OBJECTIVE: To provide experience in using the buccal musculomucosal reversed flow arterial island flap in oral, nasal and orbital mucosal defects repair.
METHODS: Six patients were operated by using the method. This flap supplied by the distal portion of the facial artery through the anterior buccal branches can be passed through the tunnel of the subcutaneous plane to repair the orbital mucosal defect or the nasal mucosal defect or septal perforation, or through the alveolar ridge cleft to close alveolar ridge cleft and oroantral fistula, or the cleft of the anterior hard palate or palatal f

A review of records of 27 patients with duodenal fistulas admitted to St. Joseph's Health Centre in Toronto since 1969, when total parenteral nutrition (TPN) was instituted, showed that in 19 patients the fistula formed after gastric resection, pyloroplasty or transduodenal sphincteroplasty. The remaining fistulas resulted from delayed presentation of perforated duodenal ulcers, trauma suffered in motor vehicle accidents and disease in neighbouring organs. Management included early nasogastric suctioning, withholding oral intake, draining the fistula contents, protecting the skin effectively,

BACKGROUND: Complications after bariatric surgery often require longterm parenteral nutrition to achieve healing. Recently, endoscopic treatments have become available that provide healing while allowing for oral nutrition. The purpose of this study was to present outcomes of the largest series to date treating staple line complications after bariatric surgery with endoscopic covered stents.
STUDY DESIGN: A retrospective evaluation was performed of all patients treated for staple line complications after bariatric surgery at a single tertiary care bariatric center. Acute postoperative leaks,

Perianal abscess and fistula-in-ano are well-described in the pediatric population. They are most common in infants less than 1 year of age and often resolve with oral antibiotics; occasionally they require drainage or fistulotomy. The etiology is commonly associated with cryptoglandular obstruction and subsequent infection, however alternative diagnoses should be considered in cases of recurrent abscesses and fistulae that are refractory to standard treatments. In this report, we present the case of an 8-year-old boy with a complex, recurrent fistula-in-ano that resulted from a rare congenita

BACKGROUND: Oesophageal fistula is a rare complication of catheter ablation of atrial fibrillation with most fistulas being atrio-oesophageal fistulas, but oesophageal-pericardial fistula can also happen in the absence of atrial perforation.
CASE SUMMARY: A 68-year-old male patient presented with chest pain 10 days after catheter ablation of paroxysmal atrial fibrillation. He was discharged after an initial negative workup that included a CT chest without contrast. He later presented again with severe chest pain and fever and was found to have an oesophageal-pericardial fistula. He underwent

When treating patients with Crohn's disease (CD), external or internal fistulas connected to the intestine were encountered, and these were often difficult to diagnose correctly. In order to diagnose such fine fistulas, we modified a method of using indocyanine green (ICG) administration to the gut lumen. The subjects of this study comprised nine patients with CD who had an obscure fistula which could not be demonstrated by barium study, fistulography, or by urological or gynecological examinations. If the administrated ICG was detected by various ways from an external fistula or from the neig

There are multiple management options for treatment of iatrogenic salivary fistulas including reduced oral intake, pressure dressings, total parotidectomy, tympanic neurectomy, surgical repair, radiation therapy, and pharmacotherapy. However, the optimal management of salivary fistulas is unclear due to uncertain efficacy and adverse outcomes. We present a case of a neonate that developed a submandibular fistula following removal of mandibular distractors and was ultimately successfully managed using intralesional botulinum toxin injection. The purpose of this communication is to summarize the

BACKGROUND: Recurrent cleft palate fistula is a challenge for maxillofacial surgeons. In severe cases, microvascular tissue transfer is necessary to treat these patients successfully.
METHODS: A 23-year-old man with cleft lip and palate, previously treated elsewhere, presented with a 2-cm defect in the posterior hard palate. A tongue flap had already been performed. It was decided to close the persistent fistula with a lateral upper-arm flap. In a first-step operation, a free graft of buccal mucosa was fixed to the lateral upper arm subcutaneously and covered with an alloplastic sheet. After

A 14-year-old female presented with extraoral inverted eruption of left mandibular permanent molars 18 and 19 at the lower left inferior border of the mandible. Both the teeth started erupting 1 year after an extraoral surgical intervention for a discharging sinus 6 years ago. The subsequent eruption to the extraoral position of the permanent molars at the inferior border of mandible may be the result of the previous surgical procedure or pathology related to the abnormally positioned teeth. This case presents an infrequent complication affecting the adjacent permanent teeth.

