polyarteritis nodosa. Superior mesenteric arteriography showed microaneurysms in jejunal branches with focal segmental necrotizing arteritis of small and medium sized muscular arteries in the jejunum. Deposits of IgA and C3 in the superficial blood vessels of the lesional skin were consistent with the features of Henoch-Schönlein purpura. The patient died about two months after initial admission in spite of cytotoxic age
