A Type IV Tibial Tuberosity Avulsion Fracture in an Adolescent - A Case Report.

INTRODUCTION: Tibial tuberosity avulsion fractures in adolescents are one of the rare physeal injuries. Sudden powerful contraction of quadriceps muscle leads to avulsion fractures and varying degrees of disruption of extensor mechanism. CASE REPORT: A 16-year-old boy sustained tibial tuberosity avulsion fracture while playing football and presented with sudden onset of pain, massive swelling, and loss of active extension of the left knee along with inability to bear weight on the left leg. Due to imminent risk of development of compartment, patient was taken for surgical correction immediate

Load-Bearing Capacity of All-Ceramic Fixed Partial Dentures-Do In Vitro Tests Overestimate Strength?

PURPOSE: To assess the impact of the testing model on the load-bearing capacity of monolithic zirconia three-unit fixed dental prostheses (FDPs) and to discuss its implications for transferring in vitro results to clinical reality. MATERIALS AND METHODS: In total, 32 CAD-designed three-unit FDPs milled from zirconia were resin-bonded to four different types of die models (n = 8 each): PM (models made from PMMA); FR (fiberglass-reinforced polymer models), NP (CoCr-alloy models), and TM (tooth mobility simulation models). Load-to-failure testing was performed, and data were analyzed (P < .05).

Comparison of weight-bearing CT arthrography with MRI to evaluate knee meniscal tears.

BACKGROUND: Magnetic Resonance Imaging (MRI) is considered the imaging modality of choice for identifying meniscal pathology. However, it can be challenging to differentiate intrameniscal signal changes from tears. Given that meniscal tears may be more conspicuous when loaded, weight-bearing imaging may better visualize tears. We hypothesized that weight-bearing CT arthrography (WBCTa) would demonstrate meniscal pathology not visualized on MRI. METHODS: This cross-sectional, observational study compared WBCTa with MRI to evaluate rates of detection of meniscal pathology. Sixty-five subjects (

Influence of self-perceived disability on squatting kinematics in individuals with patellofemoral pain.

BACKGROUND: Individuals with patellofemoral pain have a heterogeneous presentation of symptoms during functional tasks; however, biomechanical data often negates self-reported disability. The single leg squat is common in patellofemoral pain literature but may not be a pain provoking task for all individuals. Therefore, our study examined the influence of self-perceived squatting disability in individuals with patellofemoral pain on lower extremity squatting kinematics. METHODS: We analyzed two-dimensional hip frontal plane projection angle, knee frontal plane projection angle, lateral trunk

Free temporoparietal fascia flap in reconstruction of the lower extremity.

Twenty-one patients with open wounds of the non-weight-bearing foot or the ankle underwent coverage with a free temporoparietal fascia flap and split-thickness skin graft. Our indications for this thin, well-vascularized flap included osteomyelitis and exposed tendon, bone, or fixation hardware. The pliability of the flap allowed superior restoration of natural anatomical contours. Only one flap was completely lost (95.2% overall success). There were four cases (20%) of partial flap loss, none of which required flap revision or a new flap. Four patients (19%) experienced transient donor-site a

Cedell's Fracture of the Talus Managed with Herbert Screw Fixation through Posteromedial Approach - A Case Report.

INTRODUCTION: Cedell's fracture is an uncommon avulsion of the posteromedial process of the talus that may appear normal on radiographs. Early identification and anatomical fixation are essential to prevent long-term dysfunction. CASE REPORT: AA 35-year-old male resident doctor sustained a twisting ankle injury after a fall from stairs. Radiographs were inconclusive, but computed tomography (CT) revealed a displaced posteromedial process (Cedell's) fracture of the talus. The patient underwent open reduction and internal fixation through a posteromedial approach using two 3.0 mm Herbert screws

Biphasic plating improves the mechanical performance of locked plating for distal femur fractures.

Internal fixation by plate osteosynthesis is the gold standard treatment for distal femur fractures. Despite improvements that preserve the biological conditions for bone healing, there are concerns standard locked plating constructs may be overly stiff. Biphasic plating is a novel concept designed to provide suitable fracture motion and increased implant strength to support early full weight-bearing. This study aims to demonstrate that the Biphasic Plate can be incorporated into a pre-contoured distal femur plate while providing adequate flexibility and increased implant strength. The mechani

Atraumatic Subtrochanteric Stress Fracture in a 45-Year-Old Housewife: Diagnostic Challenge and Management Strategy.

INTRODUCTION: Subtrochanteric stress fractures are rare and often underdiagnosed due to their subtle clinical presentation and inconspicuous early radiological findings. While these fractures are frequently seen in elderly osteoporotic individuals or those on bisphosphonate therapy, they are uncommon in younger, premenopausal women without significant risk factors. This case highlights a rare instance of a non-displaced subtrochanteric stress fracture in a 45-year-old woman with osteopenia. CASE REPORT: A 45-year-old housewife presented with a progressively worsening, insidious pain in the an

Simultaneous Osteosynthesis and Distal Femoral Osteotomy in a Patient With Distal Femoral Fracture Combined With Valgus Knee Deformity: A Case Report.

Distal femoral fractures are commonly treated with osteosynthesis. However, in older patients with osteoarthritis of the knee, acute primary total knee arthroplasty (TKA) may be performed to treat these fractures. Notably, no studies have documented the use of osteosynthesis in combination with distal femoral osteotomy (DFO) for treating distal femoral fractures in patients with knee osteoarthritis. This report presents the case of a 66-year-old woman with lateral compartment osteoarthritis accompanied by severe valgus knee deformity who underwent osteosynthesis for a distal femoral fracture c

A triplanar osteotomy technique in arthroscopy-assisted ankle mosaicplasty.

PURPOSE: Osteochondral lesions of the talus are lesions that are seen particularly in the young age group and are often related to sports injuries and trauma. These lesions, which show late symptoms radiologically, can be determined in the early stages with magnetic resonance imaging. The aim of this study was to present a new osteotomy technique to reduce the complications of mosaicplasty surgery to a minimum and provide an early return to work. METHODS: A total of 11 patients who had cartilage lesions due to osteochondritis dissecans in the medial aspect of the talus underwent mosaicplasty
