Current Evidence on the Use of Cystoscopy in the Follow-up of Non-Muscle-Invasive Bladder Cancer: Are We Overusing It? Systematic Review of the Literature.

BACKGROUND: Non-muscle-invasive bladder cancer (NMIBC) accounts for the majority of bladder cancers. Its follow-up entails high costs and significant impact on quality of life. Cystoscopy is the standard method, although uncertainties remain regarding the optimal frequency and duration. The aim was to review the available evidence on surveillance schedules and their impact on recurrence and progression. METHODS: A systematic review was conducted following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta- Analyses). Searches in PubMed, Web of Science and Scopus included clinic

Advances in intravesical therapy for non-muscle-invasive bladder cancer.

Non-muscle-invasive bladder cancer (NMIBC) accounts for roughly 75% of newly diagnosed bladder cancer and associates with high risk of recurrence and progression. Intravesical therapy remains the cornerstone of NMIBC management following transurethral resection of bladder tumor (TURBT). For low-risk NMIBC, immediate single instillation of chemotherapy after TURBT effectively reduces recurrence. Intermediate-risk patients could benefit from either intravesical chemotherapy or Bacillus Calmette-Guérin (BCG) instillation, with emerging therapies such as UGN-102 chemoablation showing promise. Management of BCG failure in non-muscle-invasive bladder cancer the present and the future.

Bladder neoplasms are still among the most common cancer types in the Czech Republic. Even though the majority (more than 90%) of bladder tumours are of urothelial origin, the group is vastly heterogeneous in terms of biological behaviour and thus also progression of the disease. Adequate adjuvant treatment is the cornerstone of the therapy in high-risk patients for disease recurrence, particularly those with a high risk of progression to a muscle-invasive disease (T2 and higher). Intravesical BCG immunotherapy still remains such a therapy. It is a standard therapy with well-established effica

Simultaneous surgery for NMIBC and BOO: evaluating surgical safety to oncological benefit.

BACKGROUND: Current evidence demonstrates that simultaneous surgical management of bladder cancer (BCa) and bladder outlet obstruction (BOO) is oncologically safe, despite well-known concerns. This systematic review and meta-analysis primarily evaluates oncological outcomes of concurrent transurethral resection of bladder tumor (TURBT) and BOO surgery compared to TURBT alone or staged treatments; perioperative safety is descriptively summarized. METHODS: A systematic search of PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials was conducted from January 2000 to Mar

Management Dilemma for Very High-risk Non-muscle-invasive Bladder Cancer: Real-World Data Challenge the Guideline Recommendation for Upfront Radical Cystectomy.

We discuss real-world evidence that challenges the guideline recommendation of upfront radical cystectomy for all patients with very high-risk non-muscle-invasive bladder cancer. The evidence highlights the need for more accurate and dynamic tools to guide clinical decision-making. Until these are available, treatment decisions should be individualized and made in close consultation with the patient.

Molecular characterization of non-muscle invasive tumor from N-methyl-N-nitrosourea (MNU)-induced urothelial carcinoma in a rat model reveals hallmarks of both luminal and basal subtypes.

With the advances in molecular classification of bladder cancer subtypes, pre-clinical rodent models used to study urothelial tumorigenesis must be profiled to reflect clinical relevance and optimize translational research strategies. Murine models of bladder cancer such as the autochthonous N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN) model and genetically-engineered Upk3a-CreERT2; Trp53L/L; PtenL/L; Rosa26LSL-Luc (UPPL) model are muscle invasive models which have been found to correlate with basal and luminal subtypes, respectively. The rat MNU model has been used to represent non-muscle inv

Epidemiology and Risk Factors for Non-Muscle-Invasive Bladder Cancer: Insights into Prevalence, Risk Stratification, and Implications for Early Diagnosis.

Bladder cancer is the 6thth most common type of cancer in the United States and tobacco smoke remains the leading environmental risk factor of the disease. Non-muscle invasive bladder cancer (NMIBC) makes up the majority of bladder cancer cases and is a subtype of bladder cancer that has a high heterogenicity in rates of recurrence and prognosis. The aim of this article is to dive into the prevalence of NMIBC, review the literature and guidelines on how NMIBC is risk stratified, and discuss the utility of early diagnosis of this disease and ways this can be achieved.

BCG-Unresponsive Non-Muscle-Invasive Bladder Cancer: Current Treatment Landscape and Novel Emerging Molecular Targets.

Urothelial carcinoma (UC), the sixth most common cancer in Western countries, includes upper tract urothelial carcinoma (UTUC) and bladder carcinoma (BC) as the most common cancers among UCs (90-95%). BC is the most common cancer and can be a highly heterogeneous disease, including both non-muscle-invasive (NMIBC) and muscle-invasive (MIBC) forms with different oncologic outcomes. Approximately 80% of new BC diagnoses are classified as NMIBC after the initial transurethral resection of the bladder tumor (TURBt). In this setting, intravesical instillation of Bacillus Calmette-Guerin (BCG) is th