Oroantral fistula (OAF) refers to a permanent connection between oral cavity and maxillary sinus. The extraction of maxillary posterior teeth is the most common reason of OAF. Multiple techniques are available to repair this defect. The most used procedures involving local flaps include buccal flap, buccal fat pad (BFP), and palatal rotating flap. We here present an original technique of management of oroantral fistula with displacement of the root into sinus using BFP.

A pedicled musculo-mucocutaneous "flap" derived from an original tongue flap is used as one of the layers for the double-layered closure of a recurring oro-nasal fistula. The other layer used for the closure, namely the nasal "flap" layer, originates from the adjacent palatal mucoperiosteum. The oral pedicled "flap" layer and the nasal "flap" layer form the final double-layered flap. This technique gives satisfactory closure of the persistent oro-nasal fistula.

SUMMARY: Large or multiply recurrent oronasal fistulas following cleft palate repair present a challenging problem. Nasal septal mucoperichondrial flaps have been widely used for repair of skull base defects; however, their use in the repair of oronasal cleft palate fistulas has not previously been described. In this pilot study, the authors describe anterior palatal fistula repair using a nasal septal flap and review their experience with this technique over 4 years. Fourteen patients with anterior palatal fistulas not amenable to repair using local palatal flaps were included for analysis. T

In the last 50 years cocaine usage has had a growing incidence worldwide. Chronic nasal abuse of this drug can cause extensive oromaxillofacial destructive process as oronasal perforation. To author's knowledge the only type of oronasal fistula in cocaine abusers that has been reported to date has been perforation of the hard palate; a case of oronasal fistulas involving another region of oral cavity, the anterior gingival sulcus, has been reported only once. In this article authors present the first case of oronasal fistula involving hard palate and the right supero-lateral vestible induced b

An extremely rare case of congenital sinus of the upper lip is presented in a 22-year-old woman with idiopathic precocious puberty. The sinus has a pin-like orifice located in the midline of the upper lip philtum just below the base of the columella. Sexual precocity was found out when she was 7 years old. A series of exact examinations had been performed at that time. Neither neurogenic lesions of the central nervous system nor other systemic abnormalities were observed. Oral Diseases

Oroantral fistula (OAF) is a pathological connection between the maxillary sinus and oral cavity, and it usually occurs after tooth extraction. Proper repair of OAF is important for the prevention of recurrences. The present case offers a technique of OAF repair using septal cartilage and buccal fat pad for the repair of the defect after excision of fistula tract. This double-layered closure is a new technique that could be used to repair chronic OAF.

Total body pool and intestinal absorption of ascorbic acid were studied in 12 patients undergoing operation for Crohn's disease (six with fistulae and six without) and in six control patients undergoing operation for reasons other than Crohn's disease. L-[carboxyl-14C]Ascorbic acid, 0.19-0.40 megabecquerels (MBq), was given orally. After a period of equilibration, the labeled ascorbic acid was flushed out of the patient's body tissues using large doses of unlabeled ascorbic acid. Intestinal absorption of ascorbic acid, assessed from the total cumulative urinary 14C recovery, was found to be si

Complete fistulas of the second branchial cleft have a complicated course between the internal and external carotid arteries, crossing the hypoglossal and glossopharyngeal nerve. Therefore, surgical excision implies a tedious dissection of the fistula between the neck vessels and nerves and the multiple or large incisions have major cosmetic consequences. In cases of complete fistulas, we report a more simple method by stripping. Using a guide wire fixed to the first 2cm of the fistula, the fistulous tract is stripped completely inside out to its pharyngeal opening by pulling the oral end of t
